ABSTRACT

The article contains data about the peculiarities of socio-economic and health-organization factors in the rendering emergency and out-patient care for elderly and senile patients with arterial hypertension. The results of study of satisfaction by medical care quality depending on the age and gender of the patients are discussed. A large number of living alone and widowed patients among elderly patients is marked. About half of the respondents in both groups are low-income; among middle-income respondents a low proportion of the elderly is observed. The majority of patients can be admitted to the doctor in the polyclinic no earlier than 2 to 7 days from the moment of entry. The obtained data should be taken into account when forming a model of medical-social care for patients with arterial hypertension in older age groups.

ABSTRACT

A 10-year prospective study was undertaken of a category of 957 families, who emerged in 1990-1992, for the purpose of defining the factors influencing the shaping of few-children families: 276 (28.8%) of them had few children, 558 (58.4%), had an average number of children and 123 (12.8%) had many children during the mentioned time span. According to the results, a set of factors was defined accountable for few-children families with the key of them being: duration of education (34.1 and 40.2% for female and male, respectively); marriage age exceeding 25 years (47.1 and 51.1%); an unsatisfactory health state of marital partners and of their children, a high rate of divorces ((40.6%); poor material and dwelling conditions (69.6%); a low standard of social protection etc.

ABSTRACT

A prospective observation over 347 families formed through creating a new family with a new partner from among incomplete families. A total of 195 families of the above figure were formed by single mothers (103--divorced, 49--widowed). The study results showed that only 62.5% of women got married for the second time the others preferred to live temporarily together. 55% of women gave birth to common children with new partners. A number of specific features were detected, the main of which are the relations between the members of newly created families and their relatives. The nature of psychological relations in such families has an essential influence on the reproductive policy and on the health condition of their members.

ABSTRACT

A prospective study of 3 cohorts of rural families (with 10-year interval) was carried out for the first time. The first cohort included 79 families with many children, observed for 30 years since 1970; the 1980 cohort (observed for 20 years) consisted of 37 families, and the 1990 cohort (observed for 10 years) consisted of 23 families with many children. Analysis of the reproductive function of cohorts of families showed changes in the number of children in the families: the percentage of families with many children and the mean number of children in a family decreased. Causes essential for the structure of a family with children and birth rate decrease are analyzed.

ABSTRACT

Analysis of medical demographic parameters of the Nizhnii Ingash territory of Krasnoyarsk region is analyzed for the period of 1959-1999. Periods of changes (transformations) in the rural territory are distinguished. Time course of natural migration of population correlated with these periods. Number of children per rural family was estimated for the Krasnoyarsk region and Nizhny Ingash territory and the regularity of transformation of a traditional family with many children into a family with few children is described. Interviews of the population revealed the relationship between social reforms and reproductive policy of families.
