ABSTRACT

The work evaluated the long-term treatment results of 103 patients with complicated diverticular disease, who underwent the elective surgery in the volume of left hemicolectomy. The first group included 53 patients with recurrent diverticula. The second group consisted of 50 patients, who underwent the operation on diverticula of sigmoid colon with formation of sigmostoma on the first stage. The number of patients with functional gastro-intestinal and psyco-emotional disorders was more significant in the first group compared with the second group and it was associated with presence of these disorders before the operation. There was noted the reliable decrease of quality of life rates according to questionnaire scales MOS-SF 36 in the first group. The average indices of scales had some matching to the rates of health people in the patients of the second group. The authors came to conclusion, that indications for elective surgery should be determined very carefully in case of recurrent diverticula.

ABSTRACT

The possibility of formation of connective-tissue vascular prostheses on subcutaneously implanted polychlorovynil base was demonstrated in experiments on rats. Suppression of the function of phagocytic cell with carrageenan decelerates, while its stimulation with tamerit accelerates the formation of connective-tissue autoprostheses.

ABSTRACT

Experimental studies on rats have demonstrated possibility of connective-tissue grafts formation on polyvinylchloride base introduced subcutaneously. Dynamics of autograft formation is characterized by gradual thinning of wall, regulation of collagen fibers, decreased number of cells with fibrocytes predominance. Autografts in carotid position retain its patency during 3 months, and endotheliocytes appear on their internal surface.

ABSTRACT

The results of surgical treatment of 209 patients with diseases of bile ducts with use of minimum approach and special instrument are presented. Cholecystectomy was carried out in 186 patients, cholecystectomy together with other interventions-in 15, operations on hepaticocholedochus in 7, cholecystectomy and choledochostomy-in 1. Intraoperation surgical complications were observed in 0.95% of cases, postoperative ones--in 2.34%; postoperative mortality rate made up 0.95%. After cholecystectomy the patients were discharged in 3-9 days. The authors consider the operations on biliary tract from minimum approach as an alternative to conventional laparotomy as well as laparoscopic cholecystectomy.

ABSTRACT

The significance of two-stage treatment of obstructive jaundice caused by tumors of the terminal part of the biliary tract is shown on rich clinical material. The performance, in the first stage, of I.D. Prudkov's laparoscopic cholecystostomy which causes little trauma improved the accuracy of topical diagnosis to 95%. Its simple techniques and reliability made it accessible to a wide circle of surgeons. Preliminary decompression of the biliary tract increased operability to 14.9% and reduced postoperative mortality to 8.9%. In cases in which a radical operation was not possible, the risk of a palliative intervention diminished and postoperative mortality reduced to 3.1%.

ABSTRACT

The experience with non-surgical treatment of retained stones in 13 patients is described. The treatment was successful in 8 cases. The method of dissolution of the stones and their mechanical removal with the help of a lithotrap of own design was used. The conclusion is drawn, that the treatment of the patients with gallstones in presence of external biliary fistulas should start from conservative methods.

The authors report their experience with laparoscopic cholecystostomy in 37 patients. No failure in the surgery was noted, and complications were rare. The operation should be performed only by those specialists, who are experienced in the performance of laparoscopy.
