ABSTRACT

The paper summarises the experience gained in the use of immozymase in tuberculous patients to advance the disease diagnosis and treatment. Immozymase proved valuable in obtaining sputum enriched with M. tuberculosis in patients who were previously considered noncarriers. M. tuberculosis became detectable in 28.3% of them. Immozymase instillations of the urethra before its massage for stimulation of prostatic secretion led to getting secretion in all the patients (0.35 ml, on the average). Immuzymase inhalations were used in the treatment of purulent bronchitis in patients with destructive pulmonary tuberculosis in conservative and preoperative regimens. Postoperative immozymase inhalations promoted prevention of pleuropulmonary complications. Open treatment of the caverns with immozymase after cavernotomy shortens the treatment duration. Positive results were also reached in the treatment of pleural empyema.

ABSTRACT

Administration of immosimase, a proteolytic enzyme complex developed in Russia, provoked active Tb discharge when the latter is occult in cases of destructive tuberculosis. Inhalation of the preparation enriches the patients' sputum with the bacilla. This permitted active tuberculosis diagnosis in additional 28% of the examinees.

ABSTRACT

160 patients with destructive pulmonary tuberculosis of new onset were examined in a tuberculosis hospital. All of them were suspected of being ill with nonspecific bronchial disease after a questionnaire screening. Nonspecific bronchitis was diagnosed in 102 (63.7%) of them. The authors provided the criteria assessing the degree and characteristics of the bronchial tree mucous inflammation by reading endocytopulmonograms. Antituberculosis therapy combined with inhalations (including immozymase) leads to a 13% rise in the efficacy of tuberculosis treatment (cavern closure) and to a 2.5-week reduction in the terms of nonspecific bronchitis cure.

ABSTRACT

A variant of plastic surgery of the residual cavities after cavernotomy of armpit localization had been elaborated. Cicatricial tissue of the residual cavity wall is preserved and only the upper part of the residual cavity wall is used in combination with deeply-lying fatty tissue of the axillary fossa. Plastic closure of the residual armpit cavities after open treatment was successfully performed in 27 patients. The necessary condition for this operation performance is cessation of excretion of bacilli, sputum and discharge from the residual cavity on condition that the patient is in a satisfactory condition.

ABSTRACT

The results of local application of the immobilized proteolytic enzyme immunozymase (USSR) for the open treatment of a cavern in 30 cases are presented. Prolonged continuous proteolysis (for about 100 hours) of the caseous-necrotic content in an open cavern with the help of the enzyme reduces the term of full cleansing of an open cavern to 7-12 days. This provides for earlier plastic intervention, in most of the cases under discussion, aimed at removing the remaining cleansed cavity, thus reducing hospital stay of patients from 6-8 to 2-4 months.

ABSTRACT

The immediate and long-term results of surgical treatment of 162 patients with fibrocavernous pulmonary tuberculosis are described. In addition to resection interventions, the Novisibirsk Tuberculosis Research Institute developed and put into practice collapso-surgical techniques and those of a direct impact on a lung cavern. When operating 5 patients, an artificial therapeutic pneumothorax was applied to the opposite lung. All that ensured a complete clinical wellbeing in 80.8% of the cases judging by the results of their long-term follow-up. Unsatisfactory condition persisted in 8.1% of the cases, and 11.1% of the above patients died. Proper choice of surgical intervention in a specific case made it possible to decrease the associated risk and increase the surgical treatment efficiency to a great extent.

ABSTRACT

The results of osteoplastic thoracoplasty in 44 patients with antisocial behaviour and contraindications to lung resection because of the specific process marked activity are presented. Osteoplastic thoracoplasty is advantageous over the other collapse surgery procedures as there is no need of using therapeutic compressing bandages which makes it possible to discharge a patient from a hospital in case of infringing the hospital regimen. The use of osteoplastic thoracoplasty provided the clinical effect in 29.5 per cent of the patients. In 52.3 per cent of the patients it had a stabilizing effect before lung resection. On the whole, clinical rehabilitation at remote periods was stated in 81.8 per cent of the persons with antisocial behaviour.

ABSTRACT

Patients with a resected lung because of pulmonary tuberculosis and a cavernous process in the single lung were subjected to open surgical treatment on that lung. The authors consider the operation as sparing for patients with low functional respiration reserve.

ABSTRACT

The conditions of the surgical care for patients with pulmonary tuberculosis and in particular patients with chronic cavernous tuberculosis were investigated in some regions of West Siberia. In the last 5 years the activity of the surgical care and the proportion of the operated persons among the contingents of the patients with active tuberculosis increased by 63 and up to 10.5%, respectively. Surgical treatment was mainly applied to new cases of pulmonary tuberculosis since many persons with chronic tuberculosis had contraindications to such treatment due to the disease severity and asocial behaviour.
