ABSTRACT

The purpose of the investigation was to study peculiarities in the parameters of 24-hour arterial pressure monitoring (APM) in pregnant women with various extragenital and obstetric pathologies to define indications to administration and optimization of hypotensive therapy. The results of 24-hour APM of 558 pregnant women with a suspected arterial hypertension (AH) of different origins were matched with peculiarities of pregnancy and its outcome (gestosis, an abortion due to AH complications, serious obstetric complications such as intrauterine fetal death or detachment of placenta). The control group consisted of 41 women without cardiovascular diseases or any complications during pregnancy. The analysis of the results made it possible to offer a classification of 24-hour curves of systolic and diastolic arterial pressure and heart rate based on relations between these parameters. This classification is convenient for clinical application and makes it possible not only to define indications to hypotensive therapy, but also to recommend certain pharmacological group of hypotensive preparations in each particular case. Prognostication of the development of severe gestosis or serious obstetric complications.

ABSTRACT

A dynamic follow-up of 79 pregnant patients with insulin-dependent diabetes mellitus was carried out. Specific fetoplacental hormonal profiles were detected and analyzed in complex with glycemic levels, this permitting the authors obtain new prognostic criteria of fetal and newborn viability. The results helped optimize the policy of following up such patients and the developed prediction regulations allowed preventive measures aimed at improvement of the future baby health status to be carried out before clinical manifestation of fetal intrauterine suffering.

ABSTRACT

The paper deals with early prediction of fetal growth retardation and its severity in a newborn from single ultrasound fetal biometric findings (biparietal head size, chest and belly diameters) at week 20 of pregnancy. The prediction was made by employing the developed varying standards for these parameters as percentile curves and tables. A stepwise prediction of fetal growth retardation was proposed for obstetric in- and outpatient settings, which was presented as an IBM personal computer dialogue program. The positive diagnostic value of fetal growth retardation prediction was found to be 69.7%, its negative value was 86.9%. The paper discusses whether therapeutic measures and pregnancy length affect the efficiency of its prediction and stresses that the prediction is valuable for individual well-grounded tactics for pregnancy management.

ABSTRACT

This population study has examined the incidence and determinants of placental insufficiency. Predictors of placental dysfunction were ascertained. They included somatic diseases, gestational complications, a positive obstetric and gynecologic history and a series of constitutional factors. Populations of primiparae+- and multigravidae at risk of placental insufficiency were identified.

ABSTRACT

The signal of the intracavitary pressure registered through a probe is, as a rule, distorted by fluctuations of the liquid column in the probe. Nomograms enabling the velocity of the sound wave and the own frequency oscillations of the measuring system to be found from the size of the probe, elastic modulus of its material and elasticity of the bulk of the guage are described. In this way a selection of probes to secure high-quality recording of the signal is facilitated. An example of a calculation with the use of the nomograms is given.

ABSTRACT

Dynamics of fibrinolysis in a thin layer of a fibrin-agar plate containing streptokinase undergoing the action of the blood plasma enzymes taken from a healthy person was examined. A continuous recording of the area covering the lysis zone was made. The fibrinolysis process on the fibrin-ager plate is shown to proceed uniformly.
