ABSTRACT

Spinal cord stimulation (SCS)-induced analgesia was characterized, and its underlying mechanisms were examined in a spared nerve injury model of neuropathic pain in rats. The analgesic effect of SCS with moderate mechanical hypersensitivity was increased with increasing stimulation intensity between the 20% and 80% motor thresholds. Various frequencies (2, 15, 50, 100, 10000 Hz, and 2/100 Hz dense-dispersed) of SCS were similarly effective. SCS-induced analgesia was maintained without tolerance within 24 h of continuous stimulation. SCS at 2 Hz significantly increased methionine enkephalin content in the cerebrospinal fluid. The analgesic effect of 2 Hz was abolished by μ or κ opioid receptor antagonist. The effect of 100 Hz was prevented by a κ antagonist, and that of 10 kHz was blocked by any of the μ, δ, or κ receptor antagonists, suggesting that the analgesic effect of SCS at different frequencies is mediated by different endorphins and opioid receptors.

ABSTRACT

Objective:To establish a high performance liquid chromatography (HPLC) fingerprint of branches of Juglans mandshurica and to evaluate the quality of the samples from different producing areas and in different harvest periods. Method:Chromatographic separation was performed on an Agilent Poroshell 120 SB-C18 column (2.1 mm×100 mm, 2.7 μm) for gradient elution with mobile phase of 0.2% formic acid solution (A)-0.2% formic acid acetonitrile solution (B) (0-5 min, 5%-10%B; 5-25 min, 10%-16%B; 25-40 min, 16%-22%B; 40-45 min, 22%-45%B; 45-50 min, 45%-65%B; 50-52 min, 65%-100%B; 52-55 min, 100%B) at a flow rate of 0.3 mL·min-1. The column temperature was 30 ℃ and the detection wavelength was 270 nm. The quality of branches of Juylans mandshurica was evaluated by similarity evaluation, cluster analysis, principal component analysis and partial least squares-discriminant analysis. The chemical constituents of the samples were identified by HPLC coupled with quadrupole time-of-flight mass spectrometry (HPLC-Q-TOF-MS/MS). The mass spectrometry was conducted in negative ion mode with electrospray ionization(ESI). Data were acquired over a range of m/z 100-1 700 for MS and m/z 50-1 700 for MS/MS. Result:A total of 19 common peaks were confirmed in 40 batches of samples, and the similarity ranged from 0.430 to 0.995, of which the similarity of samples collected in spring and winter seasons (April, May and December) was greater than 0.90, while the similarity of most samples collected in summer (July to September) was low. Multivariate statistical analysis showed that the samples were divided into two groups according to the harvest time, but there was no obvious classification rule for the samples from different producing areas. The contents of most constituents in the samples collected in spring and winter were higher than those collected in summer. The result illustrated that different harvest periods had great influence on the quality of branches of J. mandshurica. Compared with the samples collected in summer, the quality of samples collected in spring and winter was better. A total of 22 peaks were proved to be the main constituents that contributed to the difference between samples collected in different seasons. A total of 83 chemical components were identified by HPLC-Q-TOF-MS/MS, including 49 tannins, 7 organic acids, 14 naphthalene derivatives, 1 flavonoid, 6 anthracene derivatives, 2 lignans, 3 diarylheptanoids and 1 saccharide. Totally 13 common peaks were identified. Of the peaks that contributed to discriminate samples collected in different season, 19 peaks were identified and most of them were tannins. Conclusion:The established HPLC fingerprint can provide useful information for the quality evaluation of branches of J. mandshurica. Tannin is the main constituents in the samples. Harvest period has great influence on the quality of branches of J. mandshurica.

ABSTRACT

The pain-relieving effect of acupuncture is known to involve primary afferent nerves (PANs) via their roles in signal transmission to the CNS. Using single-unit recording in rats, we characterized the generation and transmission of electrical signals in Aβ and Aδ fibers induced by acupuncture-like stimuli. Acupuncture-like signals were elicited in PANs using three techniques: manual acupuncture (MAc), emulated acupuncture (EAc), and electro-acupuncture (EA)-like peripheral electrical stimulation (PES). The discharges evoked by MAc and EAc were mostly in a burst pattern with average intra-burst and inter-burst firing rates of 90 Hz and 2 Hz, respectively. The frequency of discharges in PANs was correlated with the frequency of PES. The highest discharge frequency was 246 Hz in Aβ fibers and 180 Hz in Aδ fibers. Therefore, EA in a dense-disperse mode (at alternating frequency between 2 Hz and 15 Hz or between 2 Hz and 100 Hz) best mimics MAc. Frequencies of EA output >250 Hz appear to be obsolete for pain relief.

