ABSTRACT

Field simulation exercises (FSXs) require substantial time, resources, and organizational experience to plan and implement and are less commonly undertaken than drills or tabletop exercises. Despite this, FSXs provide an opportunity to test the full scope of operational capacities, including coordination across sectors. From June 11 to 14, 2019, the East African Community Secretariat conducted a cross-border FSX at the Namanga One Stop Border Post between the Republic of Kenya and the United Republic of Tanzania. The World Health Organization Department of Health Security Preparedness was the technical lead responsible for developing and coordinating the exercise. The purpose of the FSX was to assess and further enhance multisectoral outbreak preparedness and response in the East Africa Region, using a One Health approach. Participants included staff from the transport, police and customs, public health, animal health, and food inspection sectors. This was the first FSX of this scale, magnitude, and complexity to be conducted in East Africa for the purpose of strengthening emergency preparedness capacities. The FSX provided an opportunity for individual learning and national capacity strengthening in emergency management and response coordination. In this article, we describe lessons learned and propose recommendations relevant to FSX design, management, and organization to inform future field exercises.

ABSTRACT

In eastern Africa, Maize lethal necrosis (MLN) is caused by the co-infection of maize plants with Maize chlorotic mottle virus (MCMV) (Tombusviridae: Machlomovirus) and Sugarcane mosaic virus (SCMV) (Potyviridae: Potyvirus). With the disease being new to Africa, minimal effective management strategies exist against it. This study examined the potential of 10 fungal isolates to colonize maize plants and induce resistance against MCMV and SCMV. Maize seeds were soaked in fungal inoculum, sown and evaluated for endophytic colonization. Fungus-treated plants were challenge-inoculated with SCMV and/or MCMV to assess the effects of fungal isolates on the viruses in terms of incidence, severity and virus titers over time. Isolates of Trichoderma harzianum, Trichoderma atroviride and Hypocrea lixii colonized different plant sections. All plants singly or dually-inoculated with SCMV and MCMV tested positive for the viruses by reverse transcription-polymerase chain reaction (RT-PCR). Maize plants inoculated by T. harzianum and Metarhizium. anisopliae resulted in up to 1.4 and 2.7-fold reduced SCMV severity and titer levels, respectively, over the controls but had no significant effect on MCMV. The results show that both T. harzianum and M. anisopliae are potential candidates for inducing resistance against SCMV and can be used for the integrated management of MLN.

ABSTRACT

Intensity of human schistosomiasis infection increases with age, a peak being attained at early puberty. Hormones could be involved in the age-related changes in susceptibility to schistosomiasis. Male BALB / c mice were infected with Schistosoma mansoni either before or after puberty and worm numbers, cellular immune responses, hormonal levels and pathology analysed. Pre-puberty infected mice had a significantly higher number of adult worms (p < 0.05), more severe granulomas, higher mortality rate and higher proliferative responses as compared to post-puberty infected mice. Levels of the hormones were lower in the pre-puberty infected mice as compared to the post-puberty group early in the infection. Plasma levels of testosterone and luteinizing hormones decreased significantly (p < 0.05) in infected mice when compared to controls. Susceptibility to S. mansoni in male BALB / c mice seems to be influenced by levels of testosterone and luteinizing hormone at infection. Albeit, an infection with S. mansoni seems to lower the hormonal levels.

ABSTRACT

Adults and children have differences in their susceptibility to schistosomiasis. The relative influences of age-dependent innate resistance and acquired immunity in the differences between susceptibility to schistosomiasis are difficult to assess in humans. Therefore, we exposed juvenile and adult female rhesus monkeys to primary infection with Schistosoma mansoni. In contrast to the adult animals, the juvenile rhesus monkeys had low levels of interleukin (IL)-4 and IL-5 production by peripheral blood mononuclear cells after schistosome infection, as well as lower levels of parasite-antigen-specific antibody (IgG, IgM, and IgA) responses, and produced limited antigen-specific or total IgE. Juvenile animals had statistically nonsignificant increased worm burdens and tissue or fecal egg counts, compared with that of adults, whereas circulating schistosome antigens were significantly higher in infected juvenile monkeys. These results suggest that juvenile rhesus monkeys have reduced type 2 cytokine responses after primary schistosome infections and perhaps are more susceptible to parasite infection.
