ABSTRACT
The author draws a parallel between pathogenetic mechanisms of those changes described by N. D. Strazhesko in different forms of pathology of the cardiovascular system and its disorders in different forms of occupational pathology caused by adverse impact on the organism of chemical, physical, biological, and other factors of the working environment and labour process. This permits the methodological principles of diagnosis of the cardiovascular pathologies laid by N. D. Strazhesko to be used in management of occupational diseases.

The authors, on performing a critical analysis of reported data on clinical manifestations of lead intoxication, suggest that the initial manifestations thereof presenting as changes in no other indices but laboratory ones, be regarded as a preclinical stage. Aside from generally recognized concepts of "mild" and "prominent" stages in lead intoxication the authors consider it logical and expediant to introduce a moderately severe stage manifested by vegetosensory polyneuropathy, astheno-vegetative syndrome, dyskinesia of the biliary and intestinal tracts, spastic colitis. It is in this very stage of lead intoxication that the patient, to the author's thinking, is to be assigned to some other job where he/she will not be exposed to lead or other adverse occupational factors. This will, we believe, permit preventing development of a clinically significant stage of lead intoxication manifested by encephalopathy, motor form of polyneuropathy, lead colic, anemia and hepatitis.

A case is reported of an affliction in a worker engaged in the production of enzyme powder-like drugs who in the course of his work over 30 years had been exposed to aerosols containing pepsin and hydrochloric acid. The lung X-ray disclosed pneumothorax, with the cardiovascular shadow and trachea displaced to the right. There were signs of myocardial hypoxia and pulmonary hypertension on the electrocardiogram. The described case suggests to us its occupational causation and permits classifying the form of affection of the bronchopulmonary system as an occupational health problem.

Comprehensive investigations designate to study the working conditions and health status of workers of asphalt-bitumen plants were conducted. Measurable in the breathing zone of workers of basic professions were toxic substances that have a toxic effect on the body (benzene, oil hydrocarbons, toluene, nitric, carbonic oxides, and benz(a)pyrene). There was noted a high incidence of abnormalities of the internal organs, with the cardiovascular and respiratory pathology ranking first in frequency to other disorders. Studies on the allergologic and atopic status showed that as far as the development of the respiratory organ pathology and the external breathing functional shifts are concerned, the immunity status-related specificities of the body are to be considered along with in-plant factors. Measures have been designed aimed to prevent the development of disorders.

Data are submitted on the pattern and levels of occupational morbidity in Ukraine in different branches of national economy, by forms of pathology, with those individuals presenting with occupational health problems having gotten distributed by sex, age, and service duration. The article addresses the issue of significance in the formation of occupational pathology and its levels of different factors and causes (economical, social, medical). The status of the occupational pathology service in the country is considered, with special emphasis being placed on inadequacy of detecting occupational diseases.

The article presents a positive appraisal of efficacy of local application of the drug Fastum-gel (ketoprophen) in a combined treatment of occupational diseases of the locomotor and neuromuscular apparatus. A maximum therapeutic effect was noted in treatment of patients with above medical problems.

Medical definitions of occupational and professionally conditioned pathology are given in the article together with relevant examples. With the view to account appropriately occupational pathology in accordance with its severity classification based on the degree of working capacity loss is presented. Notions of acute occupational disease (intoxication) and professional accident are determined. International list of occupational diseases is considered. Suggestions intended at promotion of safe working conditions are made on perfection of the Ukrainian list.

Results of clinico-functional examination of workers of metallurgical plants confirmed early development and severe course of chronic bronchitis in this category of workers. The polyetiological aspect of chronic bronchitis is suggested. Changes of the pleura and pneumosclerosis which often accompany chronic bronchitis impede the differential diagnosis with interstitial form of pneumoconiosis. Diagnostic criteria of chronic bronchitis in metallurgists should be determined with consideration of concrete working conditions, endogenous factors of risk in the development of the disease and possible variants of the course.

The authors analyzed data of ane examination of 407 user of video display terminals of large computer centers in Kiev and revealed high morbidity of the circulatory digestive and locomotor organs. An analysis of 878 analogous cases in the GDR revealed lower involvement of the mentioned organs. Causes are analyzed. Ways of prophylaxis are outlined.
