ABSTRACT

OBJECTIVE: This study sought to determine the feasibility and acceptability of a facilitated advance care planning (ACP) intervention implemented in outpatient clinics, as perceived by health-care professionals (HCPs). METHODS: Data from seven focus groups (n = 27) and nine semi-structured interviews with HCPs recruited as part of a pragmatic, randomised controlled trial (RCT) were analysed using qualitative descriptive methodology. Components of the intervention included HCP education and training, tools to assist HCPs with patient selection, hardcopy information, and ACP documentation, and specialised nurse-facilitators to support HCPs to complete ACP conversations and documentation with patients and caregivers. RESULTS: Health-care professionals working in tertiary outpatient clinics perceived the facilitated ACP intervention as feasible and acceptable. Health-care professionals reported a high level of satisfaction with key elements of the intervention, including the specialised education and training, screening and assessment procedures and ongoing support from the nurse-facilitators. Health-care professionals reported this training and support increased their confidence and ACP knowledge, leading to more frequent ACP discussions with patients and their families. Health-care professionals noted their ability to conduct ACP screening and assessment in clinic was impeded by large clinical caseloads and patient-related factors (e.g., dementia diagnoses, and emotional distress). Additional barriers to ACP implementation identified by HCPs included poor collaboration, constrained time and clinical space, undefined roles and standardised recording procedures for HCPs. CONCLUSIONS: Facilitated ACP intervention in outpatient clinics is perceived by HCPs as feasible and acceptable. Addressing barriers and tailoring implementation strategies may improve the delivery of ACP as part of tertiary outpatient care.

ABSTRACT

Using the example of a psychological research project in the Andean context, this explorative interview study adds to the current debate about ethical challenges of psychological cross-cultural research projects by being one of the first to address those challenges empirically. Using a multilevel approach, we conducted interviews with three groups of experts (study participants, i.e., Kichwa mothers, research assistants and experts on research in Indigenous communities in Ecuador; N = 10). Overall, the thematic analysis identified that the establishment of horizontal research relationships allows the best adaption to context-specific values (e.g., balance between giving and receiving), norms and societal structures. On the grounds of the analysis, we derived reflective questions for addressing the complex ethical challenges in future cross-cultural projects.

ABSTRACT

Este trabalho foi realizado em uma instituição hospitalar de Curitiba e teve como objetivo desenvolver dinâmicas educativas junto à equipe de enfermagem no sentido de reorientar as ações para a perspectiva cultural do cuidado de enfermagem à puérpera e seu bebê. Utilizou-se como referencial teórico as idéias e alguns conceitos da Teoria da Diversidade e Universalidade do Cuidado Cultural de Madeleine Leininger. Procurou-se demonstrar a importância do cuidado de enfermagem que revela um compromisso cultural com a puérpera e seu bebê, bem como, a importância das ações educativas reflexivas que buscam o desenvolvimento de profissionais livres e conscientes de si mesmos, envolvidos e decidindo por práticas que incluam suas raízes, sua diversidade, onde o compromisso com a saúde possa ser um resgate de sua cidadania levando-os à atitude de promoção da saúde. O trabalho foi desenvolvido por meio de encontros nos quais eram realizadas dinâmicas educativas em saúde promovenso o desenvolvimento dos valores individuais e coletivos no grupo, com o objetivo de incentivá-los à iniciativa, capacidade de organização, capacidade de organização, dinamismo, comunicabilidade, liderança, criatividade, e o compromisso com a efetividade do cuidado cultural de enfermagem como terapêutico e gratificante na obtenção dos resultados esperados. Através de relatos da equipe de enfermagem foi possível detectar o impacto das dinâmicas educativas em saúde a partir da perspectiva cultural do cuidado de enfermagem à puérpera como relevante para a qualidade do cuidado oferecido, assim como a importânmcia do trabalho em grupo favorecendo a aproximação da equipe, motivação e melhoria no relacionamento inter-grupal.
