ABSTRACT

PURPOSE: The purpose of this study is to evaluate the role of spinal angiography and arterial embolization in avoiding spinal cord ischemia in patients undergoing CT-guided alcohol injection of aggressive vertebral hemangiomas. METHODS: In this retrospective study, patients with vertebral hemangioma who underwent CT-guided direct alcohol injection between January 2007 and October 2018 were identified. Of 28 such patients, 26 had neurological deficits, and 2 had only back pain or radiculopathy. Direct alcohol injection without prior arterial embolization was done in 17 patients. Direct alcohol injection with prior arterial embolization was done in 11 patients. Clinical outcome was assessed immediately after the intervention and at follow-up. RESULTS: Three patients, who underwent alcohol injection without trans-arterial embolization, had worsening of neurological deficits in the post procedure period due to spinal cord ischemia. No complications related to spinal cord ischemia were noted in the embolization group. There was no significant difference in the outcomes between the two groups if the three patients with complications are excluded (p = 0.34). CONCLUSION: While CT-guided direct alcohol injection is effective in the management of symptomatic and aggressive vertebral hemangiomas, spinal angiography and trans-arterial embolization of the blood supply to the vertebral body hemangioma, prior to the direct transpedicular alcohol embolization of the lesion, improves the safety of the procedure.

ABSTRACT

BACKGROUND: Functional status of cerebrospinal fluid (CSF) diversion procedure for hydrocephalus is difficult to assess on several occasions. We report the use of serial ultrasonographic measurement of optic nerve sheath diameter (ONSD) to assess the functional status of CSF diversion procedures in patients with hydrocephalus. METHODS: In this prospective observational study, ultrasonographic ONSD measurement was performed preoperatively, on postoperative days 1, 3, and 7 (n = 51 at each time point) and at follow-up (n = 31) in patients undergoing ventriculoperitoneal shunt or endoscopic third ventriculostomy for hydrocephalus. Change in ONSD values during first week after CSF diversion procedure and at follow-up were correlated with ventriculoperitoneal shunt/ETV function. RESULTS: ONSD ≥5.5 mm strongly correlated with clinical and imaging features of raised ICP (P < 0.001). Mean ONSD progressively decreased in the postoperative period and was the lowest on postoperative day 7 (P < 0.001) with >95% of patients having ONSD <5.5 mm at that time point. At follow-up (median, 12 months; n = 31), ONSD had further reduced in 78.6% of patients. All 3 patients with shunt dysfunction had an increase in the ONSD value compared with that on postoperative day 7. CONCLUSIONS: ONSD measurement on postoperative day 7 after CSF diversion correlates well with early surgical outcome but decreases further in many patients at a follow-up of 12 months. Rise in postoperative day 7 ONSD at follow-up correlates with failure of the CSF diversion procedure.

ABSTRACT

BACKGROUND: Thoracic spine has complex pedicle anatomy with a narrow canal diameter which makes pedicle screw insertion challenging. Fennell et al. have described a simple freehand technique of thoracic pedicle screw placement. We have tested the accuracy of Fennell technique using computed tomography-based (CT-based) simulation model with pedicle screw simulator (PSS). METHODS: Normal CT thoracic spine obtained from CT thorax data of five patients were used in the 3D slicer environment using PSS for simulation. Entry points and axial trajectory as described by Fennell et al. and a sagittal trajectory parallel to the superior endplate were used for simulating the freehand technique using EA (entry angle) mode in the PSS. An ideal trajectory through the midsection of the pedicle from the same entry point and a sagittal trajectory parallel to the superior endplate were simulated using the ET (Entry Target) mode. Angle predicted by the software for an ideal axial trajectory was compared with the Fennell technique and this angle difference was noted at all the levels. Presence of pedicle breach was noted while simulating the Fennell technique. RESULTS: A total of 240 thoracic pedicle screw insertions were simulated, 120 screws by each technique. A sagittal trajectory parallel to the superior endplate caused no pedicle breach in the cranial-caudal direction at any level. No medial or lateral breach was noted while using an axial trajectory of 30° at T1-T2 and 20° from T3-T10. A 20° axial trajectory at T11 and T12 resulted in a breach of the medial cortex and the ideal mean axial angles at T11 and T12 were 2.8° and 6.5°, respectively. CONCLUSIONS: Fennell technique was effectively simulated using PSS. A uniform entry point and sagittal trajectory parallel to the superior endplate serves as a useful guide for freehand insertion of thoracic pedicle screws. At T11 and 12, ideal axial trajectories are less than 10°.

