ABSTRACT

A study of the clinical presentation of acquired aural cholesteatoma in 51 Indian children is presented. An attempt has been made to assess the impact of this chronic ear disease in terms of the morbidity it incurs upon the paediatric population. The preponderance of bilateral presentation in cholesteatomatous otitis media (24.5 per cent) and the associated sensorineural or mixed hearing loss (in 25.4 per cent of the ears) is emphasized, to reflect the alarming audiological disability in the afflicted child. An attempt has been made to correlate the otoscopic profile with the extent of disease, osteitic damage and co-existing complications. It was noted that, in Indian children, mesotympanic choleasteatoma was more often associated with large defects of the pars tensa (viz. marginal, subtotal and total perforations--54.4 per cent) than with postero-superior retraction pockets (37.2 per cent) or purely attic defects (7.8 per cent). Further analysis revealed this common otoscopic presentation to be a more active process, occurring in predominantly cellular mastoids and associated with extensive disease. Based on this study, a more aggressive canal down tympanomastoid approach is advocated for children presenting with this otoscopic profile.

ABSTRACT

Oral Submucous Fibrosis (OSMF) is a disease of unknown etiology. A total of 96 patients with Oral Submucous Fibrosis have received four regimens of treatment--local dexamethasone, local hyaluronidase, local combination of dexamethasone and hyaluronidase, and local placental extract. The patients were followed up for a period varying from 3 months to 2 years. The group of patients receiving hyaluronidase alone showed quicker improvement in symptoms although its combination with dexamethasone gave somewhat better longer-term results. A new regimen for the treatment of submucous fibrosis is recommended.

Extramedullary plasmacytomas of the head and neck are very uncommon tumours. Three rare cases of extramedullary plasmacytoma are presented-one each in the nose, larynx and maxilla.

ABSTRACT

An unusual case is described of a denture in the oesophagus which caused a fatal haemorrhage from the aorta. The post-mortem examination confirmed that a projection from the denture had impinged upon the aortic wall, after perforating the oesophagus, with subsequent erosion of the aorta.

A leiomyosarcoma of the upper respiratory and food passages is a rarity. Two rare cases of leiomyosarcoma of the paranasal sinuses, of which one developed recurrence are reported.

Cutaneous horn or sebaceous horn is an extremely rare clinical entity. A case of cutaneous horn of the cheek presenting in a middle aged female is reported. It was successfully treated by surgical excision.

ABSTRACT

One hundred cases of malignant tumors of the larynx, pharynx, oral cavity, nose and paranasal sinuses were studied. The hypopharynx was the most frequent site and the most common type of tumor was squamous cell carcinoma. Lymph nodes were palpable in 59% of cases, of which 72.8% showed histological evidence of metastasis. The highest incidence of metastasis was found in nasopharyngeal growths and the lowest in the nose and the sinuses. In laryngeal tumors the site of the tumor was the most important factor influencing regional lymphatic metastasis. The grade of the tumor influenced the lymph node metastasis in the regions of the hypopharynx, tonsil and base of tongue. At other sites the differentiation of the tumor did not seem to have any correlation with the incidence of metastasis.

ABSTRACT

Maggot infestation in a series of 100 cases pertaining to the E.N.T. domain is reported. The Entomological aspects were studied in 20 cases. The clinical features of myiasis have been described. This disease commonly occurs in diseased nasal and ear cavities which allow maggots to grow. The use of maggot oil drops and irrigation with normal saline provides an effective treatment. Prevention of maggot infestation is stressed.
