ABSTRACT

Background: Severe Cognitive Impairment (CI) is a major cause of disability and dependency among the elderly worldwide, and it has a significant impact not only on individuals but also on families, communities, and societies. Early identification and management of CI are critical in geriatric medicine. Prevalence data is often scarce, and this hinders the ground-level implementation of preventive health programs. Objectives: The objective was to find the prevalence of CI and its covariates among the urban geriatric population of Haryana. Materials and Methods: This was a community-based cross-sectional study conducted among 300 geriatric participants residing in an urban area of Rohtak district, Haryana. Hindi Mental State Examination (HMSE) tool was used to assess CI. Data were analyzed using IBM Corp, released 2020, IBM SPSS statistics for windows Ver 20, Armonk, NY. Results: The prevalence of geriatric CI was found to be 27.3% and was found to be significantly associated with age, female gender, scheduled and backward caste (BC), widowhood, and low educational status and is not engaged in any occupation, low socioeconomic status, teetotalism, economic dependency, physical dependency on others, and chronic morbidity. Conclusion: Age, female gender, scheduled and backward caste, widowhood, low educational status, nil employment status, low socioeconomic status, teetotalism, economic dependency, physical dependency on others, and chronic morbidity were found to be important correlates of CI. Further analytical studies can focus on these aspects for an early targeted preventive approach.

ABSTRACT

Accurate air pollution prediction is vital for residents' well-being. This research introduces a secure air quality monitoring system using neural networks and blockchain for robust analysis, precise predictions, and early pollution detection. Blockchain guarantees data integrity, security, and transparency. Goals include real-time air quality data, secure blockchain recording, and enhanced safety through informed decisions. The research integrates blockchain and IoT for short- and long-term air quality monitoring, utilizing an optimized neural network. IoT sensors collect PM2.5, PM10, CO, NO2, and SO2, processed through noise removal and normalization, with feature extraction using N-tuple contrastive learning. Predictions utilize Graph attention-based deep Residual shrinkage Network and Bidirectional long short Term Memory (GRNBTM) categorized into five levels. An adaptive bowerbird algorithm optimizes parameters, reducing computational complexity. Blockchain integration ensures secure, tamper-proof data storage with a lightweight consensus-based algorithm. The GRNBTM model's air quality monitoring performance is extensively simulated and analyzed at 30-min, 2-h, 1-day, and 1-month intervals, demonstrating superior performance over existing techniques.

ABSTRACT

A comprehensive examination of human action recognition (HAR) methodologies situated at the convergence of deep learning and computer vision is the subject of this article. We examine the progression from handcrafted feature-based approaches to end-to-end learning, with a particular focus on the significance of large-scale datasets. By classifying research paradigms, such as temporal modelling and spatial features, our proposed taxonomy illuminates the merits and drawbacks of each. We specifically present HARNet, an architecture for Multi-Model Deep Learning that integrates recurrent and convolutional neural networks while utilizing attention mechanisms to improve accuracy and robustness. The VideoMAE v2 method ( https://github.com/OpenGVLab/VideoMAEv2 ) has been utilized as a case study to illustrate practical implementations and obstacles. For researchers and practitioners interested in gaining a comprehensive understanding of the most recent advancements in HAR as they relate to computer vision and deep learning, this survey is an invaluable resource.

ABSTRACT

Background: Depression is a common mental health disorder that is characterized by loss of interest or pleasure, feelings of guilt or low self-worth, disturbed sleep or appetite, low energy, and poor concentration, insomnia or hypersomnia, and occasionally suicidal thoughts. Apart from biological factors, sociocultural factors also play a key role in development of depression. Objectives: To determine the prevalence of depression in the study population and to study various socio-demographic correlates of depression in the study population. Methods: A community based cross-sectional study was carried out in an urban slum area of Rohtak city during 2016-2017. A total of 600 study participants were selected and interviewed by using PHQ-9 depression scale. The collected data were entered in MS Excel spread sheet and analysed using SPSS software version 20.0. Results: Mean age of the study participants was 37.91 ± 11.75 years. Almost all (97.5 %) study subjects were Hindu. Majority (52 %) belonged to General category. Overall prevalence of depression was found to be 16.2%. The distribution for factors like gender, marital status, education, occupation, socioeconomic status, type of family, living arrangement, smoking habit and death of close relatives were found to be statistically significant with depression (P<0.05). Logistic regression analysis showed that female gender, divorce/separation, illiteracy, unemployment, lower socioeconomic class nuclear family living alone, smoking habit, presence of chronic morbidity and death of close relative in past one year, as independent predictors of depression. Conclusions: The prevalence of depression among adults in an urban slum of north India was found to be 16.2%. Our findings indicate that depression in urban slum is significantly associated with determinants such as gender marital status, education, occupation, SE class, family type, smoking, living arrangement, death of close relative, chronic morbidities like neurological disorders, diabetes and hypertension.

