ABSTRACT
The paper shows the rate of pathomorphism, by comparing the major cytometrical indices of squamous-cell cancer of the lung before and after single-stage intratumoral chemotherapy on the basis of studies of puncture and intraoperative specimens from 13 patients. Morphometry of 1950 relatively preserved tumor cells was made. The degree of intratumor chemotherapeutic action on tumor cells was objectively reflected in a significant difference in the mean values of the area of the cytoplasm (from 325.1 +/- 7.52 microm2 in the puncture specimens before administration of chemicals to 426.2 +/- 12.67 microm2 in the intraoperative specimens after their administration) and nuclei (from 127.9 +/- 3.45 to 179.7 +/- 7.86 microm2) and in the nuclear shape ratio (from 1.32 +/- 0.02 to 1.48 +/- 0.02).

Routinely stained cytological preparations of puncture biopsy specimens of mediastinal tumors from 870 patients are examined. Diagnostically informative material is investigated in 759 (87.3%) cases; malignant tumors are diagnosed in 538 (62%) cases, benign in 221 (25.2%). In 59 (6.8%) cases the cytological answers were descriptive, and in 52 (6%) puncture biopsy material was not informative for cytological verification. Diagnostic sensitivity of the cytological method is 94%, diagnostic specificity 71%, and diagnostic efficacy 91%.

A complex examination of 1400 patients with pathologies of the lungs and mediastinum was carried out using roentgenologic, endoscopic, bioptic and cytologic methods. Diagnosis was confirmed morphologically in 93.7% of cases. More specific information was obtained by express cytologic examination in 97.9% of cases of malignant growth and in 64.4% with benign tumors. On the basis of the test results, preoperative intratumor chemotherapy technique suggested by the authors may be recommended for clinical use.

The complex examination of 1400 patients with pathologic masses of the lungs and mediastinal organs with the use of roentgenologic, endoscopic, puncture bioptic and cytologic methods was carried out. Morphologic verification of diagnosis was performed in 93.7% of the patients. In express cytologic study, histogenesis of neoplasms of the thoracic cavity organs was specified in 97.9% of cases of a malignant tumour, and in 64.4% of those of a benign one.

Cytologic studies of the material from pulmonary and mediastinal tumors, carried out at a room for x-ray-and-surgery and contrast methods of examination have recognized the type of pulmonary tumors in 96% of cases and of mediastinal tumors in 93.3% of cases. The histologic type of pulmonary tumors has been identified in 82% and of mediastinal tumors in 72% of cases. The method for urgent cytologic diagnosis improves the efficacy of morphologic verification of thoracic tumors before specific treatment.

The results of the observation over 15 cases of benign esophageal tumors, diagnostic procedures permitting to precise the esophageal pathology, the validity of the catheterization in the roentgenological examination of the esophagus aimed at the differentiation of a given pathology from esophageal cancer and the treatment results as well are set forth in this work. A complex of diagnostic procedures and features recommended for the differential diagnosis of esophageal diseases is suggested.
