ABSTRACT

In this work the authors reported the case of a 29-year-old woman, who had been dialysed since 1997 in the course of chronic pyelonephritis caused by vesicoureteral reflux. Due to the detrusor vesica urinariay's hypofunction (confirmed by repeated urodynamic studies) the patient was prescribed Baclofen with a dosage of 2 x 10 mg per 24 hours. In the course of Baclofen's therapy, toxic effects of the drug like weakness, nausea, vomiting and fears appeared during the second day of treatment. They had been increasing since the patient's admission to the Dialysis Centre where she had immediately undergone the planned haemodialysis. After the initial increase of the poisoning symptoms including the loss of consciousness, patient's state of health improved after approximately two hours since the haemodialysis had been performed. After five more days in hospital, the patient was discharged in general good condition (two more planned haemodialyses were performed at the time of treatment).

ABSTRACT

We have reported the case of a 65-year-old woman treated with neuroleptics because of organic delusive disturbances. She was treated for about one month with risperidone (Rispolept) and perazine (Pernazinum). Around ten days before the patient was admitted to this ward, the following had been observed: rise in temperature up to 40 degrees C, progressive weakness and blood pressure (RR) fluctuationis. Despite the applied treatment, the patient died during the fourth of hospitalization on this ward. The diagnosis of neuroleptic malignant syndrome was made on the basis of the clinical picture accomplished whole of biochemical, figurative tests as well as on the conducted differential diagnostics.

ABSTRACT

The examination was carried out in a group of 20 patients with decadent renal failure (14 males and 6 females) from 16 to 51 years of age (average 32 years) undergoing the therapy of repeated haemodialysis (HD) during a period from 1.5 to 72 months. The HD procedure was performed by the use of the cuprophan dialyze (Gambro). The samples were taken on an empty stomach, before HD and then in the 20th 60th and 120th minute of HD. The absolute number of lymphocytes and their subpopulations were determined by means of rosette tests. In the patients with uraemia, the absolute number of lymphocytes and their subpopulations in the peripheral blood was significantly lower than in healthy persons selected appropriately with respect to sex and age. In the initial phase of HD the authors found a significant decrease in the absolute number of the whole population of lymphocytes, and also their sub-populations T, B, TM and an increase in the absolute number of subpopulation TG. These changes were of transient character and became normalized almost completely in the 120th minute of the procedure.

ABSTRACT

Two groups of rabbits were studied, each consisting of 8 males to whom the sterile Hank's solution (group I) or a solution of the granulocytic factors (group II) were given intravenously. The peripheral blood lymphocytes were counted in the samples of blood taken from the femoral artery before injection and 5, 10, 20, 30, 45, and 60 minutes after the injection. No effect of injection was seen on the lymphocyte count in the group I. In the group II the lymphocyte count fell as soon as 5 minutes after the administration of the granulocytic factors with the minimum after 20 minutes. Since that moment the lymphocyte count rose but remained still low, in comparison to the starting point after 60 minutes. All the changes were statistically significant throughout the experiment.

ABSTRACT

An effect of haemoperfusion on plasma oxidizing capacity and erythrocyte sensitivity to oxidation was investigated in patients with the chronic uremia. It was found that plasma oxidizing capacity measured with NTB reduction is more pronounced in patients with the chronic uremia than in normal subjects. Oxidizing capacity of plasma is increased at the beginning of haemoperfusion. This effect is clearly seen during single pass of blood through the column with activated carbon. Erythrocyte sensitivity to oxidation following their suspension in the normal saline with phosphate buffer and measured with ascorbic-cyanate test does not differ significantly in patients with the chronic uremia and healthy subjects and does not change markedly during haemoperfusion.