ABSTRACT

Background: The United Nations defines violence against women as "any act of gender-based violence that results in, or is likely to result in, physical, sexual or mental harm or suffering to women, including threats of such acts, coercion or arbitrary deprivation of liberty, whether occurring in public or in private life".[1] Violence can take many forms, including physical, sexual or emotional and varies in its severity. Gender discrimination, norms and practices mean that adolescent girls are likely to experience certain forms of violence, such as sexual violence, at much higher rates than boys. Methods: This cross-sectional study was conducted in a rural block of North India. A total of 500 adolescent girls in the age group of 13-19 years studying in class VIII to class XII in 10 government and private senior secondary schools of the Lakhanmajra block were included in the study. A pre-designed pre-tested semi-structured interview schedule was used. Results: In this study, we found the prevalence of physical, sexual and emotional violence among adolescent girls as 6.6, 5.4 and 5.2%, respectively. The most frequent perpetrator of physical violence was the parent and of sexual violence was the neighbour followed by friends or relatives. Higher emotional violence was experienced by adolescent girls from middle-class families (P < 0.05). The prevalence of physical violence among adolescent girls was maximum in the younger age group 13-14 years (10.2%), followed by 15-17 years (4.0%). This association was found statistically significant (P < 0.05). Conclusions: There are several restrictions on free communication about violence-related topics in our highly conservative society. Blaming the victim is the rule rather than the exception and sexual abuse is usually linked to a loss of virginity and family honour in our patriarchal society. Hence, girls may be more reluctant to disclose their experience of violence.

ABSTRACT

Malignant pleural mesothelioma (MPM) is a rare, aggressive, and incurable cancer arising from the mesothelial lining of the pleura, with few available treatment options. We recently reported that loss of function of the nuclear deubiquitinase BRCA1-associated protein 1 (BAP1), a frequent event in MPM, is associated with sensitivity to tumor necrosis factor-related apoptosis-inducing ligand (TRAIL)-mediated apoptosis. As a potential underlying mechanism, here we report that BAP1 negatively regulates the expression of TRAIL receptors: death receptor 4 (DR4) and death receptor 5 (DR5). Using tissue microarrays of tumor samples from MPM patients, we found a strong inverse correlation between BAP1 and TRAIL receptor expression. BAP1 knockdown increased DR4 and DR5 expression, whereas overexpression of BAP1 had the opposite effect. Reporter assays confirmed wt-BAP1, but not catalytically inactive BAP1 mutant, reduced promoter activities of DR4 and DR5, suggesting deubiquitinase activity is required for the regulation of gene expression. Co-immunoprecipitation studies demonstrated direct binding of BAP1 to the transcription factor Ying Yang 1 (YY1), and chromatin immunoprecipitation assays revealed BAP1 and YY1 to be enriched in the promoter regions of DR4 and DR5. Knockdown of YY1 also increased DR4 and DR5 expression and sensitivity to TRAIL. These results suggest that BAP1 and YY1 cooperatively repress transcription of TRAIL receptors. Our finding that BAP1 directly regulates the extrinsic apoptotic pathway will provide new insights into the role of BAP1 in the development of MPM and other cancers with frequent BAP1 mutations.

ABSTRACT

The aim of this article is to shed light upon formulation of new guidelines of revised orthodontic practices in the post-pandemic era caused due to the novel coronavirus disease 2019 (COVID-19) and also to adapt to the new regulations to prevent further spread of infection. All relevant information pertaining to the area of concern was collected using electronic databases which include Google Scholar, PubMed, Cochrane, orthodontic journals, and health bodies such World Health Organization, the British Orthodontic Society, and the National Institute of Health. Post-pandemic visionary was also contemplated. Data collected through the electronic databases were studied and compiled to provide an overview of the possible modifications which could be employed to prevent crosscontamination during and after the orthodontic therapy in the -post-pandemic era. Refashioning of the dental set-up along with the formulation of new regulations have been elucidated. This review highlights that the post-pandemic orthodontic practice is a divergent layout requiring tedious clinical and environmental modifications. New categorization of patients requiring orthodontic treatments and procedural classification based on generation of aerosol must be taken into consideration. Formulation of new regulations and redesigning the clinical set-up is crucial yet essential.