ABSTRACT

OBJECTIVE: We studied the accuracy of the radiologic diagnosis of lateral ventricular masses in children (<20 years of age). METHODS: In this retrospective study, data were collected from children with lateral ventricular masses managed in our unit between 2001 and 2016. There were 43 patients (26 boys and 17 girls; mean age, 12.1 years; range, 8 months to 20 years). Magnetic resonance imaging was available for 36 patients, whereas in 7 patients only a contrast-enhanced computed tomography scan was available. The images were read independently by 3 radiologists, who were blinded to the pathology. Two differential diagnoses were offered for each patient. The agreement between the 3 radiologists was calculated using the Fleiss κ statistic. RESULTS: The common pathologic diagnoses were subependymal giant cell astrocytoma (SEGA) (20.9%), low-grade astrocytoma (16.3%), high-grade astrocytoma (9.3%), choroid plexus papilloma (9.3%), and meningioma (9.3%). The sensitivity, specificity, and accuracy of the radiologic diagnoses were 62%, 96.7%, and 94.2%, respectively. Low-grade tumors such as low-grade gliomas, SEGAs, choroid plexus papillomas, and meningiomas were diagnosed with a high level of accuracy. High-grade gliomas, choroid plexus carcinomas, and other malignant neoplasms were difficult to diagnose on imaging. Cavernous angiomas were also difficult to diagnose. There was only fair agreement between the 3 radiologists (Fleiss κ = 0.24). CONCLUSIONS: The pathologic spectrum of lateral ventricular tumors in children is wide, and identifying the pathology on imaging is difficult for malignant tumors. Benign tumors such as SEGAs, low-grade astrocytomas, and choroid plexus papillomas are relatively easier to diagnose. There is significant interobserver variability in the radiologic diagnosis of these tumors.

BACKGROUND: Cerebral venous thrombosis (CVT) is an important cause for stroke in the young where the role for decompressive craniectomy is not well established. OBJECTIVE: To analyse the outcome of CVT patients treated with decompressive craniectomy. METHODS: Clinical and imaging features, preoperative findings and long-term outcome of patients with CVT who underwent decompressive craniectomy were analysed. RESULTS: Over 10 years (2002-2011), 44/587 (7.4%) patients with CVT underwent decompressive craniectomy. Diagnosis of CVT was based on magnetic resonance venography (MRV)/inferior vena cava (IVC). Decision for surgery was taken at admission in 19/44 (43%), within 12 h in 5/44 (11%), within first 48 h in 15/44 (34%) and beyond 48 h in 10/44 (22%). Presence of midline shift of ≥ 10 mm (p<0.0009) and large infarct volume (mean 146.63 ml; SD 52.459, p<0.001) on the baseline scan influenced the decision for immediate surgery. Hemicraniectomy was done in 38/44 (86%) and bifrontal craniectomy in 6/44 (13.6%). Mortality was 9/44 (20%). On multivariate analysis (5% level of significance) age <40 years and surgery within 12 h significantly increased survival. Mean follow-up was 25.5 months (range 3-66 months), 26/35 (74%) had 1 year follow-up. Modified Rankin Scale (mRs) continued to improve even after 6 months with 27/35 (77%) of survivors achieving mRs of ≤ 2. CONCLUSIONS: This is the largest series on decompressive craniectomy for CVT in literature to date. Decompressive craniotomy should be considered as a treatment option in large venous infarcts. Very good outcomes can be expected especially if done early and in those below 40 years.
