ABSTRACT

The results of laparoscopic hernioplasty, using TAPP method and operation of Lichtenstein in 80 patients, suffering incarcerated inguinal hernia, were analyzed. While achievement of similar good and excellent immediate and late results, laparoscopic procedure has several advantages and may be used as a method of choice for treatment of inguinal hernia.

ABSTRACT

The analysis of clinical and laboratory material in patients, suffering extended peritonitis,was conducted. There was established, that application of the sorption-transmembranous dialysis and abdominal cavity sanation guarantees the CD3 lymphocytes protection and the endotoxicosis severity reduction.

ABSTRACT

The results of diagnosis and treatment of 31 patients, suffering abdominal sepsis of various origin, are analyzed. There was elaborated algorithm of postoperative treatment ot the patients. Depending on peculiarities of postoperative tactic of treatment, all the patients were divided into two groups. In 12 patients of the main group in a complex of treatment intraabdominal transmembranic dialysis with a sorption therapy was applied. In 19 patients of a comparative group a transmembranic dialysis was not used. Realization of the elaborated surgical tactic of rational and staged application of intraabdominal sorption-transmembranic dialysis, using Sylics, have permitted to reduce a postoperative complications rate down to 25%, postoperative lethality--to 16.6%.

ABSTRACT

The dynamics of the catecholamines content in the blood serum of the patients, suffering gastroduodenal ulcer, complicated by hemorrhage, was analyzed. The biggest raising of the investigated index level was observed in patients while presence of gastric cancer, complicated by hemorrhage. These changes correlate with the blood loss severity enhancement, the state of unstable endoscopic hemostasis, high activity of the inducible NO-synthase of the peri-ulceral zone mucosa. The data obtained permit to prognosticate the pathological process and to improve the treatment program.

ABSTRACT

The deep statistical analysis of patients treatment with the ulcerous gastroduodenal bleeding set 2 years works of Center of the gastroduodenal bleeding of Dnepropetrovsk is conducted. It is exposed, that wide application of methods of endoscopic haemostasis and endoscopic monitoring allowed substantially to reduce the amount of operations at this category of patients and improve the results.

ABSTRACT

Deep clinico-morphological analysis was performed in patients, suffering gastroduodenal ulcer, complicated by hemorrhage. The most severe inflammatory changes were revealed in gastric antrum mucosa. These changes correlated with features of unstable hemostasis and massive blood loss. The data obtained permit to prognosticate the severity course of pathological process and to improve the program of treatment.

ABSTRACT

The experience of performance of elective and urgent herniotomy intervention in 1548 patients for inguinal hernia was analyzed. There were compared the results of laparoscopic hernioplasty according to TAPP method and Lichtenstein operation. It was established, that achieving similarly good and excellent results, laparoscopic method owes several advantages and can be used as a method of choice in the treatment of inguinal hernia.

ABSTRACT

The profound clinico-morphological and immunological analysis was performed in patients suffering Mallory-Weiss syndrome (MWS). Doing endoscopic and morphological investigations the most dangerous foci of the affection were revealed and there links with the disease course severity and the recurrences rate established. The signs of unstable haemostasis were revealed according to the endoscopic investigation data. Morphological changes of gastric mucosa in patients suffering MWS are similar to those in duodenal ulcer disease. Polymorphism of mucosal microorganisms with E. coli, Streptococcus beta-haemoliticus, Klebsiella pneumoniae, Candida albicans predominance, which severe the disease course and cause the haemorrhage recurrence, was characteristic for MWS in 85% of observations. These causing agents are sensitive to II-III generation cephalosporins and fluoroquinolons in 75% of observations. The complex of conservative treatment must include, besides restorational, untiulcer, antihelicobacter and antibacterial therapy.

ABSTRACT

The results of surgical treatment of 384 patients with gastric ulcer disease within the follow-up period of 1 to 10 and more years have been analysed. Two hundred and fifty patients underwent gastric resection, 85--organ-preserving operation with vagotomy, 49--closure of a perforative ulcer. On the basis of data of clinico-functional investigations, the advantages and disadvantages of the methods of surgical treatment used have been studied.

ABSTRACT

The article presents materials of clinical examination of 407 patients with ulcer disease of the stomach and duodenum complicated by bleeding. The nearest and long-term results of vagotomy and resection of the stomach are shown in comparison. Totally, organ-preserving interventions give more promising results than resection of the stomach especially by the Billroth-II method. After vagotomy, however, recurrent disease was observed in 8.4% and dyspeptic symptoms took place in 17% of the patients examined. By the data of medical labour examination 3-5 years after operation, advantages of vagotomy become increasingly notable, social labour characteristics of vagotomy as a method of treatment is considerably more preferable than resection of the stomach.

ABSTRACT

The state of lipid metabolism was investigated in 72 patients with acute pancreatitis using the method of thin-layer chromatography on "Silufol" plates. It was established that the level of general phospholipids underwent a reduction in destructive pancreatitis. The level of triglycerids reduced sharply after treatment with 5-fluoruracil. The revealed changes were the basis for instituting fatty emulsions and lipid mixtures in the treatment of these patients.
