ABSTRACT

OBJECTIVE: To identify recommended components for adopting, implementing and enforcing bans or restrictions targeting flavoured tobacco products. METHODS: Between April and June 2019, semistructured interviews were conducted with 17 high-level experts across the USA and Canada with expertise in flavoured tobacco product policies. Participants included health department staff, researchers, legal professionals and local government officials. Interviews were recorded, transcribed and analysed for key themes. RESULTS: Major findings were organised into four categories: programme planning and legislative preparations; education and community outreach; implementation and enforcement; and policy impact. Critical pre-implementation elements included using comprehensive policy language, identifying enforcement agents, examining potential economic costs, deploying media campaigns and engaging community partners and retailers. Recommended implementation processes included a 6-month preparation timeline, focus on retailer education and clearly outlined enforcement procedures, particularly for concept flavours. CONCLUSIONS: Flavoured tobacco policies have successfully limited sales, withstood legal challenges and become more comprehensive over time, providing useful lessons to inform ongoing and future legislative and programmatic efforts. Identifying and sharing best practices can improve passage, implementation, efficacy and evaluation of flavoured tobacco policies.

ABSTRACT

INTRODUCTION: On 1 January 2016, Hawaii raised the minimum legal age for tobacco access from 18 to 21 years ('Tobacco 21 (T21)') statewide, with no special population exemptions. We assessed the impact of Hawaii's T21 policy on sales of cigarettes and large cigars/cigarillos in civilian food stores, including menthol/flavoured product sales share. METHODS: Cigarette and large cigar/cigarillo sales and menthol/flavoured sales share were assessed in Hawaii, California (implemented T21 in June 2016 with a military exemption), and the US mainland using the only Nielsen data consistently available for each geographical area. Approximate monthly sales data from large-scale food stores with sales greater than US$2 million/year covered June 2012 to February 2017. Segmented regression analyses estimated changes in sales from prepolicy to postpolicy implementation periods. RESULTS: Following T21 in Hawaii, average monthly cigarette unit sales dropped significantly (-4.4%, p<0.01) coupled with a significant decrease in menthol market share (-0.8, p<0.01). This combination of effects was not observed in comparison areas. Unit sales of large cigars/cigarillos decreased significantly in each region following T21 implementation. T21 policies in Hawaii and California showed no association with flavoured/menthol cigar sales share, but there was a significant increase in flavoured/menthol cigar sales share in the USA (7.1%, p<0.01) relative to Hawaii's implementation date, suggesting T21 may have attenuated an otherwise upward trend. CONCLUSIONS: As part of a comprehensive approach to prevent or delay tobacco use initiation, T21 laws may help to reduce sales of cigarette and large cigar products most preferred by US youth and young adults.

ABSTRACT

Hawai'i has comprehensive statewide tobacco control policies and was the first US state to raise the minimum age of sale, purchase, and possession of tobacco products to age 21 ("Tobacco 21") in a policy including not just cigarettes, but also electronic smoking devices and other tobacco products. This insights article provides strategic thinking about tobacco control advocacy planning. Specifically, we identify formative factors critical to building and sustaining our cross-sector, statewide advocacy infrastructure that has been able to address many ongoing challenges of tobacco-use prevention and control over time. This can provide new insights for other large-scale tobacco-control advocacy efforts.

ABSTRACT

This cross-sectional study explores the differences in ethnicity, sex, immigration (place of birth of student and parents), and acculturation (based on language spoken at home) on current cigarette smoking among public high school students in Hawai'i, and especially examine if this affected smoking among girls. Previous behavior risk surveys of youth in Hawai'i showed higher smoking rates among girls, although these were not found to be statistically significant differences. Multiple years of data were compiled from the Hawai'i Youth Tobacco Survey (YTS) for years 2005, 2007, 2009, and 2011, for a total sample size of N=5,527. Multivariate logistic regression analysis was conducted to evaluate the likelihood of current cigarette smoking (in the past 30 days) in relation to a variety of factors. The analysis revealed that Hawai'i-specific ethnicity, grade, and sex were all significant predictors of smoking. Girls whose mothers were born in Hawai'i or in another United States state were more likely to smoke than those whose mothers were born in a foreign country. The model showed girls were more likely to smoke than boys. Eleventh and twelfth graders were more likely to smoke than ninth graders. Whites, Filipinos, Native Hawaiians, Pacific Islanders, and Other ethnic groups were more likely to smoke than those who identified themselves as Japanese.
