ABSTRACT

The purpose of this research is to evaluate the accuracy of AI-assisted quantification in comparison to conventional CT parameters reviewed by a radiologist in predicting the severity, progression, and clinical outcome of disease. The current study is a cross-sectional study that was conducted on patients with the diagnosis of COVID-19 and underwent a pulmonary CT scan between August 23th, 2021 to December 21th, 2022. The initial CT scan on admission was used for imaging analysis. The presence of ground glass opacity (GGO), and consolidation were visually evaluated. CT severity score was calculated according to a semi-quantitative method. In addition, AI based quantification of GGO and consolidation volume were also performed. 291 patients (mean age: 64.7 ± 7; 129 males) were included. GGO + consolidation was more frequently revealed in progress-to-severe group whereas pure GGO was more likely to be found in non-severe group. Compared to non-severe group, patients in progress-to-severe group had larger GGO volume percentage (40.6%± 11.9%versus 21.7%± 8.8%, p ˂0.001) as well as consolidation volume percentage (4.8% ± 2% versus 1.9% ± 1%, p < 0.001). Among imaging parameters, consolidation volume percentage and the largest area under curve (AUC) in discriminating non-severe from progress-to-severe group (AUC = 0.91, p < 0.001). According to multivariate regression, consolidation volume was the strongest predictor for disease progression. In conclusion, the consolidation volume measured on the initial chest CT was the most accurate predictor of disease progression, and a larger consolidation volume was associated with a poor clinical outcome. In patients with COVID-19, AI-assisted lesion quantification was useful for risk stratification and prognosis evaluation.

ABSTRACT

Increasing the efficacy of allergen-specific intranasal immunotherapy (INIT) has recently been the main goal of several studies to establish this route as a safe delivery method through mucosal pathways. In this case, the present study evaluated the potential of INIT using ovalbumin (OVA)-loaded mesenchymal stromal/stem cell (MSC)-derived exosomes (Exo-OVA) in an allergic asthma mouse model. Together with control groups, sensitized Balb/c mice underwent intranasal immunotherapy with Exo-OVA (10 µg OVA per dose) for three consecutive weeks. Serum-specific immunoglobulin E (IgE) levels, transforming growth factor-beta (TGF-ß), interleukin (IL)-4, and interferon-gamma (IFN-γ) production by cultured spleen cells, lung histopathologic analysis, and nasopharyngeal lavage fluid cellular examinations were then conducted. The results showed that INIT using Exo-OVA significantly increased IFN-γ and TGF-ß secretion, while allergen-specific IgE and IL-4 production were dramatically decreased compared to the control group receiving phosphate-buffered saline. In addition, the eosinophil and total cell counts in the nasopharyngeal lavage fluid were reduced, and inflammatory conditions and cell accumulation in lung tissue were ameliorated. In conclusion, the Exo-OVA improved the INIT efficacy compared to free OVA. Therefore, this formulation could be introduced as an effective approach for immunomodulatory purposes with a shorter treatment duration and reduced side effects.

ABSTRACT

BACKGROUND: There is no valid and reliable tool to measure COVID-19 healthcare stress felt by healthcare students. A scale was developed to assess COVID-19 stress in healthcare students and its psychometrics was examined. METHODS: This is a two phases mixed-method study including a qualitative stage consisting of student interview and literature review to develop content of the tool. In the quantitative stage, the psychometrics of the scale was examined in 2020-2021. RESULTS: The COVID-19 related healthcare student stress scale (CHSSS) featured five factors including fear of catching coronavirus, social constraints, changes in education, non-compliance of health protocols and worrying news and overload information, which totally explained 51.75% of the total variance. CONCLUSION: Validity and reliability of CHSSS with 17 items were supported to measure COVID-19 stress in healthcare students as a self-assessment tool. Researchers can utilize this tool to assess COVID-19 stress in healthcare students and introduce policies and intervention especially designed for healthcare students.

ABSTRACT

BACKGROUND: Due to the fact that pre-diabetic people are at higher risk of developing diabetes, it is possible to reduce the risk by taking preventive measures. Therefore, the present study aimed to determine the effect of theory-based education on promoting a healthy lifestyle and fasting blood sugar (FBS) in pre-diabetic women. METHODS: This is a cluster-randomized controlled trial that was performed on 71 pre-diabetic women referred to Arak Comprehensive Health Service Center. Thus, using cluster sampling method, one center was randomly assigned to the intervention group and one center to the control group. The data collection tool was a questionnaire based on the theory of planned behavior (TPB) and healthy lifestyle behavior that was completed before and at least 3 months after training and FBS test was performed. The experimental group received 3 training sessions of 60 to 90 min and finally the data were analyzed using statistical software. RESULTS: After the intervention, the mean scores of knowledge (P < 0.001), attitude (P = 0.047) and perceived behavioral control related to physical activity (P = 0.046) and dietary function (P = 0.01) increased significantly in the intervention group. In addition, fasting blood sugar in the intervention group (99.70 ± 11.06) improved significantly compared to the control group (110.94 ± 17.09) (P = 0.003). CONCLUSION: Education based on the theory of planned behavior, by holding face-to-face meetings along with following up the samples after the educational intervention, can promote healthy lifestyle of pre-diabetic women. Therefore, designing and implementing similar interventions on all pre-diabetic individuals seem necessary. ABSTRACT

The properties of the sound field, which is scattered from the sea surface, must be considered in any system that uses surface-reflected acoustic waves. In this paper, the probability density functions (PDFs) for time and the angle of arrival of the acoustic wave scattered from the sea surface are proposed. The trajectories of the emitted rays that reach the receiver are obtained by using the sea surface elevation and slope and the positions of the source and receiver. In this approach, the phenomenon of shadowing, which plays an important role in small grazing angles, is taken into account. The shadowing effect causes some parts of the sea surface to be shadowed by other parts. To show the validity of the proposed approach, the results are compared with experimental data and the arrival time and angle fluctuations obtained from a ray-tracing model with realistic one-dimensional sea surface boundaries. Also, it is shown that the standard deviation of the received angle fluctuations in this model is in close agreement with the experimental results reported in the literature. The importance of this model is that the PDFs of the arrival angle and time are presented theoretically, considering the shadowing effect.

ABSTRACT

AIM: Cancer is one of the most important chronic diseases which affects caregivers. Therefore, in this study, we decided to increase the self-efficacy and sense of coherence of caregivers by using virtual learning via mobile to increase the power of managing in such situations. METHODS: This study is a clinical trial performed on 60 caregivers of cancer patients. Questionnaires of demographic information, Bandura self-efficacy, and Antonovsky's sense of coherence were completed for both intervention and control groups. Then, for the intervention group, a virtual educational intervention was performed daily for 1 month using the mobile phones, and for the control group, no intervention was performed. RESULTS: The mean score of self-efficacy in the intervention group was (18.09±93.21) before and (135.12±11.14) after the intervention, which was statistically significant (P=0.001). While in the control group, this means was (84.71±18.55) before and (84.75±17.13) after the study, but this difference was not statistically significant (P=0.2). Also, the mean score of sense of coherence in the intervention group was (73.25±7.28) before and (89.11±7.11) after the intervention, which was statistically significant (P=0.001). While in the control group, this means was (76.42±7.45) before and (76.89±7.27) after the study, but this difference was not statistically significant (P=0.31). CONCLUSION: Educational interventions, even virtually, can lead to improved self-efficacy and a sense of coherence in caregivers of cancer patients and can be used as one of the low-cost and effective methods in managing and controlling the disease.
