ABSTRACT

INTRODUCTION: The efficacy of liraglutide for treating type 2 diabetes mellitus and obesity is well established, but their role in the treatment of weight regain after bariatric surgery remains unclear. METHODS: We searched PubMed, Embase, and Cochrane Library databases in January 2024. A random-effects model was employed to compute mean differences (MD) and events per 100 observations with 95% confidence intervals (CI) for continuous and binary endpoints. Statistical analysis was performed using R software. RESULTS: A total of 16 studies were included and 881 individuals. Patients were mostly female (50%), aged 36 to 55 years, with a mean body mass index (BMI) of 39.4 kg/m2, and had BS surgery 5 years prior. Over a mean follow-up time ranging from 3 months to 4 years, it was observed a statistically significant reduction in BMI (MD - 8.56 kg/m2; 95% CI 3.34 to 13.79; p < 0.01) and a mean reduction in total weight (MD - 16.03 kg; 95% CI 0.03 to 32.02; p = 0.05) after liraglutide use. Additionally, 65% of patients undertaking liraglutide showed total body weight loss (BWL) above 5% (65.8 events per 100 observations; 95% CI 54.96 to 75.20; p < 0.01), while 26% lost more than 10% of total BWL (26.77 events per 100 observations; 95% CI 19.17 to 36.02; p < 0.01). A limitation is a variability between the studies. CONCLUSIONS: Our findings support the use of liraglutide for weight management in patients who experience weight regain after BS. Liraglutide is well tolerated and promotes significant weight loss, providing clinicians with a therapeutic option for this clinical challenge.

ABSTRACT

No Sistema Único de Saúde do Brasil (SUS), os níveis de atenção à saúde se inserem no modelo hierárquico através do sistema de referência e contrarreferência. Em um projeto para atender às demandas represadas do nível primário ao secundário, denominado "Mutirão da Saúde", realizado em Porto Alegre, os médicos neurologistas do Hospital de Clínicas de Porto Alegre, após cada atendimento realizado, responderam a um instrumento para avaliar os encaminhamentos realizados pela rede de atenção primária à saúde (APS). A avaliação foi positiva, mostrando que os encaminhamentos foram realmente necessários em 85 por cento dos casos; entretanto, foi parcialmente contraditória, tendo em vista que 41,7 por cento dos encaminhamentos eram situações clínicas que deveriam ser manejadas no atendimento primário. A avaliação também revelou que 50 por cento dos casos necessitavam de exames complementares. Devido a uma possível regionalização aleatória dos encaminhamentos, o resultado não nos possibilitou uma estimativa apropriada da prevalência por territórios das unidades de APS, informação esta importante para a organização do fluxo de encaminhamentos e planejamento dos recursos alocados pelos gestores, tanto locais quanto municipais.


In the Unified Health System in Brazil (SUS), the levels of health care fall within the hierarchical model with the reference and counter-reference systems. In a project to meet the repressed demands of primary and secondary levels, called "Mutirão da Saúde", held in Porto Alegre, the neurologists of the Hospital de Clinicas de Porto Alegre, after each call made, answered a questionnaire to assess referrals performed by the network of primary health care (PHC). The evaluation was positive, showing that the referrals were really needed in 85 percent of cases; however, it was partially contradictory, given that 41.7 percent of referrals were medical conditions that should be managed in primary care. The evaluation also revealed that 50 percent of cases needed further investigation. Due to a possible randomized regionalization of referrals, the result did not allow us to estimate the appropriate prevalence according to areas of the PHC units, information that is important to organize the flow of referrals and planning of resources allocated by managers, both local and municipal.

ABSTRACT

Topiramate infrequently induces anion gap metabolic acidosis through carbonic anhydrase inhibition on the distal tubule of the nephron--a type 2 renal tubular acidosis. We report on a 40 years old woman previously healthy that developed significant asymptomatic metabolic acidosis during topiramate therapy at a dosage of 100 mg/day for three months. Stopping medication was followed by normalization of the acid-base status within five weeks. This infrequent side effect appears unpredictable and should be given careful attention.

ABSTRACT

Topiramato pode produzir raramente uma acidose metabólica através da inibição da anidrase carbônica no túbulo distal do néfron - acidose tubular renal do tipo 2. Relatamos o caso de mulher de 40 anos previamente saudável que desenvolveu quadro de acidose metabólica assintomática grave, sem outra etiologia identificável, durante uso de topiramato na dose de 100mg/dia por três meses. Este efeito colateral, embora infrequente, parece ser imprevisível e requer atenção cuidadosa.
