ABSTRACT

The authors have studied the effect of preoperative administration of Vazaprostan-20 into the pulmonary artery of patients with a defect of the interventricular septum (DIVS) and high pulmonary hypertension. By the end of the second day the patients examined had lower pressure in the pulmonary artery from 18% to 38%, higher intracardiac output in patients with the III A and III B degrees of pulmonary hypertension. In all the patients plasty of the DIVS was fulfilled under conditions of hypothermal extracorporeal circulation. The degree of residual pulmonary hypertension in the group under study was 42% at an average, and in the control group it was 50%. The preoperative administration of Vazaprostan-20 decreased the operative risk and facilitated the postoperative period. It allowed to make operations on the wider group of patients whose possibility to be operated on was doubtful. The patients of the group under study were discharged from the hospital in a satisfactory state.

ABSTRACT

In patients with ischemic heart disease impairments in blood plasma lipid spectrum were more distinct in the chronic form of the disease. At the same time, the phospholipid composition was altered in thrombocytes as compared with that of persons free of ischemic heart disease and of stenosing coronary atherosclerosis.

ABSTRACT

The data of postal questionnaire were compared to those obtained as a result of examination (anthropometry) of a random sample (n-806) of persons working at one of the Moscow industrial enterprises. It has been shown that the postal questionnaire makes it possible to assess the body weight with an accuracy of up to +/- 3 kg in 67 percent of men and in 69 percent of women. Meanwhile the height could be assessed with an accuracy of up to +/- 3 cm in 93 percent of men and in 80 percent of women. The relationship was established between the error made in the subjective assessment of the body weight and the genuine magnitude of the body weight. The possibilities of correcting the data obtained have been also demonstrated. The postal questionnaire permits the identification of excess body weight (Quetelet's index over 28) in 68 percent of men and in 78 percent of women and can be used for studying the prevalence of excess body weight among the population without employment of additional manpower for examinations.

ABSTRACT

A new algorithm is proposed for the questioning of female subjects in screenings for angina of effort, that differs in certain respects from the anginal diagnosis algorithm using standard cardiologic questionnaire currently accepted in epidemiologic studies. The advantages of the modified questionnaire make it more suitable for the screening of female populations.

ABSTRACT

Seven cases of traumatic defects of the interventricular septum are described. All the patients were subjected to operations. The operations were fulfilled under conditions of the artificial blood circulation. The authors believe the greatest difficulties to occur when correcting defects localized in the muscular part of the apical area of the septum. One patient died within 4 years after the operation due to recurring cardiac fibrillation and chronic heart failure.

ABSTRACT

The authors operated 1056 patients on "dry" heart under the conditions of hypothermia. The total lethality constituted 4.5%. Since 1970 they have been applying craniocerebral hypothermia method using the apparatus "Kholod-2F". Secondary failures of the interatrial septum and valve stenosis of the pulmonary artery in patients aged from 18 to 20 are the most common indications for operation. 450 and 248 patients, respectively, were operated upon; lethality was 3.7% and 4%. The 16 years' experience with the superficial hypothermia in the correction of congenital heart diseases has proved this method to be highly effective and economical one, allowing to obtain quite favourable results after a thorough preoperative diagnosis and strict selection of patients.

ABSTRACT

During a 18-year period the authors operated 305 patients with aortic coarctation, the mortality being 2.2%. Sixty three patients (20.6%) aged from 20 to 40 years were operated upon with the mortality in 7.9%. A direct anastomosis of the aorta was constructed in 31 patients, alloprosthesis of the aorta was employed in 23 cases, indirect isthmoplasty--in 8 cases, in one case an aortic prosthesis was replaced. The most frequent intra- and postoperative complication was intrapleural hemorrhage. In estimating the efficacy of surgery within the terms up to 14 years the best results were obtained after an end-to-end anastomosis.