ABSTRACT

The even reinforcing-reducing technique of acupuncture is one of the common needling methods in clinical practice at present, but there are various understandings on it among scholars. In the paper, the origin of the even reinforcing-reducing technique of acupuncture was explored on the base of literature collection and clinical practice. The comments were put forward in association with the initial understanding of reinforcing and reducing techniques of acupunctures and actual clinical applications. In ancient time, the even reinforcing-reducing method includes two meanings. The first one refers to the compound reinforcing-reducing needling technique for mixture of cold and heat or of deficiency and excess, in which, the reinforcing and reducing techniques are used alternatively at same acupoint. The second one implies the imbalance of and with human body or meridian involved. For example, in the syndromes as excess in the upper part of body and deficiency in the lower part, heat on the left and cold on the right, the treatment for reinforcing the lower and reducing the upper, or reinforcing the right and reducing the left is adopted to rebalance and . Hence, it is regarded as a treating principle or a needling method. No matter at the same acupoint or at different acupoints, the final target of treatment is to harmonize and . The acupuncture scholars in modern times seldom describe even reinforcing-reducing technique. Since the foundation of People's Republic of China, the relevant technique is again called even reinforcing-reducing manipulation, characterized as evenly lifting, thrusting and rotating of needle and has been developed gradually as the common needling technique in present clinical practice, which is widely divergent from the traditional understanding. The reinforcing-reducing techniques of acupuncture are different in understanding between the ancient time and the present. It is necessary to identify the origin for the better contributions to clinical application.

ABSTRACT

<p><b>BACKGROUND</b>The prognosis for patients with gastric cancer and synchronous liver metastases is very poor. However, a standard therapeutic strategy has not been well established. The clinical benefit and prognostic factors after hepatic surgical treatment for liver metastases from gastric cancer remain controversial.</p><p><b>METHODS</b>Records of 105 patients who underwent gastrectomy regardless of hepatic surgical treatment for gastric cancer with synchronous liver-only metastases in our center between 1995 and 2010 were retrospectively reviewed.</p><p><b>RESULTS</b>The overall survival rate for the 105 patients was 42.1%, 17.2%, and 10.6% at 1, 2, and 3 years, respectively, with a median survival time of 11 months. Multivariate survival analysis revealed that the extent of lymphadenectomy (D) (P < 0.001), lymph node metastases (P < 0.001), extent of liver metastases (H) (P = 0.008), and lymphovascular invasion (P = 0.002) were significant independent prognostic factors for survival. Among patients who underwent D2 lymphadenectomy, those who underwent hepatic surgical treatment had a significantly improved survival compared with those who underwent gastrectomy alone (median survival, 24 vs. 12 months; P < 0.001). However, hepatic surgical treatment was not a prognostic factor for patients who underwent D1 lymphadenectomy (median survival, 8 vs. 8 months; P = 0.495). For the 35 patients who underwent gastrectomy plus hepatic surgical treatment, D2 lymphadenectomy (P < 0.001), lymph node metastases (P = 0.015), and extent of liver metastases (H1 vs. H2 and H3) (P = 0.017) were independent significant prognostic factors for survival.</p><p><b>CONCLUSIONS</b>D2 lymphadenectomy plus hepatic surgical treatment may provide hope for long-term survival of judiciously selected patients with hepatic metastases from gastric cancer. Patients with a low degree of lymph node metastases and H1 liver metastases would make the most appropriate candidates. However, if D2 dissection cannot be achieved, hepatic surgical treatment is not recommended.</p>

ABSTRACT

This proficiency testing program is established to evaluate the pharmaceutical preparation analysis capacity of laboratories recommended by 18 countries and economies. It was authorized by Asia Pacific Laboratory Accreditation Cooperation (APLAC), and organized by Shanghai Institute for Food and Drug Control (SIFDC) and China National Accreditation Service for Conformity Assessment (CNAS). The 0.3sigma test is used to evaluate the homogeneity and stability of the proficiency testing sample. The results of the laboratories were assessed by Z-score. The robust average and the robust standard deviation of the participants' results were calculated as assigned value and standard deviation for performance assessment of hydrochlorothiazide and captopril using robust statistics. Thirty-three of 38 laboratories recommended by 18 countries and economies sent their results back. Twenty-four laboratories' results were observed as satisfactory. Five laboratories were identified as having reported at least one questionable result. Four laboratories were identified as having reported at least one unsatisfactory result.

