ABSTRACT

Low back pain (LBP) is a common complaint among individuals engaged in physically demanding occupations, such as construction workers, luggage lifters, manual laborers, and drivers. One of the main problems facing modern healthcare is treating these people. The identification of distinct patient subgroups with non-specific LBP and the development of specialized, more effective therapies are of crucial significance to enhancing evaluation and treatment regimens. This case report describes the evaluation and management of non-specific LBP in a male construction worker who complained of severe low back discomfort. Enhancing the muscular endurance, strength, and flexibility of the back muscles and soft tissues is the main goal of exercise therapy, which is the key to the management of nonspecific LBP. This patient receives a four-week treatment regimen that includes movement control exercises and several advanced therapeutic modalities. The direction of movement control ensures the way patients sit when their back muscles contract. Back muscle activation rates are greater in the active extension group and lower in the flexion group. A comprehensive rehabilitation program that was effective for our patient, who was experiencing lower back discomfort. We assessed the efficacy of our outcome measures using a variety of outcomes, including the modified Oswestry disability index, visual analog scale, range of motion, Quebec back pain disability scale, and pressure biofeedback unit for muscle strength. In addition to a standard physiotherapy course, providing modern physiotherapeutic treatments was found to be more beneficial for enhancing the patient's overall health and quality of life.

A prevalent musculoskeletal disorder known as nonspecific low back pain (NSLBP) is characterized by lumbar discomfort or pain that lacks a distinct, identifiable etiology. It is the main root of disability in all corners of the globe, affecting individuals across diverse age groups and occupations. NSLBP is often categorized as a multifactorial condition, encompassing a range of potential contributing factors such as poor posture, sedentary lifestyle, muscle imbalances, and psychosocial elements. According to current standards, there is a good prognosis for acute nonspecific back pain, although this prognosis is mostly reliant on return to function. Various treatment strategies are available, totally reliant upon the underlying cause of the discomfort. This case report presents the combination of traditional therapy and William's flexion exercises in a 25-year-old female nursing student who presented with complaints of low back pain (LBP) for the last three months. This study investigates the effect of William's flexion exercises in nonspecific low back pain to manage pain and range of motion (ROM), and improve the overall quality of life, which was evaluated using the visual analog scale (VAS), modified Schober's test, pressure biofeedback unit, and modified Oswestry disability questionnaire. The patient received an enhanced physiotherapy program that increased the flexibility and range of motion in the lumbar extensor, hip flexor, and hamstring muscles. The outcome measure shows notable gains after the therapeutic interventions.

The maxilla is among the jaw bones with a lot of blood flow. Maxillary bone necrosis is uncommon and can be caused by infection, trauma, or unusual metabolic abnormalities. The maxilla is a vital bone that forms the roof of the mouth cavity. Mucormycosis is a prevalent fungus that infects the maxilla, particularly in diabetic and immune-compromised people and post-COVID-19 patients. Osteomyelitis is an inflammatory condition affecting the bone and marrow tissues. It is an opportunistic infection that occurs due to the host's susceptibility to illness due to the complication of other diseases. The patient, in this case, has the chief complaint of a defect in the palate. Based on a CT brain, buccal X-Ray, and HRCT scan of Thorax. The patient's condition was diagnosed as a surgical defect in a post-operated case of mucormycotic osteomyelitis of maxilla post-COVID-19 infection. The right lower lobe's posterior segment noted a tiny soft tissue density nodule. The operated case of a surgical defect of mucormycotic osteomyelitis of maxilla post-COVID-19 infection was treated with a surgical intervention that is surgical curettage and debridement of the left maxillary sinus, bilateral fibrotomy, and reconstruction with bilateral nasolabial flap under general anesthesia. However, our objective is to enhance the quality of life, increase ventilation, increase the shoulder joint's range of motion, and enhance the joint play of the temporomandibular joint so that the patient can go back to his usual activities without difficulty. A complete pulmonary rehabilitation plan was designed to meet the patient's objectives, executed, and followed for one month. It comprised myriad interventions like bedside sitting, facial expression exercises, mouth opening exercises, neck exercises, dynamic quadriceps and hamstring, active range of motion exercises for shoulder joints, thoracic expansion exercises, and breathing exercises like diaphragmatic breathing.

ABSTRACT

Metaplastic breast cancer is an uncommon and fatal condition. It is described histologically as a tumor having epithelial differentiation into squamous and mesenchymal components, with numerous elements frequently co-existing in the same tumor. This case study sought to provide analgesic treatment in a case with malignant pleural effusion related to breast cancer based on evidence. A 67-year-old female with a known history of metaplastic breast carcinoma came to the tertiary care hospital with complaints of breathlessness which progressed to grade II on the Modified Medical Research Council (mMRC) Dyspnoea Scale, a cough with mucoid expectoration, restlessness, nausea, and reduced appetite for eight days. Diagnostic findings revealed bilateral pleural effusion (left>right). Laboratory investigations revealed that the excess fluid accumulated was transudate in nature, according to the Light's criteria. This case report illustrates the strategy, management, and importance of adherence to pulmonary rehabilitation and painkiller care physiotherapy for patients with metaplastic breast cancer and pleural effusion to achieve the best possible physical and mental health.

Anaplastic astrocytoma is a kind of astrocytoma (a type of brain cancer) that is classified as World Health Organization (WHO) grade III. Headaches, poor mental status, focal neurological impairments, and seizures are the most prevalent early signs and symptoms of anaplastic astrocytoma. Anaplastic astrocytomas have also been linked to earlier exposure to vinyl chloride and large doses of brain radiation treatment. Anaplastic astrocytomas are a form of astrocytoma that also falls under the umbrella term of gliomas, which are tumors that develop from glial cells. This is because astrocytes are a kind of glial cell. As a result, anaplastic astrocytomas (grade III) are also known as "grade III gliomas" or "high-grade gliomas. In this case study, we present a case of a 35-year-old male who presented to our cancer hospital with complaints of weakness in the right upper and lower limbs for two years. He was then diagnosed with anaplastic astrocytoma, grade III. But after tumor resection, he developed right hemiplegia with involvement of the right upper extremity and lower extremity. This case study demonstrates how the neuro-physiotherapy rehabilitation protocol in the case of hemiplegia after brain tumor resection aids in improving motor function and functional independence. Physiotherapy treatment that is modulated according to the patient's needs plays a vital role in improving the quality of life and helping to delay the worsening of symptoms, thereby helping to increase the life span of patients diagnosed with anaplastic astrocytoma grade III.
