ABSTRACT

This study aimed to determine the general labor well-being of Latin American dentists according to sociodemographic characteristics during the COVID-19 pandemic. A cross-sectional study was conducted in a final sample of 2214 participants from 11 countries. A validated online questionnaire on general work well-being was used (data collection period from 1 June to 10 July 2021), containing two dimensions: psychosocial well-being and collateral effects. The sociodemographic characteristics of the dentists and their perception of the economic impact of the pandemic were also recorded. A multivariate linear regression analysis was performed (hierarchical regression model) to evaluate the joint effect of the explanatory variables on labor well-being and the changes in the variance between each model. A score of psychosocial well-being of 233.6 + 40.2 and collateral effects of 45 + 20.1 was found. Psychosocial well-being was associated with sex, country of origin, academic training achieved, type of dental activity, and perceived impact during the pandemic (p < 0.05). Somatization was frequently manifested through back pain (88.2%) and muscular tensions (87.2%). Women, those who worked 41 or more hours and had between 1 to 15 years of professional experience presented a greater collateral effect (p < 0.001). The impact of the COVID-19 pandemic a year and a half after it began on the labor well-being of Latin American dentists was evidenced with important interactions with social characteristics.

ABSTRACT

Costa Rica ha presentado un progreso en los indicadores de salud bucodental en los últimos 10 años, esto gracias a la estructura de su Sistema Nacional de Salud. Los servicios de atención de la salud están divididos en servicio público, privado y otro mixto. La Caja Costarricense de Seguro Social es la entidad que domina el sector público y es el principal, dividido en tres niveles de atención escalonados. Este artículo busca exponer la estructura de las instituciones que conforman la vigilancia y atención de las enfermedades bucodentales más prevalentes en la población costarricense. De la misma manera, presenta el progreso de dichas condiciones a través de los últimos años. Se concluye que, a pesar del progreso obtenido, es necesario el fortalecimiento interinstitucional para afrontar los desafíos en la situación de salud bucodental actual.


Costa Rica has presented progress in oral health indicators in the last 10 years due to its National Health System structure. Health care services are divided into public, private and mixed services. The Costa Rican Social Security Fund is the entity that dominates the public sector and is the main one, divided into three tiered levels of care. This article seeks to expose the structure of the institutions that make up the surveillance and care of the most prevalent oral diseases in the Costa Rican population. In the same way, it presents the progress of these conditions through the last years. It is concluded that, despite the progress made, inter-institutional strengthening is necessary to face the challenges in the current oral health situation

RESUMEN La atención clínica del paciente portador de epilepsia (EP) asociada a discapacidad intelectual (DI), representa un reto para el estomatólogo por su difícil manejo conductual. Estos pacientes requerirán de alternativas para poder llevar a cabo un tratamiento odontológico exitoso. La anestesia general (AG), es una alternativa ante la falla de las técnicas de manejo conductual, existencia de compromiso sistémico que lo amerite, acceso médico especializado distante al lugar de residencia o por circunstancias particulares de cada caso. Se presenta el caso de un paciente varón de 27 años, con diagnóstico de EP asociada a DI, de difícil manejo conductual atendido bajo anestesia general en el servicio de Estomatología de Pacientes Especiales de la Universidad Peruana Cayetano Heredia. Objetivo : Este artículo busca presentar una revisión de la literatura y el Tratamiento Odontológico Integral bajo anestesia general de un paciente con de Epilepsia asociada a Discapacidad Intelectual moderada. Conclusiones: Los pacientes con estas patologías de fondo son propensos a presentar deterioro del órgano bucal, por lo que requerirán prestaciones estomatológicas constantes. Por la poca colaboración de este tipo de pacientes, se debe considerar a la AG como alternativa para brindar el TOI en un solo acto operatorio.


SUMMARY The clinical care of patients with epilepsy (EP) associated with intellectual disability (ID) represents a challenge for the stomatologist because of its difficult behavioral management. These patients will require alternatives to carry out a successful dental treatment. General anesthesia (AG), is an alternative to the failure of behavioral management techniques, existence of systemic commitment that warrants it, specialized medical access distant from the place of residence or due to particular circumstances of each case. We present the case of a 27-year-old male patient, with a diagnosis of EP associated with ID, of difficult behavioral management treated under general anesthesia in the Special Patients Stomatology service of the Peruvian University Cayetano Heredia. Objective: This article seeks to present a review of the literature and Comprehensive Dental Treatment under general anesthesia of a patient with Epilepsy associated with moderate Intellectual Disability. Conclusions: Patients with these underlying pathologies are prone to present deterioration of the oral organ, so they will require constant stomatological benefits. Due to the low collaboration of this type of patients, the AG should be considered as an alternative to provide the TOI in a single operative act.
