ABSTRACT

Cannabis access laws allow for the use of cannabis in private and the trade, purchase and use of hemp-related products as a complementary medicine and for other benefits. Cannabidiol (CBD) has the treatment potential for several conditions but, with the lack of resources in South Africa to maintain the legislation, products contaminated with delta-9-tetrahydrocannabinol (Δ9-THC) are sold by some suppliers who do not comply with the legislative provisions in terms of the threshold concentrations for Δ9-THC. This dilemma complicates a medical review officer's decision regarding intentional use of Δ9-THC or otherwise, since a CBD user may have purchased the product legally and in good faith. Hemp- and CBD-containing products were analysed by gas chromatography-mass spectrometry and compliance was assessed for CBD and Δ9-THC purity against the legislative thresholds. A strategy based on metabolite ratios is suggested to distinguish between intentional or irresponsible cannabis use and legitimate CBD use.

ABSTRACT

Acute disseminated encephalomyelitis (ADEM) is an immune-mediated acute inflammatory demyelinating disorder, which typically occurs after viral infections or immunisation. We present a case of a man with acute Rickettsia conorii infection whose diagnosis was delayed. He presented with fever, headache, an eschar and an acute paraplegia. The R. conorii IgM serum titre was 1:128. Magnetic resonance imaging showed multifocal lesions in the brain and spinal cord consistent with inflammatory demyelination. The patient responded well to doxycycline and a short course of high-dose corticosteroids. To our knowledge this is the first case of ADEM associated with Mediterranean spotted fever - we found a previous report of ADEM in a child with Rocky Mountain spotted fever, whose diagnosis of rickettsial infection was also delayed. We hypothesise that delayed diagnosis of spotted fever group rickettsial infections could rarely result in ADEM.

ABSTRACT

BACKGROUND: Due to the migratory flow of infected people with severe acute respiratory syndrome virus (SARS COV-2), the number of confirmed cases of coronavirus disease 2019 (COVID-19) is accelerating globally; preclinical evidence of antiviral agents that can combat this pandemic is still elusive. We identified published articles on SARS-COV efficacy experiments in which some selected compounds were used to test the reduction of the virus load in mice. METHODS: A systematic search of articles was conducted in PubMed, Web of Science, and Scopus. We then developed a combined model based on a systematic review, meta-analyses, and molecular docking studies to evaluate the effect size of preclinical studies of compounds that have been tested against SARS-COV. Because substantial heterogeneity was expected, random effect model meta-analyses were carried out to estimate the overall pooled disease's prevalence. All meta-analyses were performed with Stata version 15.0. Subgroup analyses on therapies were conducted as well. Molecular docking studies of the inhibitors in the active pocket of COVID-19 protease were also performed. RESULTS: From all screened articles, six studies were appropriate for ultimate meta-analysis and systematic review. The residual amount of heterogeneity was high (τ2 =0.02; heterogeneity I2 =85.5 % with heterogeneity chi-square =103.57, a degree of freedom =15, and p <0.001). The overall random pooled prevalence of infected mice treated with the selected compounds was 78.1 % [95 % Confidence Interval (CI): 14.7-17.0 %]. Prophylactic has a significantly higher pooled prevalence than therapeutic, with 21.8 % (95 % CI: 16.4 % to 28.8 %). Our results indicated that most of the SARS-COV inhibitors analyzed were less effective in reducing the lung virus titer of SARS-COV infection in animal models. The findings from molecular docking studies also identified COVID-19 inhibitors that are good for optimization and drug development to fight against COVID-19 infection. CONCLUSIONS: Findings from the review showed that studies on the preclinical compounds targeting SARS-COV and COVID-19 are limited. Furthermore, molecular docking studies and meta-analysis results substantiated three compounds, i.e., EIDD-2801, GS-5734, and amodiaquine. HIPPOKRATIA 2020, 24(3): 99-106.

ABSTRACT

This study was designed to determine the extent to which alluvial and bedrock gold mining activities in the Kette-Batouri region of Eastern Cameroon are responsible for the discharge of potentially harmful elements (PHEs) into natural water bodies. This is considered to be of great public concern, since elevated levels of PHEs in these water bodies, which are widely used for domestic purposes, could have adverse health and environmental effects on the population and nearby ecosystems. The investigation into the levels of PHEs in natural water bodies of this area was conducted to ascertain the toxicity posed by mine waste stockpiles as data from the literature on this subject are scarce. Forty-two water samples from the region were analysed for some 60 PHEs by ICP-OES, of which 22 that were not below the detection limit were considered for further data analysis. Maximum total concentrations in water of As, Cr, Pb, and V and Zn are above the World Health Organization (WHO) maximum allowable concentration (MAC) levels and are as follows (µg l-1): As (21.90-50.9); Cr (1.80-57.30); Pb (0.50-34.70); V (24.70-77.20) and Zn (3.10-481.70). This information is consistent with that of our recent research efforts which have indicated moderate pollution by heavy metals in the soils with a slight deterioration of site quality in this region. The data generated from this investigation is important in the formulation of water management strategies and recommendations for remediation of water bodies at abandoned mine sites for meeting water quality standards.

ABSTRACT

A survey of UK postgraduate centres during September 1997 found that almost 60% were connected to the Internet, but librarians were still the largest user group, few had more than one access point and support to users was limited. Significant progress was made during the following 18 months--by April 1999 all but one of the remaining sites had Internet access in either the postgraduate centre or in a separate library. Lack of funds and decisive policy were stated to be the most common reasons for delay in connecting. The Internet was rated highly as source of the latest research information and support for evidence-based medicine and continuing education. Expansion of the role of librarians to include assistance with sophisticated searches seemed a logical move to the majority, yet the survey suggests that a significant investment in time and money will be required to support the use of the Internet in continuing medical education in the broadest sense. It is likely that the traditional boundaries between IT departments, libraries and user groups will need to be redrawn if the technology is to be exploited effectively.

ABSTRACT

Two thyroidal evaluation systems in euthyroid cats (n = 12) were compared. A single, confirmed hyperthyroid cat was included for interest. Firstly, thyroidal uptake of an intravenous bolus of approximately 111 MBq (3 mCi) 99MTcO4 was estimated by using a scintillation camera and calculating the ratio of thyroid to salivary activities at 20 min and 4 h. Thyroid to salivary activity ratios were 1:1 at 20 min and 2:1 at 4 h. Two discrete areas of salivary uptake were identified, namely a parotid/mandibular complex and a more rostral buccal/sublingual complex. These results were compared to radioiodine uptake of an oral dose of approximately 0.925 MBq (25 microCi) 131I using a standard thyroid uptake system, measured at 1, 2, 4, 6, 8, 10, 12, 24 and 48 h after administration. Mean radioiodine thyroidal uptake started at 33% at 1 h, stabilised at 21% between 4 and 24 h, and dropped to 18% at 48 h. There was a significant correlation between the early thyroid:salivary ratio of the parotid/mandibular complex and the radioiodine uptake at 12 h.
