ABSTRACT
Bladder diverticula can be either congenital or acquired. They are the formation of a urothelial hernia through the muscle wall fibers of the bladder. Congenital bladder diverticula are rare and there are no previous reports showing them to have been found through an antenatal ultrasound exam revealing the presence of hydronephrosis. Here we describe the case of a large congenital bladder diverticulum revealed through an antenatal hydronephrosis diagnosis and whose proximity to the ureter led to its removal and a ureteral reimplantation.

A criação do Sistema Único de Saúde determina a criação de um novo modelo de atenção à Saúde, cujo eixo de organização é a integralidade. Para tanto, o Ministério da Saúde cria o Programa de Saúde da Família, em 1994, com o intuito de estruturar seu sistema de saúde, a partir da Atenção Primária à Saúde. Sua implementação e expansão não foram suficientes para transformar as práticas hegemônicas de saúde de cunho estritamente biomédico. Assim, com o objetivo de rediscutir e transformar as práticas sanitárias, o Ministério da Saúde lança a Política Nacional de Humanização, apresentando o acolhimento como diretriz operacional para mudança do processo de trabalho. O ordenamento legal ou políticas indutoras não têm se mostrado capazes de transformar as práticas sanitárias, mantendo o modelo biomédico, que segue soberano nos serviços de saúde. A universidade como instrumento de transformação social e pensamento crítico e reflexivo tem o papel de induzir e fomentar novas práticas sanitárias, tendo como referência as necessidades de saúde da população e as políticas governamentais. Considerando tal situação, o objetivo da pesquisa foi identificar a percepção dos acadêmicos de Medicina sobre Acolhimento e auxiliar no processo de reflexão e transformação das práticas de formação, tendo como referência as Diretrizes Curriculares Nacionais e o Sistema Nacional de Saúde. A pesquisa foi de cunho qualitativo, realizada por meio de entrevistas semiestruturadas com acadêmicos do décimo segundo período do curso de medicina da UNIVALI. Os resultados apontaram que ainda existe uma forte influência do Modelo Biomédico na formação, sustentados especialmente pelos cenários de prática do SUS, com alguns lampejos por parte dos acadêmicos, em forma de crítica, apontando ainda de forma desordenada a integralidade na atenção à saúde.
The creation of the Unified Health System determines the creation of a new health care model, whose axis of organization is comprehensiveness. Therefore, the Health Ministry created the Family Health Strategy in 1994 in order to structure its health system on the basis of Primary Health Care. Its implementation and expansion were not sufficient to transform the hegemonic health practices that are strictly biomedical. Thus, in order to revisit and transform health practices, the Health Ministry launched the National Humanization Policy, presenting "User Embracement" as an operational guideline for changing the work process. The legal framework or the inducive policies have not proven capable of transforming health practices, maintaining the sovereignty of the biomedical model in health services. The university as an instrument of social transformation and critical and reflective thinking has the role of inducing and promoting new health practices, based on the population's health needs and on government policies. Thus, the objective of this research was to identify the perceptions of medical students regarding "User Embracement" and assist in the process of reflection and transformation of academic training practices, with respect to the National Curriculum Guidelines and the National Health System. This was qualitative research, carried out through semistructured interviews with students in the twelfth semester of medical school at UNIVALI. The results showed a strong influence of the biomedical model in their academic training, supported by National Health System scenarios of practice, with some critical glimpses from the students that point to, in a yet disorderly manner, comprehensiveness in health care.
