ABSTRACT

A report is presented of five aged patients with hemorrhagic colon ulcer, which was strongly suspected to be a side effect of non steroidal anti-inflammatory drugs (NSAID). All patients were suffering from orthopedic diseases and NSAIDs were administered for pain: Zaltoprofen for one patient and slow-releasing diclofenac for the other four. Four patients had being treated underlying diabetes mellitus and three of them were being treated with sulfonylurea. Appetite loss was the earliest symptom, 1-2 weeks after administration of NSAID began. Diarrhea occurred 1-2 weeks after appetite loss, and finally hemorrhagic stool developed 1-2 weeks after that. Acute gastric mucosal lesion, hemorrhagic colon ulcer and colitis were diagnosed in all patients by emergency gastro-duodenocolonoscopy. NSAID and oral diet were ceased, and intravenous hyperalimentation was instituted when the patients revealed severe anemia due to bleeding. All patients could take an oral diet after a few weeks. In conclusion hemorrhagic colon ulcer must be prevented in patients treated with NSAID especially those who are aged and have a history of diabetes mellitus.

ABSTRACT

The clonal growth of gastric carcinomas was investigated immunohistochemically in C3H<==>BALB/c chimeras using a strain specific antibody. C3H, BALB/c and chimeric mice were given N-methyl-N-nitrosourea 0.5 mg/mice once a week for a total of 10 times by intragastric intubation and observed until week 50. In normal gastric mucosa of the chimeras, each gland was composed entirely of C3H strain specific antigen (CSA)-positive or -negative cells and no mixed glands were found. Cells of all adenomatous hyperplasias and adenocarcinomas in chimeric mice were, in each case, homogeneous for one or other of the parental types, while comprising both surface mucous cell and pyloric gland cell forms. The results clearly suggest that individual cancers are derived from single cells with multi-potential activities and that cellular differentiation of gastric cancer cells occurs secondarily.

ABSTRACT

An operation is described in which a free vascularized fibular graft with a peroneal cutaneous flap is used for tibio-talo-calcaneal fusion and simultaneous skin coverage in treatment of comminuted open fractures of the talus and the calcaneus involving large skin loss. Nine months postoperatively, the tibio-talo-calcaneal arthrodesis was successful, with good coverage of the skin defect on the medial aspect of the sole. One year 7 months postoperatively, the patient is free of pain and able to walk with full weightbearing on the foot.

ABSTRACT

Synovial osteochondromatosis of the foot is a rare disorder. We present the first report of synovial osteochondromatosis of the intermetatarsal/tarsometatarsal joints. When a mass develops in the sole of the foot and heterotopic ossifications are revealed radiographically, synovial osteochondromatosis should be considered in the differential diagnosis.
