ABSTRACT

Commonly, small bowel obstruction (SBO) is caused by either postoperative adhesions or external hernias. Internal hernias are rare, accounting for less than 2% of all cases of intestinal obstruction. An internal hernia through the falciform ligament is extremely uncommon and is usually secondary to a congenital or iatrogenic defect caused by trocars insertion. In this article, we report a case of SBO in a virgin abdomen that appeared to be caused by a congenital defect in the falciform ligament. A search of the literature was done identifying all reported cases of internal hernias caused by falciform ligament defect in order to guide diagnosis and management as well as avoidance of hernias caused by iatrogenic defects.

ABSTRACT

BACKGROUND: Although Lebanon is a highly polluted country, so far no study has specifically been designed to assess the association between outdoor air pollution and chronic bronchitis in this country. OBJECTIVE: To assess the association between exposure to outdoor air pollution and chronic bronchitis in Lebanon. METHODS: A pilot case-control study was conducted in two tertiary care hospitals. Cases consisted of patients diagnosed with chronic bronchitis by a pulmonologist and those epidemiologically confirmed. Controls included individuals free of any respiratory signs or symptoms. After obtaining informed consent, a standardized questionnaire was administered. RESULTS: Bivariate, stratified (over smoking status and gender) and multivariate analyses revealed that passive smoking at home (ORa: 2.56, 95% CI: 1.73-3.80) and at work (ORa: 1.89, 95% CI: 1.13-3.17); older age (ORa: 1.75, 95% CI: 1.55-2.39); lower education (ORa: 1.44, 95% CI: 1.21-1.72); living close to a busy road (ORa: 1.95, 95% CI: 1.31-2.89) and to a local power plant (ORa: 1.62, 95% CI: 1.07-2.45); and heating home by hot air conditioning (ORa: 1.85, 95% CI: 1.00-3.43) were moderately associated with chronic bronchitis; an inverse association was found with heating home electrically (ORa: 0.58, 95% CI: 0.39-0.85). A positive dose-effect relationship was observed in those living close to a busy road and to a local diesel exhaust source. CONCLUSION: Chronic bronchitis is associated with outdoor air pollution.

ABSTRACT

Waterpipe smoking and its association with chronic bronchitis has not been assessed in Lebanon. This case-control study in Beirut in 2009/2010 evaluated this relationship: 274 cases of chronic bronchitis and 559 controls without the condition aged > or = 40 years were enrolled. Data were collected by questionnaire on: sociodemographic characteristics, respiratory symptoms, smoking (waterpipe and cigarette) and nicotine dependence. ANOVA, Student, Kruskal-Wallis, chi-squared and Fisher exact tests were used when applicable and logistic regression analysis was carried out. Previous waterpipe smoking (OR = 6.4), previous mixed smoking (OR = 38.03) and current mixed smoking (OR = 7.68) were significantly associated with chronic bronchitis (P < 0.001 for all) but current exclusive waterpipe smokingwas not (OR = 1.87, 95% CI: 0.74-4.72). Current waterpipe dependence was significantly associated with chronic bronchitis (OR = 3.74, P < 0.001). After adjustment for covariates/confounders, ever waterpipe smoking > 20 WP-years (P < 0.001) was significantly associated with chronic bronchitis.

ABSTRACT

Familial clustering of elevated antiphospholipid antibody levels has been described, but the reports are heterogeneous with regard to the characterization of the antiphospholipid syndrome (APS), coexisting autoimmune diseases and clinical implications. We report a familial occurrence of APS in two patients, in the presence of SLE in the mother and absence of autoimmune diseases in the daughter along with acquired circulating inhibitors in both cases: platelet inhibitor and factor IX inhibitor, respectively.

ABSTRACT

A localized thrombus involving the ascending aorta and arch rarely occurs in the absence of an underlying etiology such as chest trauma, atherosclerosis, a hypercoagulable state or instrumentation. A review of the literature between 1966 - 2003 yielded 38 reported cases of localized aortic arch thrombi, 21 of which were treated by surgical excision of the thrombus using different approaches. In this communication, we describe this clinical entity, with its diagnosis and management. The technical details that are important to ensure the safe conduct of the procedure are discussed.

ABSTRACT

To estimate the incidence of pleural mesothelioma and its relationship with the occupational and environmental exposure to asbestos in Chekka region. Between 1991 and 2000, 22 cases of malignant mesothelioma were diagnosed at Hôtel-Dieu de France Hospital. Eighteen cases were epidemiologically investigated. Fifteen among these 18 patients (83%) had a positive exposure history: exposure was occupational in 11 cases and environmental in 4 cases. The tumor was attributable to Eternit Company in 12 cases among the exposed 15 (80%). These 12 cases were secondary to occupational exposures in 8 and to environmental exposure in 4 cases. Mean latency period between exposition and diagnosis was 29 years. Fifteen patients died from the progression of their disease after a median survival of 8 months. The relationship between pleural mesothelioma and Eternit Company with the related occupational and environmental risk in Chekka region is obvious. The assessment of the incidence needs a national cancer registry. Despite the protective measures taken by the government since 1996, an increase in the incidence is suspected in the coming ten years because of the long latency period of the disease.