ABSTRACT

Objective To compare the antibody response between preterm and full-term infants after primary immunization of hepatitis B vaccine (HepB).Methods Infants who were aged 7-12 months and had completed primary immunization with 5 μg HepB made by recombinant dexyribonucleic acid techniques in saccharomyces cerevisiae (HepB-SC) or 10 μg HepB made by recombinant dexyribonucleic acid techniques in Hansenula polymorpha (HepB-HP) on 0-1-6 schedule were investigated in four provinces (municipality) including Beijing,Shandong,Jiangsu and Guangxi of China.Among them, all preterm infants were selected to form the preterm group and the 1:1 matching full-term infants with the same month-age,gender and residence were randomly selected to form the full-term group.Their HepB history was determined by immunization certificate and all of their parents were interviewed with standard questionnaire to get their birth information.Blood samples were obtained from all anticipants and were tested for Anti-HBs by chemiluminescence microparticle immuno-assay (CMIA).Results Total anticipants were 648 pairs of infants. The rates of non-response,low-response,normal-response and high-response after the primary immunization were 1.39%,8.64%,45.83% and 44.14% in the preterm group, respectively. The corresponding rates were 1.08%,9.26%,44.91% and 44.75% in the full-term group. The above four rates did not show significant differences between the two groups (P>0.05).The geometric mean concentrations (GMC) of anti-HBs in the pre-term and full-term group were 755.14 and 799.47 mIU/ml respectively. There was no significantly difference in the GMCs between the two groups (P>0.05).Results from multivariable conditional logistic analysis showed that preterm was not an influencing factor to the antibody response after HepB primary immunization among newborns even after debugging the other influencing factors. Conclusion The autibody response after HepB primary immunization were similar among the preterm and full-term infants. The preterm newborns could be immunized under the same HepB immunization strategy.

ABSTRACT

Objective To compare the antibody response induced by primary immunization with 5 μ g and 10 μ g hepatitis B vaccine made by recombinant DNA techniques among the newborns. Methods Healthy infants who had completed primary immunization with 5 μg hepatitis B vaccine made by recombinant dexyribonucleic acid techniques in Saccharomyces (Hep-SC) or 10 μg hepatitis B vaccine made by recombinant dexyribonucleic acid techniques in Hansenula polymorpha (HepB-HP) were included in the study.Kids under study were 7-12 months of age and had been on 0-1-6 schedule.Standardized questionnaire was used and blood samples were collected. The titer of antibody to hepatitis B surface antigen (anti-HBs) was detected by Chemiluminescence Microparticle Imunoassay (CMIA).If anti-HBs happened to be under 10 mIU/ml,HBV DNA was further detected by nested-PCR to distinguish occult hepatitis B virus infection.Sero-conversion rate and titer of anti-HBs were compared between the two kinds of hepatitis B vaccines.Multivariate analysis was used to find the relationship between the kind of hepatitis B vaccine as well as the antibody response after debugging the other influencing factors including month-age,gender,birth-weight,premature birth and mother' s HBsAg status. Results 8947 infants vaccinated with 5 μg HepB-SC and 4576 infants vaccinated with 10 μg HepB-HP were investigated. In the 5 μg group, the rates of non-,low-,normal- and high-response were 1.88%,15.18%,61.42% and 21.52% respectively. In the 10 μg group, the corresponding rates were 0.15%,2.16%,29.42% and 68.26% respectively. The non-,low-,normal-response rates were all higher in 5 μg group than in 10 μg group (P<0.01),while the high-response rate was much higher in 10 μg group than in 5 μ g group (P<0.01).The geometric mean concentration (GMC) of anti-HBs were 354.81 mIU/ml (95% CI:338.84-363.08 mIU/ml) and 1778.28 mIU/ml (95%CI:1698.24-1819.70 mIU/ml) in the 5 μg group and 10 μg group respectively. The GMC was statistically higher in the 10 μg group than in the 5 μg group (P<0.001).The seroconversion rate and GMC were significantly different between the two groups even after debugging the other influencing factors. Conclusion Better anti-HBs response could be achieved by primary immunization with 10 μg HepB-HP than with 5 μg HepB-SC among newborns.

ABSTRACT

To develop a reverse genetics system of Peste des petits ruminants virus(PPRV), five pairs of oligonucleotide primers were designed on the basis of the full-length genomic sequence of PPRV Nigeria 75/ 1 strain. Using RT-PCR technique, five over-lapping cDNA fragments, designated as JF1, JF2, JF3, JF4 and JF5, respectively, were amplified, followed by cloning into pcDNA3.1(+)vector. An AscI restriction enzyme site and a T7 promoter sequence were introduced immediately upstream of 5'-end, while a PacI restriction enzyme site was engineered downstream of 3'-end. Using pok12 as a plasmid vector, the full-length cDNA clone pok12-PPRV of Nigeria 75/1 was assembled by connecting the five cDNA fragments via the unique restriction endonuclease site of PPRV genome. The resultant nucleotide sequence of the PPRV Nigeria 75/1 strain in the study was compared with other members of genus morbillivirus, and phylogenetic analysis was used to examine the evolutionary relationships. The results showed that PPRV Nigeria 75/ 1 was antigenically closely related to Rinderpest virus and Measles virus. ABSTRACT