ABSTRACT

Mesothelioma is a universally lethal cancer lacking effective therapy. The spindle poison vinorelbine exhibits clinical activity in the relapsed setting, and in preclinical models requires BRCA1 to initiate apoptosis. However, the mechanisms underlying this regulation and the clinical implications have not been explored. Here, we show that BRCA1 silencing abrogated vinorelbine-induced cell-cycle arrest, recruitment of BUBR1 to kinetochores, and apoptosis. BRCA1 silencing led to codepletion of MAD2L1 at the mRNA and protein levels consistent with its status as a transcriptional target of BRCA1 Silencing of MAD2L1 phenocopied BRCA1 and was sufficient to confer resistance to vinorelbine. This was recapitulated in cell lines selected for resistance to vinorelbine, which acquired loss of both BRCA1 and MAD2L1 expression. Following ex vivo vinorelbine in 20 primary tumor explants, apoptotic response rate was 59% in BRCA1/MAD2L1-positive explants compared with 0% in BRCA1/MAD2L1-negative explants. In 48 patients, BRCA1 and/or MAD2L1 loss of expression was not prognostic; however, in a subset of patients treated with vinorelbine, survival was shorter for patients lacking BRCA1/MAD2L1 expression compared with double-positive patients (5.9 vs. 36.7 months, P = 0.03). Our data implicate BRCA1/MAD2L1 loss as a putative predictive marker of resistance to vinorelbine in mesothelioma and warrant prospective clinical evaluation.

CONTEXT: Despite so many efforts to control dengue, the disease has a huge impact on the health, well-being, and economy of the population. The key success to control dengue depends not only on services provided by health authorities but also on the awareness of the community about preventing practices and their health-seeking behavior. Hence, the present study was conducted to assess the awareness regarding dengue and its determinants among urban adult population of Rohtak. AIMS: To assess the awareness regarding dengue and its determinants among urban adult population of Rohtak. SETTINGS AND DESIGN: A cross-sectional study. METHODS AND MATERIALS: Data were collected using a pretested, semi-structured interview schedule from 210 participants which included information on the demographic profile of the subject, awareness about the symptoms, transmission, vector behavior in terms of biting time and breeding places, preventive measures and treatment of dengue fever, and responsibility of vector control. Cumulative awareness was calculated on the basis of correct answers provided. STATISTICAL ANALYSIS USED: Categorical data were presented as a percentage. Pearson's Chi-square test was used to evaluate differences between groups for categorical variables. RESULTS: The level of awareness was better among males (statistically significant). The difference in the level of awareness with respect to education, occupation, and the socioeconomic class was found highly statistically significant. CONCLUSIONS: Awareness regarding dengue and its preventive measures were specifically low in women; mostly housewives, in participants of the lower socioeconomic group, and those who were illiterate.

ABSTRACT

The oxidative activation of alkyl C-H bonds vs arene C-H bonds with Pd(OAc)2 has been found to be generalizable to a number of nucleophilic substrates allowing the formation of a range of hindered quaternary centers. The substrates share a common mechanistic path wherein Pd(II) initiates an oxidative dimerization. The resultant dimer modifies the palladium catalyst to favor activation of alkyl C-H bonds in contrast to the trends typically observed via a concerted metalation deprotonation mechanism. Notably, insertion occurs at the terminus of the alkyl arene for hindered substrates. Two different oxidant systems were discovered that turn over the process. Parameters have been identified that predict, which substrates are productive in this reaction.

ABSTRACT

Analysis of healthcare data becomes a tedious task as large volume of unlabelled information is generated. In this article, an algorithm is proposed to reduce the complexity involved in analysis of healthcare data. The proposed algorithm predicts the health status of elderly from the data collected at health centres by utilizing PCA (principle component analysis) and SVM (support vector machine) algorithms. The performance of proposed algorithm is assessed by comparing it with well-known methods like quadratic Discriminant, linear Discriminant, logistic regression, KNN weighted and SVM medium Gaussian using F-measure. At that point, the pre-prepared information is subjected to the dimensionality decrease process by playing out the Feature Selection errand. So, chosen component analysis are investigated by the proposed work SVM-based enhanced recursive element determination, and its precision is assessed and contrasted with the other customary classifiers, for example, quadratic Discriminant, Linear Discriminant, Logistic Regression, KNN Weighted and SVM Medium Gaussian. Here, we built up a shrewd versatile information module for the remote procurement and transmission of EHR (Electronic Health Record) chronicles, together with an online watcher for showing the EHR datasets on a PC, advanced cell or tablet. So as to characterize the highlights required by clients, we demonstrated the elderly checking system in home and healing facility settings. Utilizing this data, we built up a portable information exchange module in light of a Raspberry Pi.

ABSTRACT

INTRODUCTION: Pregnancy and motherhood is a physiological phenomenon. However, approximately 830 women die from preventable causes related to pregnancy and childbirth every day. Birth preparedness and complication readiness (BPACR) improves preventive behavior and improves knowledge of mothers about danger signs. OBJECTIVE: The objective of the study is to assess the status and sociodemographic determinants of BPACR among the women who have experienced motherhood recently. MATERIALS AND METHODS: The study was carried out among 200 women in a rural block of Haryana over a period of 6 months. The tool used to collect data was adapted from survey tools of Johns Hopkins Program for International Education in Gynaecology and Obstetrics/Maternal and Neonatal Health Program. RESULTS: BPACR index came out to be 66.93 and 58.5% women were well prepared for BPACR. Education and occupation of participants, education of participant's husband, socioeconomic status, and caste were found to be significantly associated with BPACR. CONCLUSION: BPACR is a comprehensive strategy to ensure safer pregnancy and motherhood. Providing educational and skill acquisition opportunities for rural women for their empowerment and increasing their role in decision-making are imperative in order to improve BPACR and promote utilization of skilled attendants at every delivery.