ABSTRACT

BACKGROUND: Advanced peri-implant intrabony defects require comprehensive surgical treatment regimens different from periodontal therapy strategies. The purpose of this longitudinal trial was to evaluate the peri-implant outcomes following guided bone regeneration with 3 treatment protocols. METHODS: In 25 patients, 41 peri-implant defects with supporting bone loss >50% of the implant length were treated with flap surgery plus autogenous bone grafts alone (FG) (controls, n = 12) plus non-resorbable (FGM) (test 1, n = 20) or bioabsorbable barriers (FGRM) (test 2, n = 9) and supportive antimicrobial therapy. Following submerged healing, the membranes were removed (FGM), and the peri-implant probing depths (PD), probing bone levels (BL), mobility scores (PT), and intrabony defect height (DH) were radiographically evaluated at baseline, 6 months, and 1 and 3 years post-therapy. RESULTS: Non-surgical/anti-infective therapy resulted in a limited improvement of PD scores after 6 months. At the 3-year visit, surgical treatment revealed significant changes from baseline for the controls and both of the test groups for PD: 5.1 +/- 2.7 mm (FG), 5.4 +/- 3.0 mm (FGM), and 2.6 +/- 1.6 mm (FGRM), and for BL: 3.2 +/- 2.4 mm (FG), 3.4 +/- 2.4 mm (FGM), and 2.3 +/- 1.6 mm (FGRM), Mann-Whitney test, P < or = 0.05. The changes for DH and PT were significant only for FG- and FGM-treated subjects. The overall improvement for FGRM-treated patients during the 3-year observation was less marked. However, the differences between the 3 surgical treatment protocols did not affect the treatment outcomes after 3 years. CONCLUSIONS: Autogenous bone grafting is an appropriate treatment regimen to augment open crater-formed peri-implant defects. Although certain clinical situations require an additional fixation of barrier membranes, their routine application should be approached with caution.

ABSTRACT

This article presents a technique for soft tissue reconstruction and covering defects associated with maxillary implant-supported restorations. A pedicle subepithelial connective tissue flap is prepared from the palatal mucosa near the area to be treated and is displaced into the receptor site. The donor site remains primarily covered. An increase in soft tissue volume is achieved at the receptor site, which is advantageous for various reasons. The pedicle graft has been used for different indications: closure of the alveolus after immediate implant placement, papilla reconstruction, defect and dehiscence repair, and multiple-layer closures after bone grafting and treatment of peri-implantitis. Over a 32-month period, 103 patients were treated with this method. Partial flap necrosis occurred in only 2 patients. All other patients showed significant improvement over the preoperative condition.

ABSTRACT

Along with osseointegration and restoration of function, the patient's subjective satisfaction with the esthetic result is a touchstone of the success of implant therapy. Although esthetic restoration of natural teeth can be achieved routinely through appropriate tooth preparation and a natural-looking design on the part of the dental laboratory, the road to success is much more complicated with implants, because of atrophy of bone and mucosa. Surgical techniques, paths of incision, and useful instruments for implant therapy are described, from implant placement to exposure. These methods help to provide durable, functional, and esthetic results.

ABSTRACT

Between 1991 and 1995, 216 sinus-lift procedures were accomplished as part of a clinical study. The study involved placing 467 implants in the atrophic posterior maxillae of 142 female and 74 male patients. The initial bone height at the implant site was between 1 and 5 mm. The implants were supported subantrally with bone block grafts harvested from the retromolar or symphysis areas of the mandible. Perforations of the maxillary sinus membrane were observed in 51 patients; these were repaired with fibrin adhesive. The spaces remaining above the bone graft were filled with various materials. A total of 28 implants failed. All the remaining implants were deemed successfully osseointegrated, based on radiographic and clinical (including periodontal health) criteria. No patients experienced maxillary sinus complications. Clinically and radiographically, the best bone regeneration was observed in those patients in whom the surgically created space was completely grafted with autogenous bone that included a high percentage of resorption-resistant cortical bone. In those patients having bone grafts harvested from the mandibular symphysis, none of their facial profiles were adversely affected; however, some patients experienced neurosensory deficits involving the mandibular anterior incisors and adjacent alveolar mucosa. Occasionally, these symptoms persisted for up to 1 year following the procedure.

