ABSTRACT

In conditions of societal transformations, social institutions undergo significant alterations that are manifested by institutional inconsistency and dysfunctionality. The transformation of social professional structure of health care system resulted in change of status and trends of social mobility of social professional groups. The purpose of the study is to typologize social predictors of professional activity of physicians in health care of the Russian Federation. The social economic transformation and reforms of the National health care system of recent decades initiated trends of deprofessionalization in medicine, status inflation and marginalization of professional groups. The phenomenon of professional marginalization of physicians is discussed from perspective of the "concept of status inconsistency" by G. Lensky. In aurhoring typology low income of physicians, financial deprivation, reducing of prestige and relative status inflation of medical profession, professional stress, social frustrations and professional burnout are considered as social predictors of professional activity of physicians. The destructive consequences of status inflation are considered as social threat to preservation of manpower potential of health care of the Russian Federation.

ABSTRACT

In Russia, the oncological diseases significantly impact public health. The study of oncological medical care organization in the Saratov Oblast, that has a number of territorial demographic and medical social characteristics, affecting oncological situation, was carried out. The purpose of the study is to verify systemic factors determining specific characteristics of the oncological medical care organization in the Saratov Oblast. MATERIALS AND METHODS: To elaborate objective comprehension of specific characteristics of the oncological medical care organization in the Saratov Oblast medical and sociological focus groups study was carried out, involving sampling of 12 experts oncologists and health care organizers. THE RESULTS: The original guide scenario developed for the focus group study included expert discussion on trends in oncological medical care organization at the Oblast level and directions of improving continuum of oncological care. The study data analysis was implemented in accordance with specified scenario. CONCLUSION: The focus group study elaborated objective understanding of the specific characteristics of oncological medical care organization in the Oblast and predetermined verification and ranking of systemic factors that significantly impact the continuum of oncological care.

ABSTRACT

The achievements of modern medicine resulted in an increase of number of patients living with chronic diseases that turned out as significant burden for national economics. The oncologic diseases are one of the most widespread ones, determining patient vital prognosis. The modern achievements in medicine determine possibility of optimizing clinical prognosis and chronic course of disease. In modern Russia, the health care system functions in accordance with model of urgent, fragmented medical care that does not meet current needs of patients with chronic diseases. In Russia, actual procedures of medical care of oncologic patients do not consider interdisciplinary continuum scenario. Therefore, the need for its conceptualization. The article proposes model of continuum of oncologic care of patients in the context of an integrated health care system, that presupposes complex interdisciplinary follow-up of patients with chronic disease in situations of disease.

ABSTRACT

The specificity of functioning of health care institutions presupposes high determinacy of efficiency and quality of their organizational behavior and level of motivation of employees. The purpose of study is to determine the degree, determinants of manifestation and means of management of level of in-depth motivation of activities (work) and its outcomes in different professional groups of personnel of medical organization. The study was carried out on the basis of 6 military hospitals using technique of continuous anonymous questionnaire survey of sampling of 583 respondents. The sampling was allocated to such professional groups as "Administration", "Specialists", "Nursing Personnel", "Others" and "Paramedical Personnel". The answers to questionnaire questions presupposed evaluation according 7 points scale named by degree of expression of analyzed characteristics. The "Indicator of Potential of Activity Motivation" was calculated for representatives of professional groups. The character of its relationship with social characteristics of individuality of employees was determined. The professional groups of medical organization are characterized by significant characteristics of manifestation by employees in their work the in-depth motivation of activity. Its highest level is established in respondents from professional groups "Administration" and "Specialists"/ The average level is established in respondents from professional groups "Nursing Personnel" and "Others". The low level is established in respondents from professional group "Paramedical Personnel". The factor is established effecting manifestation of in-depth motivation of activity: exceeding of controlling work of employee over one's autonomy. The attractiveness of implemented work is determined by social characteristics of individuality: need in career development, correspondence of knowledge and skills to implementing work, satisfaction with work context. The elaborated questionnaire can be applied as a tool for analysis of motivation sphere and attitude of health care employees to their professional work.

ABSTRACT

Since XX century, health care is a first-rate social institution. The analysis of tendencies of functioning of institution of health care in modern Russia is presented by the article in methodological plane of the system of social structural functions (AGIL) proposed by T. Parsons. The patient is the main participant of medical organizational process. The activity of other participants of process of organization of medical care is to be focused primarily on satisfaction of needs of patient during medical care rendering. The society implements training of subjects for executing their professional roles that determines professionalization of executed functions. The most important purpose of modern training programs in medical education is determined by leading level of cognition, forecasting and achievement of socially significant aftermaths of future during structuring of educational process. In the context of integrative function the coordination of activities of participants of interaction is implemented. In conditions of actual tendencies of market economy the interaction of participants of the process of medical care rendering and the process of quality control of medical care is developed on the basis of implementation of standards of medical care. In Russia, the institutionalization of health care presupposes cooperation and interaction of subjects of system differing by degree and amount of collaborative work. The latent function (maintenance of value pattern) determines regularity, predictability, stability of functioning of social relationships. The social control supports expedient behavior of participants of process of medical care rendering. The dysfunctional practices of modern Russian health care are considered in the context of concept of effective interaction of participants of medical organizational process targeted to maintenance of rights of patients for accessible and qualitative medical care. As a result of applied analysis, the problems were revealed decreasing effectiveness of functioning of system of organization of medical care of population. The minimization of risks of development of social inequity and increasing of accessibility of qualified medical care are considered in the context of verification and overcoming of dysfunctions during organization of medical care of population.
