ABSTRACT

BACKGROUND AND OBJECTIVE: Retrospective analysis correlating serologic titers of ocular syphilis with posterior segment manifestations. PATIENTS AND METHODS: This study consisted of 40 patients (80 eyes imaged, 68 affected) with positive rapid plasma reagin (RPR) and Treponema Pallidum immunoglobulin G. We collected demographic and presentation data including HIV status, absolute CD4 count, RPR, cerebrospinal fluid-venereal disease research laboratory (CSF-VDRL) test, and retinal zone. We categorized imaging into syphilitic outer retinopathy (SOR), acute syphilitic posterior placoid chorioretinopathy, retinitis/chorioretinitis (RC), and papillitis. Multivariate analysis correlated HIV status, RPR, and VDRL titers with posterior segment findings and zone. RESULTS: Mean age of 42.8 ± 10.7 years, with 70% male patients. Presenting visual acuity (logMAR) 0.66 ± 0.74 did not correlate with RPR, nor was it associated with papillitis, RC, or acute syphilitic posterior placoid chorioretinopathy. Higher RPR (≥ 1:128) positively associated with SOR (P = 0.031) and zone 1 (odds ratio [OR], 1.62; P = 0.02), but negatively associated with zone 2 (OR 0.35; P = 0.005). HIV positivity increased RC odds (OR, 4.45; P = 0.047). CONCLUSION: Higher RPR correlated with SOR and zone 1, whereas HIV positivity correlated with RC. ABSTRACT

Patients who undergo heart valve replacements with mechanical valves need to take Vitamin K Antagonists (VKA) drugs (Warfarin, Nicoumalone) which has got a very narrow therapeutic range and needs very close monitoring using PT-INR. Accessibility to physicians to titrate drugs doses is a major problem in low-middle income countries (LMIC) like India. Our work was aimed at predicting the maintenance dosage of these drugs, using the de-identified medical data collected from patients attending an INR Clinic in South India. We used artificial intelligence (AI) - machine learning to develop the algorithm. A Support Vector Machine (SVM) regression model was built to predict the maintenance dosage of warfarin, who have stable INR values between 2.0 and 4.0. We developed a simple user friendly android mobile application for patients to use the algorithm to predict the doses. The algorithm generated drug doses in 1100 patients were compared to cardiologist prescribed doses and found to have an excellent correlation.

ABSTRACT

Sheath blight (ShB) disease, caused by Rhizoctonia solani, is one of the major biotic stress-oriented diseases that adversely affect the rice productivity worldwide. However, the regulatory mechanisms are not understood yet comprehensively. In the current study, we had investigated the potential roles of miRNAs in economically important indica rice variety Pusa Basmati-1 upon R. solani infection by carrying out in-depth, high-throughput small RNA sequencing with a total data size of 435 million paired-end raw reads from rice leaf RNA samples collected at different time points. Detailed data analysis revealed a total of 468 known mature miRNAs and 747 putative novel miRNAs across all the libraries. Target prediction and Gene Ontology functional analysis of these miRNAs were found to be unraveling various cellular, molecular, and biological functions by targeting various plant defense-related genes. Quantitative reverse transcription polymerase chain reaction (qRT-PCR) was performed to validate the miRNAs and their putative target genes. Out of the selected miRNA-specific putative target genes, miR395a binding and its cleavage site on pentatricopeptide were determined by 5' RACE-PCR. It might be possible that R. solani instigated chloroplast degradation by modulating the pentatricopeptide which led to increased susceptibility to fungal infection.

ABSTRACT

An effective heterojunction with robust charge separation and enormous degradation efficiency is the major task for photocatalyst preparation. In this study, we have prepared the FeCo2O4-loaded g-C3N4 nanosheet by the sol-gel-assisted calcination method for photo-Fenton-like degradation under visible-light irradiation by activating persulfate. The nanocomposite exhibits a higher charge separation efficiency than pure g-C3N4 and FeCo2O4 for the degradation reaction against naproxen drugs. An effective interaction between the nanoparticles increases the degradation efficiency up to 91% with a synergistic index of 73.62%. Moreover, the nanocomposite exhibits a 78% mineralization efficiency against the naproxen pollutant under visible-light irradiation. For practical implementation, the degradation reaction was tested with various pH values, different water sources (DI, lake, and tap water), and light sources (LED (visible)/direct sunlight (UV-visible)). Moreover, the possible degradation mechanism predicted by the elemental trapping experiment and the recycling experiment clearly revealed that the heterojunction composite has a high enough degradation stability.

ABSTRACT

Metastatic breast cancer in the elderly encompasses a group of patients with worse cancer-specific survival and more barriers to care than their non-elderly counterparts. Metastatic breast cancer has a longer natural history than many other common metastatic solid tumors. Palliative care access in this group is thus affected by increased need from (I) more aggressive disease status with more severe symptoms, (II) increased barriers to care, (III) more patients living with the disease than other metastatic solid tumors, and (IV) increased incidence of breast cancer as compared to other solid tumors. This review outlines the rationale for outpatient palliative care, the needs for this specific population of cancer patients, and the barriers that the medical community faces in providing palliative care access for these patients.
