ABSTRACT

Ultrasound imaging to verify effective emptying of seminal vesicles is proposed, during spermograms included in investigation of couple sterility. The aim is to detect distal obstructive seminopathies, often unilateral and inaccessible to enzymatic analysis of sperm. A review of recently drafted case reports of 41 sterile men showed that in half of them an insufficient ejaculate volume always corresponded to poor emptying of seminal vesicles as seen on ultrasound images. If an obstruction is present it is usually in the ejaculatory canal zones. Two patients with azoospermia were treated by endoscopic resection of terminal part of ejaculatory canals with only one good result in the spermogram. With respect to counting of spermatozoa, it is not possible in patients with decreased emptying of seminal vesicles, a part from those with azoospermia, to establish any correlation between type of obstructive seminopathy and its effect on the spermogram. Of the 41 men in this series, a half had problems with their partner and the other half another known sterility factor.

ABSTRACT

The authors report their experience of male genitourinary ultrasound studies of the anterior pelvis over a period of 4 years and involving almost 500 patients. The authors first used a classical ultrasound apparatus in real-time via an anterior abdominal approach. They now have an Aloka mode B apparatus in real-time which by use of three approaches (anterior abdominal, rectal and urethral) offers the possibility of analysis of the male anterior pelvis with four planes of section: 1 sagittal and 3 transverse. Thus MGUUS provides 12 views of the male pelvis. The authors define the techniques used in the light of their experience with this apparatus. They then describe the normal ultrasound anatomy of the male anterior pelvis. Finally, they present their results in pathology with this technique. These results have been obtained in benign and malignant prostatic pathology, everyday routine, longitudinal analyses before and after treatment: surgery, radiotherapy and medical. The results are promising in more recent indications: tumours of the bladder, endocrine andrology, pathology of fertility and in the investigation of the pathological scrotum. Implantation of MGUUS at the centre of technical approaches in urology is felt by the authors to be a necessity, making possible the rational and economic use of the apparatus for both in- and out-patients and the cost of an ultrasound apparatus for every hospital department of urology would appear to be reasonable.

ABSTRACT

The authors have used alpha-blockers in the everyday treatment of prostatic symptoms for the past four years. The results are good and would appear to be more effective than the majority of other treatments. Efficacy is remarkable in dysuria and acute urinary retention. The authors report their experience based upon 300 cases. This alpha-blocker therapy using phenoxybenzamine makes it possible to delay operation and to avoid it in certain cases. In particular, it facilitates the pre- and post-operative phases. Marketing of this drug in France should be encouraged.
