ABSTRACT

The congenital clubfoot of infancy to-day is curable in 90% of cases. Aim of the treatment is normalization of form, function and growth. With combined therapy according to Imhäuser optimal results are possible: 1. Immediately after birth--depending on the general condition of the newborn--reposition of all components without the talipes equinus component; plastercasts are changed frequently till the third or fourth month of life; 2. surgical development of the heel; calcaneal equinus is corrected by elongating the tendocalcaneus combined with arthrolysis of the ankle joint; 3. intensive clinical treatment by physiotherapy; 4. ambulant orthopedic controls at regular intervals till the end of skeletal growth.

ABSTRACT

The manifestation of Perthes disease after succesfully treated lymphoblastic leukemia is extremely rare. At the first signs of a femoral head necrosis--sonsidering the basic disease--and after cortisone treatment one has to think of a possible femoral head necrosis. It has been observed by several authors after long-term cortison therapy. The case of a 7 year old boy with Perthes disease after acute lymphatic leukemia is demonstrated. After treatment with chemotherapy, cortisone and radiation of the head he was free of pain for 2 years, then the Perthes disease became manifest with a deformed femural head. It could be cured but operation is necessary now or at the latest after termination of growth. In the case there seems to be no connexion between cortisone treatment and the manifestation of Perthes disease. It is more probable that this developed independently of the basic disease and its successful treatment because of the unilateral affection of the hip joint and the interval between treatment of leukemia and manifestion of the first symptoms of Perthes disease.

ABSTRACT

Fractures of the vertebral column due to riding accidents are frequent. In most of the cases we observe compression fractures of a vertebral body without compression symptoms of the spinal cord. If fractures of the spine heal with a gibbus deformity degenerative changes of the vertebral joints with severe back pains may ensue. Therefore it seems necessary to treat fractures of the vertebral spine with immediate reposition (ventraler Durchhang) and following immobilisation with a plastic corset Lightcast, Hexcelite). Occasionally operative stabilisation is necessary, especially in fractures of the cervical spine.

ABSTRACT

Epicondylitis is mostly related to a tendinosis found in the region of the radial or ulnar epicondylus, where the common origin of the extensor or flexor muscles is anchored. Several causes are associated with the etiopatho-genesis, most important is mechanical stress. The working hand is involved in the majority of cases. The age of the patients ranges mainly from 40-60 years. In 90% of the cases conservative measures are successful; if surgical treatment is necessary we recommend a modified version of Hohmann's operation.
