ABSTRACT

Twenty-seven patients with various types of internal TMJ disorders were examined at Voronezh Dental School Oral and Maxillofacial Surgery Department. All the patients underwent comprehensive examination and double-circuit TMJ arthroscopy, as well as joint fluid sampling for subsequent biochemical studies. A correlation between the degree of pathological changes of the TMJ intraarticular structures and biochemical parameters in traumatic TMJ lesions was discovered. The patterns observed may serve as a useful tool for more accurate diagnostics and treatment planning.

ABSTRACT

The objective of the present work was to improve the quality of diagnostics and treatment of hypervascular masses in ENT structures based on the multidisciplinary approach. The study involved 51 patients presenting with hypervascular masses in ENT structures. All of them underwent comprehensive medical examination including fibrolaringoscopy, multispiral computed tomography, magnetic resonance tomography, ultrasound dopplerography, selective carotid angiography, and/or various combinations of these techniques. The patients were given surgical, endovascular, and combined treatments. The study has demonstrated the advantages of application of the multidisciplinary approach to combined diagnostics and treatment of hypervascular masses in ENT structures as exemplified by neck hemangioma.

ABSTRACT

The article summarizes the experience of diagnostics in 60 patients with large face and neck angiodysplasia. Compression test, cytology, Doppler ultrasound with color Doppler mapping, X-ray computed tomography and angiography with intravenous contrast, magnetic resonance imaging with angiography and selective carotid angiography of branches of external carotid arteries were used for adequate diagnostics. The efficiency of a multidisciplinary approach in diagnosis and treatment of extensive face and neck angiodysplasias is shown in the study.

ABSTRACT

In article experience of diagnostics and treatment of 30 patients with angiodysplasia of the tongue was given. The surgical treatment was made to 21 patients with vascular malformation of the tongue, selective bilateral catheterization of lingual arteries and endovascular microembolization by PVA - 250-400 µ was successfully performed in 4 cases, transcutaneus sclerotherapy with solution aethoxyskleroli 0,5% in 3 cases was successfully provided. Advantages of endovascular diagnostics and treatment methods angiodysplasias of the tongue were shown.

ABSTRACT

This study included analysis of the results of treatment of 136 patients at the age from 19 to 92 years presenting with pathological changes in the frontal sinuses. 126 of them had undergone surgery for the treatment of inflammatory diseases, osteomas, and injuries to the frontal sinuses in the preceding period. 62.7% of the patients in the latter group developed relapses of the disease; therefore, 44.1% of them needed the second operation. In order to improve the efficiency of treatment of chronic recurrent and postoperative frontitis, radical operation on the frontal sinuses in 56 patients was completed by obliteration of the sinus using the osteoplastic composite material MK-9M. Dynamic observations during the three subsequent years demonstrated stable recovery in all the 56 patients.

ABSTRACT

We experimented on 60 white laboratory rats of line of "Wistar". The efficiency of use of the preparation of "Hypoxen" has been studied. Where there was the acute supportive and necrotic inflammation of soft tissues. The visual and morphological examination of the rate of regeneration was carried out. In the group of animals which have got the "Hypoxen" the accelerated healing of post operational injuries was observed.

ABSTRACT

Surgical intervention is the only method of radical treatment of carotid paraganglioma (CP). Intraoperative blood loss may exceed 2 litters. Ligation of the external carotid artery (ECA) is not always possible technically due to tumour invasion to the ECA's walls. Prior to operation, in order to diminish blood loss it is suggested to perform endovascular embolization (EE) of the vessels feeding the tumour. The case report presented herein concerns an 18-year-old male patient with CP after comprehensive diagnosis including ultrasonographic duplex scanning with Doppler colour mapping, enhanced-contrast spiral computed tomography, selective carotid angiography. The patient was subjected to selective. EE of the afferent vessels of CP. Embolization of the branch of the ascending pharyngeal artery travelling to the lower pole of the tumour, and that of the large branches feeding the upper pole of the tumour from the initial portion of the occipital artery with poly vinyl alcohol (PVA-300) and hydrogel "Embox" was followed by surgical intervention: removal of the tumour, closure of the ostia of the tumour's own arteries, ligation of the EC A. The volume of intraoperative blood loss amounted to 350 ml. The postoperative period proved uneventful with nothing to report. During the check-up examinations of the patient, performed 3, 6, 12 and 24 months after the operation, a good postoperative therapeutic outcome was noted to persist.

