ABSTRACT

The risk factors of venous thromboembolism (VTE) in 1146 patients who were underwent operated for the diseases of digestive organs, for obliterating atherosclerosis of abdominal aorta and peripheral arteries, diseases of bones and joints were studied. The methods of VTE prevention in these patients were studied. To evaluate the risk of VTE we designed the special chart. Three levels of risk of VTE: low, moderate and high were distinguished. The specific prophylaxis of the VTE using low molecular weight heparin fraxiparine was given to the patients moderate (90.7%) and high (93.3%) risk degree.

ABSTRACT

Treatment of intestinal necrosis, caused by an acute mesenterial ischemia, represents the most complex field of surgery. Results of surgical treatment of 74 patients with various forms of mesenterial ischemia, complicated by intestinal infarction, were analyzed. Revascularization ought to be conducted if the ischemia may be eliminated. High frequency of anastomotic sutures insufficiency is noted in performance of primary resection of intestine. Syndrome of the polyorgan dysfunction constitutes the main cause of death in patients after the operation.

ABSTRACT

Based on experimental investigations in 26 mongrel dogs, we established that due to acute ileus (AI) proximal and distal regions of small intestine are colonized by pathogenic aerobic and anaerobic microorganisms, which causes occurrence of enteral insufficiency syndrome (EIS). Absorption of microorganisms and their toxins through intestinal wall leads to bacteriemia, endotoxemia and morphological insufficiency of parenchymatous organs. The results of treatment of 486 patients with AI of different etiology are studied. Based on clinical and diagnostical changes 4 stages of EIS are separated. The rate of post-operative complications depends on severity of EIS. When treating the patients with AI the severity of EIS should be taken into consideration. Specific pre-operative preparation, the relevant technology of surgical intervention and post-operative treatment should be used differently at every stage of the disease.

ABSTRACT

Based on the author's findings a conclusion has been reached that it is expedient to use fraxiparin, a low-molecular heparin, in the early postoperative period after reconstructive operations on the arteries, and ticlid, a deaggregant on a regular basis, in the remote postoperative period, this drug having been found to increase the percentage of spared critically ischemized limbs compared with aspirin.

ABSTRACT

In atherosclerosis there occur changes in hemodynamics [34], blood rheology [29], functional condition of the endothelium, thrombocytes and leukocytes [7, 13], in the blood coagulation system [1, 2]. In the presence of vascular affections of the lower part of the body the above changes make for development of critical ischemia of the inferior limbs, which, in its turn, aggravates the above-mentioned disorders.
