ABSTRACT

Fournier's gangrene, originally described by Fournier in 1883, is a rare and unusual ischaemic necrosis of the male external genitalia. Reports on the idiopathic penile and/or scrotal gangrene are relatively rare in newer publications. The aetiology of the disease which is characterized by a sudden onset, most commonly without prodromal symptoms, is still not fully understood. There is strong evidence, however, that Fournier's gangrene is a specific infection due to Streptococcus haemolyticus, group A, and/or anaerobic Streptococci. Presenting a case of our own, we take the opportunity to discuss the pathogenesis, pathomorphology, bacteriology and treatment of the disease.

ABSTRACT

In milk-of-calcium renal cysts a suspension of fine calcific sediment is contained in a pyelogenic cyst. This very rare condition may be misinterpreted as solid renal calculi. The true roentgenologic diagnosis is established by films taken with the patient upright or sitting: the calcific material gravitating to the bottom of the cyst results in the characteristic half-moon contour.

ABSTRACT

Priapism is a very rare problem in childhood. Although sickle cell disease and leukemia may produce persistent erection, juvenile priapism is most commonly idiopathic. The physiology and pathology of erection are reviewed, and the pathomechanisms of diseases which may produce priapism are described. If red cell sickling and leukemia are excluded, immediate surgical intervention by either corporosaphenous or corporospongiosum shunts is recommended for resolution of priapism and preservation of potency. A case of juvenile priapism is presented in which HCG medication as underlying factor is discussed.

ABSTRACT

A rare condition of formation of a renal arteriovenous fistula after blunt renal trauma is described. Renal angiography confirmed the diagnosis. The auscultation of a vascular murmur in the upper abdomen is most suggestive of this lesion.

ABSTRACT

Judging according to the frequent description of the syndrome of aplasia of the abdominal muscle above all in paediatric literature (up to now more than 250 cases), the knowledge of the symptomocomplex (aplasia of the abdominal muscle, malformations of the kidneys and the urinary tracts, cryptorchism) might be of particular interest for the urologist, since the urologic malformations decisively determine the prognosis quoad vitam of these patients. On the basis of three own cases the importance of the removal of infravesical obstruction is emphasized, whereas the reconstructive intervention for dilatation of upper urinary tracts is seldom mandatory.

ABSTRACT

Issuing from the pathophysiological fundaments of the incontinence after operative sanation of the outlet of the urinary bladder the principal surgico-prosthetic possibilities for the removal of this complication are describes: 1. artificial systems of the sphincter with arbitrary increase and decrease of the occlusion pressure of the proximal urethra, 2. permanent compression of the bulbar urethra. On the last mentioned principle is based the incontinence prosthesis after Kaufman, which in its rate of success corresponds to the artificial sphincter systems.

ABSTRACT

Small-Carrion penile prostheses were implanted in 61 patients who have been followed for 3 to 21 months. Of these 61 patients 55 have achieved satisfactory results, with a return of normal sexual function. The importance of preoperative evaluation and preparation is stressed.

ABSTRACT

Due to increasing number of radical surgeries for malignant tumors of the pelvis the incidence of iatrogenically caused erectile impotence rises. Available surgical management comprises implantation of rigid penile prothesis or inflatable device. The authors prefer a rigid implant (Small-Carrion penile prothesis) on account of its simplicity and have been impressed with the results.

ABSTRACT

The insertion of two silicone rods into the penile corpora cavernosa (Small-Carrion penile prosthesis) is a considerable procedure for operative management of erectile impotence. Of 61 treated patients, 55 had a satisfactory anatomical and functional result, most of them regained the same sexual activity as prior to onset of impotence. Their partners were satisfied, too. Preoperative evaluation of causes of impotence is particularly important. Type and number of complications in this operative procedure are reported in detail.

ABSTRACT

Comparison of the MMPI and Mini-Mult scales yielded significant scale-by-scale correlations. However, the group means were different in 6 of the 11 possible scales, and there was a general tendency for the Mini-Mult to over-estimate MMPI scales. The Mini-Mult proved to be highly ineffective to detect MMPI invalid profiles. In addition, agreement on profile configurations or high points and degree of pathology as indicated by number of significant clinical scale elevations was very poor. The results of this investigation are similar to the findings of Rybolt and Lambert (1975), as well as those of Platt and Scura (1972), and lead one to recommend that the Mini-Mult not be employed as a clonical assessment instrument with incarcerated adult female populations. Its multiple deficits make clinical use hazardous.
