ABSTRACT

The authors discuss the problem of widely spread chronic carrier state of HBs-antigen by healthy persons. The majority of antigen carriers show both moderate clinico-biochemical and ultrasonic signs of chronic liver involvement, and different disorders of the microcirculation which are formed in conditions of prolonged persistence of the virus of B hepatitis in the body. Early diagnosis of generalized disorders of the terminal blood circulation in antigen carriers may be useful in the determination of pathogenetically substantiated treatment tactics and prognostication of unfavourable outcomes of chronic HBs-antigen carrier state in preclinical period of the disease.

ABSTRACT

Clinico-laboratory characteristics of liver lesions occurring in 64 immune donors of blood plasma of 0(1) group are presented. The diseases ran as subclinical, latent, anicteric forms of hepatitis, and only in 5 persons as acute icteric hepatitis. In 50% of the patients there were signs of the disease becoming chronic: long-term increase in the blood serum of the activity of alanine aminotransferase, alkaline phosphatase, variations in bilirubin levels. Certain unresolved and disputable problems of etiological diagnosis of hepatitis in plasma donors, especially in differentiation of autoimmune hepatitis and non-A-non-B hepatitis are discussed.

ABSTRACT

The complex clinico-epidemiological and laboratory surveys of the children and the staff in 71 institutions for children of preschool age were made at the period of increased viral hepatitis morbidity in these institutions, and the dynamic observations of 11 children's institutions having children with prolonged HBs-antigenemia were carried out. In the foci of viral hepatitis the level of antigenemia in the children and the staff exceeded (p less than 0.01) that in the control group. The dynamic observations (lasting up to 6 years) of the groups containing HBsAg-carriers revealed that 6 children, previously HBsAg-negative, showed a short-term HBs-antigenemia in the absence of any evidence of the parenteral (instrumental) route of infection. This fact indicates that the role of HBsAg-carriers as the possible sources contributing to the transfer of hepatitis B through everyday contacts in children's institutions cannot be ruled out. Periodic clinico-epidemiological and laboratory surveys of groups with HBsAg-carriers are recommended.
