ABSTRACT

BACKGROUND: Compared to other countries, Germany has the highest number of intensive care unit (ICU) beds, but, despite this, a shortage in ICU care is evident. Currently, little comprehensive data on ICU staffing and on subsequent closure of ICU beds are available. The current survey therefore aimed to systematically investigate the closure of ICU beds. METHOD: A survey was performed among authorized professional trainers in ICU medicine. RESULTS: Overall, a shortage of ICU beds following bed closure was evident in 76% of all ICU floors with 22% reporting daily ICU bed closure. In 47%, two ICU beds were not available. Emergency care was unrestricted in only 18%, while restrictions were reportedly frequent or even constant in 30%. The main reasons for ICU bed closure were the unavailability of ICU nurses (44%) and the co-existing unavailability of nurses and physicians (19%). On average, the nurse/patient ratio was 1:2.5 in the morning, 1:2.6 in the afternoon, and 1:3.1 in the night shift. CONCLUSIONS: ICU bed closure regularly occurs in Germany. The underlying main reason has been identified to be the unavailability of ICU nursing staff. This is suggested to directly interfere with emergency care. For this reason, an action plan is urgently needed.

ABSTRACT

Published data were used to compare the distance decay of similarity in parasite communities of three marine fish hosts: Atlantic cod Gadus morhua, the dab Limanda limanda and the flounder Platichthys flesus in two adjacent areas that differ with respect to the strength of a salinity gradient. In the Baltic Sea, which exhibits a strong salinity gradient from its connection with the North Sea in the west to its head in the north-east, parasite communities in all three fish hosts showed a significant decline of similarity with increasing distance. In contrast, among host populations in the North Sea, which is a fully marine environment, there was no such decline or only a weak relationship. The results suggest that environmental gradients like salinity can be strong driving forces behind patterns of distance decay in parasite communities of fishes.

ABSTRACT

A 66-year-old female patient developed severe Serratia liquefaciens sepsis following vitamin C infusion treatment by a naturopathic practitioner. The clinical course of the infection was characterized by several complications, and the direct costs of the hospital stay amounted to about 40000 Euro. Genotypically identical S. liquefaciens was isolated from the residue of the infusate given to the patient, as well as from the washbasin overflow and from two other infusion bottles. A careful inspection of the dispensing facilities and review of procedures used to prepare the infusate revealed several indications of poor hygiene. However, the source of contamination could not be fully clarified. This case report raises questions about the local facilities and personal qualifications required for naturopathic practitioners to conduct invasive procedures and demonstrates that lapses in hygiene can lead to severe morbidity and high cost.

ABSTRACT

A 42-year-old man was admitted with heavy retrosternal pain lasting 30 min. Electrocardiography showed typical signs of acute anterior myocardial infarction. The patient reported only attacks of coughing for a couple of days, and no serious diseases. The physical examination was normal. Laboratory tests showed a white blood cell count of 45/nl, platelet count of 58/nl, and hemoglobin of 14.4 g/dl. Blood chemistry showed elevated lactic dehydrogenase (413 U/l) but no elevation in creatine phosphokinase or glutamic-oxaloacetic transaminase. Therefore no thrombolysis was administered, but coronary angiography was performed. This showed a long-distance, subtotal thrombotic occlusion of the left anterior descending artery. After percutaneous transluminal coronary angioplasty and implantation of serial stents a normal perfusion of the artery was observed. The patient's blood and bone marrow films revealed acute myeloid leukemia FAB M2. Various conditions can cause a myocardial infarction in leukemias. We discuss the clinical management and the possible reasons for a subtotal thrombotic occlusion of the coronary artery.

ABSTRACT

AIMS: The effects of orthostasis on the morphology of the ventricular-evoked response were investigated. METHODS AND RESULTS: Seven patients (five females; 74.9 +/- 4.6 years) with second- or third-degree atrioventricular block received DDD pacemakers Physios CTM 01 with fractal-coated ventricular leads (both Biotronik, Germany). At their 3-month follow-up sessions, these patients underwent tilt-table tests. Heart rate, mean arterial blood pressure and ventricular-evoked response were measured continuously. Signals were averaged over 15 consecutive beats. Coefficients of correlation between selected ventricular-evoked response parameters and heart rate or mean arterial blood pressure were calculated. Tilt upwards affected heart rate, mean arterial blood pressure and ventricular-evoked response extremes showed non-uniform, patient-specific changes that were not related directly to other parameters. Heart rates significantly increased in six patients following tilt upwards. For all patients, ventricular-evoked response plateau amplitudes immediately increased with tilt upwards. Furthermore, repolarization times correlated with changes in heart rates (r = -0.78). CONCLUSIONS: Monitoring of the ventricular-evoked response during tilt-table testing revealed that the plateau amplitude of the ventricular-evoked response depends on the patient's position; ventricular-evoked response increased in the upright position and decreased in the supine position. Furthermore, repolarization time and heart rate correlated inversely; higher heart rates resulted in a shortening of the ventricular-evoked response.

ABSTRACT

Though rare, cardiac echinococcosis should be included in the differential diagnosis of cardiac tumors, particularly in patients originating from endemic areas or with manifestation of hydatid disease in other organs. Diagnosis and localisation of the cysts is best accomplished with non-invasive methods such as 2-D transthoracic and transesophageal echocardiography, computed tomography and NMR. Excision of the cardiac cyst from the interventricular septum in a 21 year old patient with polyvisceral echinococcosis was performed with cardiopulmonary bypass. Adjunctive medical therapy with mebendazol is being continued for 24 months postoperatively. Twelve months after surgery the patient is asymptomatic without echocardiographic signs of recurrence.
