ABSTRACT
Report on a diabetic patient with IgA nephropathy whose kidneys were examined by biopsy four times during a period of four years. Morphologically the typical findings were observed: Outstanding deposits of IgA, hypercellularity and increase of matrix within the mesangium. The findings persisted during the period of observation. The differential diagnosis between IgA nephropathy and a slight diffuse glomerulosclerosis in diabetes mellitus is discussed. IgA nephropathy is diagnosed with certainty only by the aid of immunohistology.

In 35 biopsy specimens taken from 21 patients with membranous glomerulonephritis (mGN) the ultrastructure of the glomeruli was investigated with special regards to the degradation of the immune complexes. Electron microscopy allows the most comprehensive assessment of the manifold basement membrane changes. This is of high value for the practical diagnosis.

This paper describes four cases (where rheumatoid arthritis is the basic ailment of the patients involved) of immune complex nephritis and stage I membranous glomerulonephritis, respectively. Treatment was by means of penicillamine. Also discussed are light-microscopical, immunohistological, and electron-optical results.