ABSTRACT

Morphological methods were employed to prognosticate the clinical course of abscesses in deep-lying perimandibular and pharyngeal soft tissues. It was shown that soft tissues surrounding the suppurative focus in patients presenting with the long-standing inflammatory process contain cells with nuclei of relatively low ploidy. This finding provides a rationale for the inclusion in the combined treatment of abscesses of an additional range of medicines normalizing reparative processes in the soft tissue.

Comparative analysis of cytogenetic indices and soft tissues regenerative activity of postoperative wounds was carried out in 69 patients with abscesses and phlegmons of maxillofacial region. The reparative regeneration potential was reduced that was directly connected with infectious mutagenesis in patients with prolonged inflammatory process running.

ABSTRACT

Checkup and treatment of 187 patients with different forms of chronic sialoadenitis was done, 38 from them parotid gland sialoendoscopy was performed. Sialoendoscopic criteria of chronic sialoadenitis forms and clinical results of the fulfilled sialoendoscopic procedures in the immediate postoperation period were described. The direct visual assessment of parotid duct system let with high degree of exactness to determine the sialoadenitis form and simultaneously to make surgical intervention that made sialoendoscopy the choosing method in diagnostic and treatment of chronic inflammatory pathology of major salivary glands.

ABSTRACT

Two cases of combined treatment of auricular vascular neoplasms (arteriovenous malformation and hemangioma) are presented. Their two-step management included endovascular microembolization of the posterior auricular artery with polyvinyl alcohol (PVA 300, 500 mcm) and surgical resection of the tumour. The results confirm the feasibility of combination of traditional and endovascular surgery for the treatment of this pathology. Preoperative endovascular microembilization not only arrests hemorrhage from angiodysplasia but also decreases its size and isolates it from healthy tissues. Embolization of the posterior auricular artery can be performed only after the comprehensive angiographic evaluation of the blood flow in angiodysplasia and the adjacent vessels using superselective catheterization and officinal emboli.

ABSTRACT

To 17 patients after lower jaw resection with disarticulation (on the occasion of neoplasm and traumas) the defects were replaced by vascularized autotransplants of the 2nd radius of pedis and vascularized fibula autotransplants in combination with titanium implants. In all cases positive results were received. The authors consider that microsurgery with vascularized fibula autotransplants in combination with titanium implants was an optimal way to replace lower jaw defects after its resection with disarticulation in TMJ.

ABSTRACT

The article deals with the description of a rare clinical case wherein a 44-year-old female patient presenting with bleeding capillary angioma of the left auricle obstructing the auditory passage was fist subjected to endovascular microembolization of the posterior auricular artery by means of polyvinyl alcohol (PVA-500), followed by a second stage of virtually bloodlessly surgical removal of the tumour carried out five days thereafter. This case report once again points to feasibility of combined, i. e. endovascular and conventional surgical management of capillary angiomas. Preoperative microembolization of a bleeding haemangioma arrests haemorrhage from the tumour, resulting in the decreased size thereof, as well as leading to circumscribing the tumour from healthy tissue. Attention is paid to the fact that embolization of the auricular artery should be performed only after a thorough angiographic evaluation of the pattern of the blood flow in the tumour and adjacent vessels by means of the technique of selective catheterization and using only the officinal emboli.

ABSTRACT

Special method of complex diagnosis of mandibular injury is under consideration. It is based on the informational technologies. The study of the mechanisms of the injury's formation has been made on 109 patients of the skull-jaw-facial surgery department of the hospital were under investigation. Two main types of jaw-facial injuries have been revealed. The first type: falling down from the height of one's own size (stature). The second type: blow (stroke) of a blunt object. The decrease of the number of the inflammatory complications of the broken jaw due to the usage of new algorithms on the basis of informational technologies has been noted.

ABSTRACT

This paper presents a combined method for prevention of deformation of the alveolar process of the jaws after tooth extraction. This method was used in 26 patients of different age. Use of colapol in the KP-3l modification and laser coagulation method in combination with immediate orthodontic treatment led to reduction of the incidence of inflammatory complications. The method is recommended for practical use.
