ABSTRACT

A sparing endoscopic establishment of gastric fistula was developed by the authors. The procedure does not require any special kits for endoscopic gastrostomy. Pezzer 36 Fr catheter is used as a gastrostomy tube. The diameter is sufficient for not only fluids, but relatively solid substances. At the same time, the tip of the Pezzer catheter forces the anterior wall of stomach against the front abdominal wall, preventing leakage and loss of the tube. Thus, the developed method of endoscopic gastrostomy is modern ind minimally invasive. It is efficient for the recovery and long-term preservation of enteral nutrition.

ABSTRACT

The use of the improved puncture needle was evaluated on the basis of 126 biopsies of thyroid nodular lesions. Benign lesions were diagnosed in 114 (90,5%) patients, malignant tumors - in 10 (7,9%) cases. The method proved to be highly effective and provided correct preoperative diagnosis in 96,8% of patients with nodular goiter. Sensitivity of thin-needle aspiration thyroid biopsy was 100%, specificity - 99,95%.

ABSTRACT

Results of surgical treatment of 192 patients with toxic goiter are analyzed. A new method of thyroid resection, based on the intraoperative morphofunctional evaluation of thyroid activity, was performed in all cases. The optimal volume of the reserved thyroid was determined by the size of the patint's little finger nail bone. The individual tactics of the extent of resection allowed to decrease the postoperative hypothyroidism and toxic goiter recurrence to 0,5%.