ABSTRACT

Augmentation of the maxillary sinus in the atrophied edentulous posterior maxilla is an integral part of implant prosthodontics. This study examined the clinical outcome in 50 periodontally compromised successfully treated subjects with severe maxillary atrophy following oral implantation with Brånemark, IMZ or Frialit-2 endosseous implants between 1991 and 1994. Simultaneous sinus augmentation was achieved using autogenous bone grafts harvested from the anterior mandible. Oral implants in 37 periodontally healthy patients directly placed in the stable local maxillary bone served as controls. The oral rehabilitation included implant supported restorations or removable superstructures over a period between 3 and 5 years. The peri-implant status of implant abutments inserted in the periodontal compromised augmented maxilla resulted in values comparable to the local maxillary bone except for the GCF rates with enhanced levels of 63.9 +/- 49.9 (controls 37.9 +/- 40.7). The average peri-implant Periotest values in the augmented maxillary sinus (test group) were -3.1 PT and +0.2 PT in the controls. The Periotest scores in the sinus area ranked between -7.0 and +5.0 with mean PT values of -1.5 for IMZ, -3.2 for Brånemark and -4.0 for Frialit-2 abutments. The functional integration of oral implants following sinus augmentation with autologous bone grafts and conventionally placed endosseous implants in the local bone was similar. The additional implant stabilization within the mandibular cortical bone grafts resulted in very low Periotest scores. In periodontally compromised subjects treated for chronic adult periodontitis with minimal maxillary bone height less than 5 mm the endosseous implantation with simultaneous sinus augmentation is recommended as an appropriate technique for long-term oral implant rehabilitation.

ABSTRACT

All lung cancer cases admitted in our hospital from the 1st of January 1979 to the 31st of December 1995 were reviewed to allow for the study of the "behaviour" of the disease in Lebanon. A total of 386 cases included 87.5% males and 12.5% females. Mean age was 60 years. 87.3% were smokers. Squamous cell carcinoma was the most frequent histologic type (37.3%) and predominant in men. Adenocarcinoma was more frequent in women (50% v/s 29.9%). Patients often consulted their physician after delays of several months. Diagnosis is commonly confirmed by bronchoscopy. Transthoracic needle aspiration has become, since 1990, a very useful procedure for diagnosis of peripheral lesions with a sensitivity of 90.6%. Metastases are recorded in 30.9% of cases. Histological type distribution and advanced cancer stages found in our study are similar to that encountered in extensive international studies over the last few decades.

ABSTRACT

Clagett's thoracostomy is a widely used procedure in the initial treatment of postpneumonectomy empyema. We describe in this paper an original one-step operation of thoracic filling after empyema sterilization. The triple plasty includes an apical thoracoplasty limited to the first 3 ribs, a pedicle flap of the latissimus dorsi transferred inside the thoracic cavity, and an upper translation of the diaphragm to the 4th rib in order to reduce residual pleural space. Three patients were operated and followed up clinically (for 20, 29, and 30 months), and radiologically with a MRI on the 12th month. Complete tissue healing and loss of all residual space was observed in all patients. We therefore recommend the use of this technique rather than thoracic wall coverage procedures.

ABSTRACT

The anti-ischemic and cardioprotective effects of various angina drugs have been evaluated during angioplasty. This special situation of transitory regional ischemia was used to test verapami given by intra-coronary injection in 15 patients undergoing balloon dilatation of the left anterior descending artery, suffering from stable angina and with normal left ventricular kinetics. Each patient serving as his own control, parameters indicating oxygen consumption and clinical and electrocardiographic (ST segment changes) markers were measured during two successive inflations. Continuous monitoring of comparative variations between a first inflation without treatment and a second inflation after intra-coronary administration of 1 mg of verapamil revealed, following 60 seconds of ischemia, the absence of any change in heart rate, a moderate fall in mean blood pressure (101.7 as against 96.8 mmHg, p = 0.033) and a notable 60% reduction in the amplitude of maximal ST depression (5.07 +/- 3.08 mm as against 2.07 +/- 1.44 mm, p = 0.0002). These results confirm the anti-ischemic properties of verapamil, the action of which is based upon an indirect peripheral vasodilator model but also on a probable direct cardioprotective effect on anoxic myocardial regions.

ABSTRACT

In a randomized double blind study effects and tolerability of the local anesthetics Prilocaine 3% with Felypressin, Articaine 4% with Epinephrine 1/200,000 and 1/100,000, and Lidocaine 2% with Epinephrine 1/100,000 were tested. There was no significant difference between the four agents as far as effect on blood pressure, pulse, and tissue rehabilitation are concerned. Prilocaine can be used for extractions and surgical tooth removal. It can not be recommended for apicoectomy or implantation. The better results of Articaine 4% compared to Lidocaine 2% were statistically not significant.

ABSTRACT

In a prospective, randomised double blind study we recorded the rate of complications in dental anesthesia in more than 1500 patients using 4 commonly known anesthetic solutions. 228 of our patients (18.3%) noted headaches postoperatively. Furtheron we found syncopes, failures of anesthesia and nausea. Double vision was noticed once intraoperatively (Articain) and postoperatively (Lidocain). We found a high standard of security in dental local anaesthesia.