Objective To understand the epidemicological condition of human brucellosis in Zhengzhou city to provide scientific evidence for future preventive and control measures. Methods Surveillance on high risk populations were done in the designated monitoring Gongyi City and other randomized monitoring cities or counties (4 or 5 towns or farms were selected in each city or county) during the year from 2002 to 2007. The intradermic allergy test with brucellin were carried out among high risk populations who had been in close touch of domestic animals or their products in those cities, and standard tube agglutination test(SAT) were used for those who had suspected clinic symptom or positive result of intradermie allergy test during 2002 to 2005. The SAT investigations for enterprises of livestock breeding were performed in non-designated monitoring cities. All manipulation methods and judging standards were according to the manual of prevention and treatment for human brucellosis. Results In the designated monitoring city Gongyi from 2002 to 2007, a total of 13 831 people were investigated, among whom 3744 peoples were tested by intradermic allergy test and the positive rate was 1.01%(38/3744). Fifty-four cases were positive in SAT among 2693, and the positive rate of SAT was 2.01%(54/2693). Forty-four new cases were diagnosed. In the 11 non-designated monitoring city from 2002 to 2005, 13 136 persons were treated by intradermic allergy test, the positive rate was 0.62% (82/13 136), among these 1316 persons were treated by SAT and 66 patients were positive. The positive rate was 5.02% (66/1316) from 2002 to 2007. Fifty one new cases were diagnosed. In the enterprises of domestic animals breeding, among 242 and 688 employees were treated by SAT, 17 and 16 patients were positive, the positive rate were 7.02%(17/242) and 5.02%(16/688) in 2006 and 2007, respectively. Conclusions The infection of human brucellosis in Zhengzhou is rising up. The governments at all levels must put large amounts of funds for the monitoring, investigating and disinfecting to prevent the disease from increasing and outspreading.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of ecoimmunonutrition supplement on intestinal microecology, epithelial tight junctions, and barrier function in rats with surgical stress.</p><p><b>METHODS</b>Seventy SD rats after surgical trauma were randomly divided into four groups:(1) placebo group,(2)total parenteral nutrition(TPN) group,(3)enteral nutrition(EN) group and (4)ecoimmunonutrition (EEN)group respectively. Rats received isocaloric and isonitrogenous nutrition. Nutrients were administered via the neck vein and the needle jejunostomy for five days. The homogenated tissues of liver, lung, and mesenteric lymph nodes were cultured to determine the bacterial translocation rate. The transmembrane binding proteins(occludin) was measured by immunohistochemistry. The ultrastructure and morphology of intestinal epithelial tight junctions in the intestine were observed by electron microscope. The feces in cecum was cultured for anaerobic bacterial growth and analysed.</p><p><b>RESULTS</b>The amounts of lactobacteria and bifidobacteria in EEN group were significantly higher than those in TPN group(P<0.05). The expression levels of occludin in the intestine was significantly higher in EEN group than that in TPN and EN group. Furthermore, the intestinal epithelial tight junction and microvilli of EEN group were more intact compared with those of TPN group. The bacterial translocation rates of liver, lung and mesenteric lymph nodes were significantly lower in EEN and EN group than those in TPN group(P<0.05).</p><p><b>CONCLUSION</b>Application of ecoimmunonutrition can protect intestinal mucosal barrier in rats with operative stress, increase the expression of occludin, maintain the gut epithelial tight junction, and eliminate gut bacterial translocation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficacy of hepatitis B viruse (HBV) vaccination and its influencing factors among children in rural area of Jiangsu province.</p><p><b>METHODS</b>Twenty-five hundred and twenty-two children born after 1998 in rural area were selected as the study population using multistage cluster sampling method. HBsAg and anti-HBs were detected by enzyme linked immunoassay (ELISA) and radio-immunoassay (RIA), respectively. Anti-HBs negative children were boosted using different hepatitis B vaccines and the efficacy was compared. Factors causing HBV infection in HBsAg positive children were also investigated.</p><p><b>RESULTS</b>HBsAg positive rates in 1-7 year olds were 0.28%-1.28%, and the anti-HBs positive rates decreased from 76.7% to 45.5%. The HBsAg positive rate in children not timely vaccinated was significantly higher than those with HBV vaccine injection within 24 hours after birth (1.4% vs. 0.5%, P = 0.031). More than 90% of the anti-HBs negative children had protective level of anti-HBs after boosted with HBV vaccine.</p><p><b>CONCLUSION</b>HBsAg positive rate in children born after 1998 in rural area of Jiangsu province decreased significantly, with an average of 0.8%. The reason for HBsAg carriage in children might be attributed to mother-to-infant transmission or not timely HBV vaccination.</p>
