ABSTRACT

The results of surgical treatment of 293 patients with postbulbar duodenal ulcers (PBU) have been analysed. It was established that PBU in the majority of cases are associated with several complications of the ulcer, the main of which being penetration of the ulcer into the pancreatic head. Comparing the results of resections of the stomach and organ-saving procedures in PBU, the author suggests that organ-saving methods in combination with vagotomy have some advantages over resectional methods.

ABSTRACT

Laparoscopic decompression of the gallbladder in elderly and senile patients having acute cholecystitis and in patients with severe coexistent diseases allows arresting the acute inflammatory process in the gallbladder, reducing the amount of forced operations which are extremely dangerous for this contingent of patients. Sanitation of the gallbladder with the electrolytic silver solution and irradiation of the bladder mucosa with laser is highly effective. Microcholecystostoma performed with a catheter having a diameter not less than 2.5 mm maintains adequate decompression of bile ducts and allows performing complete examination of them.

ABSTRACT

The authors treated 106 patients with retroampullary duodenal ulcer. Resections were performed in 16, the organ-preserving operations--in 89 patients. In 58 (64.8%) cases, the duodenoplasty with the different types of vagotomy and preservation of the pyloric sphincter using the techniques developed in the clinic was performed.

ABSTRACT

Results of rightsided hemicolectomy performed in 23 patients using the compression suture with the help of apparatus AKA-2 and apparatus NZhKA-60 supplied with silicon paddings are described. The use of compression sutures have considerably shortened the time of formation of anastomosis and large intestine stump and allowed to standardize the technical methods of the operation. The ideal union of the sutured intestinal walls, antiseptic condition of the operation, high hermetic property facilitated the healing of anastomoses and the stump by first intention. The proposed method gives better functional results due to advantages of the compression method of joining the tissues.

ABSTRACT

In diagnosis of acute pancreatitis of special importance is determination of the activity of transamidinase, phospholipase A, lipase, concentration of copper, calcium and triglycerides in blood serum as well as laparoscopy with biopsy and local thermometry of the pancreas. The dosage of 5-fluoruracil in the complex treatment of patients should be differentiated corresponding to the form of the disease: 3 mg/kg in the endomatous form, 5 mg/kg in the destructive form. The general lethality was 1.4%. In conservative treatment there were no lethal outcomes.

The authors report on the diagnostic value of the reaction of morphological dissociation of the coagulated blood, which, in addition to other clinico-rentgen-laboratory data, can be used as an additional and helpful hematological test for differential diagnosis in a mechanical jaundice of various geneses (obstructive jaundice, hepatocirrhosis, malignant tumors).
