ABSTRACT

Echocardiography was used to examine the intracardiac hemodynamics, contractile and diastolic functions of the heart in 64 patients with postinfarction cardiosclerosis in the presence of eukinetic circulation and the effects of verapamil depending on the index of a cardiometric product (CMP). A relationship was found between the action of the drug and the sizes of the heart and severity of left ventricular hypertrophy. In patients with normal cardiometric parameters, the antianginal effect of the drug was achieved by peripheral arterial vasodilation and a significant rhythm slowness of heart performance. As the CMP index increased due to the antianginal effect of the drug, an effect of transient cardioplegia was prevalent. The value of the CMP index is an important indication of indirect assessment of the left ventricular dysfunction in patients with postinfarction cardiosclerosis concurrent with eukinetic circulation.

ABSTRACT

The author studied the state of intracardiac hemodynamics, contractile and diastolic function of the heart in patients with hypertensive disease (stage II) depending on the index of the double cardiometric product reflecting the heart sizes. It was established that disorders of the cardio- and hemodynamics in patients with hypertensive disease depend not only on the stage of the disease but also on the degree of hypertrophy of the left cardiac ventricle and sizes of the heart. Groups of patients are distinguished with a different level of exertion of the adaptation-compensation mechanisms at rest and level of functional myocardial insufficiency. The proposed index enables to evaluate the physical working capacity without ergometric examination.

ABSTRACT

The author compared volume-time parameters of the cardiac cycle with the index of cardiometric product in 64 patients with ischemic heart disease with a history of myocardial infarction. Cardiomegaly was more frequently observed in the anterior location of scar fields. In cases of moderate heart dilatation realization of the hemodynamic compensation occurs due to equal contribution of the period of rapid filling of the left ventricle and emptying of the left atrium. In marked dilatation of the left ventricle cavity occurs a reduction of the role of the period of rapid filling in providing cardiac ejection and its compensation is realized mainly due to diastasis and its terminal phase--left atrium systole.
