ABSTRACT

BACKGROUND: Carbon dioxide (CO2) lasers and phenol chemical peels have been used extensively for facial skin resurfacing. OBJECTIVE: The purpose of this study is to compare the histologic effects of phenol chemical peels and CO2 laser ablations. METHODS: Local preauricular areas were treated with lasers on one side of the face and a phenol peel on the other. One day after the procedures, the affected areas were biopsied for histologic evaluation. The areas were biopsied again after 3 months. RESULTS: The initial biopsies demonstrated that the CO2 lasers ablate more superficial skin than the peels, but the 3-month biopsy specimens showed that the zone of new collagen formation was thicker as a result of the phenol peels. CONCLUSION: With the parameters used in this study, phenol peels resulted in the formation of a thicker zone of collagen despite the deeper ablation depth of the laser.

ABSTRACT

We describe a case of pulmonary artery aneurysm in which clinical clues and conventional imaging suggested a lung tumor, and the actual nature of the lesion was discovered at the time of thoracotomy. This case shows the importance of an awareness of this condition in the formulation of a differential diagnosis for a lung mass.

ABSTRACT

A patient with the acquired immunodeficiency syndrome (AIDS) developed severe cyanosis after bronchoscopy (oxygen saturation 34%) from methaemoglobinaemia. This was thought to be due to enhanced absorption of local anaesthetic from the nasopharynx or trachea as a result of candidiasis. The patient responded dramatically to intravenous methylene blue.

A method of resurfacing the nose using tissue of like thickness and color is presented. The midline forehead flap is a reliable source of tissue that can be developed using standard procedures of reconstructive surgery.

ABSTRACT

Patients with esophageal stricture being treated with dilatation, who developed esophageal perforation or local peritonitis and subsequent central nervous system infection, are reviewed. It is suggested that the vertebral venous system may be the route by which the metastatic infection is carried. Physicians caring for patients with esophageal stricture, who are receiving dilatation, should be aware of the possible intracranial complications and should be alert to the first sign of central nervous system abnormality.

ABSTRACT

Ocular muscle imbalance is a common complication in fractures of the zygomaticomaxillary complex. Involvement can be direct; caused by entrapment of either the muscle or tendon in the fracture; or indirect, produced by central or peripheral damage to the cranial nerves supplying the muscles, massive orbital edema or hemorrhage. A case of complete paralysis of the extraocular muscles caused by a critical reduction of the size of the orbit as a result of fracture of the zygomaticomaxillary complex is described.

ABSTRACT

The subject of combined therapy for carcinoma of the head and neck is as perplexing now as it was in its beginning. Low vs high dose pre-operative radiation for carcinoma of the larynx remains a valid question. A total of 92 patients with advanced laryngeal carcinoma were studied at the University of Illinois. The results of treatment for 2 patients receiving planned low dose pre-operative radiation and 30 patients receiving planned high dose pre-operative radiation have been studied, using the actuarial method to calculate survival. Improved survival and decreased complications are evident in the low dose pre-operative group as compared to the high dose pre-operative group.