OBJECTIVES: Medication errors continue to contribute substantially to global morbidity and mortality. In the context of the recent launch of the World Health Organization's (WHO) Third Global Patient Safety Challenge: Medication Without Harm, we sought to establish agreement on research priorities for medication safety. METHODS: We undertook a consensus prioritisation exercise using an approach developed by the Child Health and Nutrition Research Initiative. Based on a combination of productivity and citations, we identified leading researchers in patient and medication safety and invited them to participate. We also extended the invitation to a further pool of experts from the WHO Global Patient Safety Network. All experts independently generated research ideas, which they then independently scored based on the criteria of: answerability, effectiveness, innovativeness, implementation, burden reduction and equity. An overall Research Priority Score and Average Expert Agreement were calculated for each research question. FINDINGS: 131 experts submitted 333 research ideas, and 42 experts then scored the proposed research questions. The top prioritised research areas were: (1) deploying and scaling technology to enhance medication safety; (2) developing guidelines and standard operating procedures for high-risk patients, medications and contexts; (3) score-based approaches to predicting high-risk patients and situations; (4) interventions to increase patient medication literacy; (5) focused training courses for health professionals; and (6) universally applicable pictograms to avoid medication-related harm. Whilst there was a focus on promoting patient education and involvement across resource settings, priorities identified in high-resource settings centred on the optimisation of existing systems through technology. In low- and middle-resource settings, priorities focused on identifying systemic issues contributing to high-risk situations. CONCLUSIONS: WHO now plans to work with global, regional and national research funding agencies to catalyse the investment needed to enable teams to pursue these research priorities in medication safety across high-, middle- and low-resource country settings.

Introduction BRCA1 associated protein-1 (BAP1) is a key tumor driver in mesothelioma and a potential biomarker predicting response to several targeted therapies in clinical testing. Whether it also modulates response to cytotoxic chemotherapy is undetermined. This study used retrospective response analysis of BAP1 expression in archival tumor biopsies taken from patients in the MS01 trial (NCT00075699). We aimed to determine if BAP1 expression correlated with overall survival within the three treatment arms in this trial, namely active symptom control (ASC); ASC plus mitomycin, vinblastine and cisplatin (MVP); and ASC plus vinorelbine. Materials and methods We used immunohistochemical analysis of tumor samples from the MS01 trial to identify subgroups with and without nuclear BAP1 expression. We performed correlative analysis of clinical characteristics (age at diagnosis, sex and histological subtype) and overall survival within treatment arms with nuclear BAP1 expression. Results 89 tumor samples from the 409 patients originally in the trial were available for analysis. Of these, 60 samples harbored a positive internal control, in the form of positive staining of inflammatory cells for BAP1, and were carried forward for analysis. Correlative analysis suggested no significant association between loss of nuclear BAP1 expression and age at diagnosis, sex and histological subtype. Kaplan Meier survival analysis revealed a small, though non-significant, overall survival disadvantage associated with BAP1 expression in tumors from patients treated with vinorelbine. Discussion This exploratory analysis suggests BAP1 expression may modify response to vinorelbine in MPM, possibly due to prevention of mitotic microtubule formation. We suggest ongoing and planned clinical studies of vinorelbine in MPM assess BAP1 expression as a predictive biomarker of response.

ABSTRACT

Malignant mesothelioma (MM) is poorly responsive to systemic cytotoxic chemotherapy and invariably fatal. Here we describe a screen of 94 drugs in 15 exome-sequenced MM lines and the discovery of a subset defined by loss of function of the nuclear deubiquitinase BRCA associated protein-1 (BAP1) that demonstrate heightened sensitivity to TRAIL (tumour necrosis factor-related apoptosis-inducing ligand). This association is observed across human early passage MM cultures, mouse xenografts and human tumour explants. We demonstrate that BAP1 deubiquitinase activity and its association with ASXL1 to form the Polycomb repressive deubiquitinase complex (PR-DUB) impacts TRAIL sensitivity implicating transcriptional modulation as an underlying mechanism. Death receptor agonists are well-tolerated anti-cancer agents demonstrating limited therapeutic benefit in trials without a targeting biomarker. We identify BAP1 loss-of-function mutations, which are frequent in MM, as a potential genomic stratification tool for TRAIL sensitivity with immediate and actionable therapeutic implications.
