ABSTRACT

The aim of our study was to assess the causes of renal failure in patients with prostatic adenoma (PA) and outcomes after draining of the urinary bladder. A retrospective study comprised 239 PA patients with a serum creatinine level over 120 mcmol/l. Outcomes were assessed after 3-5 day and 2.5 month bladder draining. The Spearman test was used for statistical analysis. We found that age, prostate volume, PA growth pattern, urinary tract infection, concomitant pathology (arterial hypertension, diabetes mellitus, atherosclerosis) do not correlate with a serum creatinine level. The presence of bilateral retention of the upper urinary tract was associated with a higher serum creatinine level. There is a significant correlation between lowering of a serum creatinine concentration after bladder draining and the level of upper urinary tract retention.

ABSTRACT

The article presents results of measuring and dynamics of intraabdominal tension (IAT) by 108 patients with peritonitis. Was proved that in conditions of peritonitis IAT increases more then 10 mm Hg in 77.8% cases, also syndrome of hypertension was marked in 8.3% cases. Was found a fixed statically important correlation connection between sign of IST and frequency of inflammatory process of stomach and after stomach space (p < 0.05), and also between level of IAT and gravity of condition on scale APACHE II, SOFA (p < 0.05). Using of method of laparostomya in combination with intubation of bowel is the most effective method, directed on prophylaxis and treatment of intraabdominal hypertension among ill persons with peritonitis.

ABSTRACT

A great variety of subjective and objective disorders related with chronic prostatitis make difficulties in its diagnosis. The proposed principle of chronic prostatitis classification identifies 33 clinically significant variants of diagnosis by combination of classification grades. The system of fuzzy decisive rules for prediction and differential diagnosis of chronic prostatitis supports decision on the level 0.9 and higher. Use of energetic characteristics of diagnostically significant biologically active points raises quality of prognosis of the onset and exacerbation of chronic prostatitis and helps to select optimal schemes of acpuncture. Application of decision support systems for urological decision on prognosis, diagnosis, prevention and treatment of chronic prostatitis improves quality of medical aid and adequate management of patients with chronic prostatitis.

ABSTRACT

Processes of oxygen activation in monocytes stimulated with adhesion to glass were studied by methods of luminol-dependent and lucigen-independent chemiluminescence. It was shown that monocyte chemiluminescence was caused by cell adhesion to glass surface. Generation of reactive oxygen species at monocyte adhesion to glass was dependent on calcium ion concentration in the medium. The increase in the level of cytosolic calcium, as the extracellular calcium concentration elevated, was accompanied by the activation of phospholipase A2, 5-lypoxygenase and cycloxygenases. Magnesium ions exerted no influence on oxygen activation by cells. Incubation of cells in glucose-free medium, or the addition of glycolysis blocker (2-deoxy-D-glucose) to cell suspension led to a decrease in chemiluminescence intensity. By means of inhibitory analysis, it has been established that processes of oxygen activation are related to arachidonic acid metabolism, and depend on the activity of phospholipase A2.

ABSTRACT

The results of measurement and dynamics of intra-abdominal pressure obtained in 129 patients with acute destructive pancreatitis are presented. It was revealed that in 78% cases the pancreatonecrosis is accompanied by increase in intra-abdominal pressure (IAP); the intra-abdominal hypertension have developed in 9,3% patients. The prolonged intra-abdominal hypertension in the patients with progressive pancreatonecrosis requires the performance of laparotomy, which contributes to decrease in intra-abdominal pressure. The dependence between IAP value and dissemination of pancreatogenic inflammatory process in abdominal cavity and retroperitoneal space as well as between IAP level and condition gravity according to APACHE II scale was revealed.

ABSTRACT

Morphometrical analysis of microtubules (MT) and coated vesicles (CV) was done on electron micrographs of the murine mammary gland sections at the final stages of functional maturation of secretory cells (SC), as well as at different stages of the cell secretory cycle (CSC). The results obtained allow to make the following conclusions: 1) dynamics of changes in the MT count in different subplasmalemmal cell compartments is similar, being directly associated with heterochronical character of CSC processes; 2) the maximum MT count in SC was seen during formation of the secretion product, this increase occurring at the expense of short MT, which are disassembled before and during exocytosis of secretory vesicle (SV) contents; 3) changes in the number of MT and CV are of similar character, which was revealed by morphometrical analysis of SC in the course of maturation. The number of MT and CV in SC increased rapidly and significantly on the 1st and 2nd days after parturition, compared to that observed 1-2 days before parturition. However, the number of MT and CV decreased by the 10th day of lactation, when the mammary secretory activity reached its maximum. Both correlation and regression statistical analyses made during the CSC development point to a linear relation between MT (x) and CV (y) numbers. Regression of y on x is: y = -0.89 + 12.43x. A hypothesis about the possibility of MT participation in CV and SV transport and in the formation of a barrier on the path leading to exocytosis of SV contents is suggested.

ABSTRACT

Gonadotropin-releasing hormone's synthetic analogue (surfagon) injected intraperitoneally exerted a dose-dependent influence on humoral immune response values in rats and on phagocytic and functional activity of neutrophiles in mice. Castration induced a reliable decrease of the immune response and character. It may be concluded that the surfagon effect is not mediated by steroids.
