ABSTRACT

With a purpose to study the state of platelet haemostasis and vasoactive prostanoids system 78 patients with ischemic heart disease aged 28 to 59 years were examined. Among them there were 22 patients with "isolated" hypo-alpha-cholesterolemia and 14 patients who had hypo-alpha-cholesterolemia combined with hypertriglyceridemia (HTG). The data obtained show that hypo-alpha-cholesterolemia is accompanied by platelet disorders aggravation and characterized by activated platelet percentage growth. There is a reverse relationship between alpha-cholesterol plasma level and activated platelet percentage (r = -0.52). There is a direct relationship between high-density lipoprotein cholesterol plasma level and concentration of stable prostacyclin metabolite (6-keto-PGF1 alpha) in plasma (r = 0.64). Hypo-alpha-cholesterolemia in combination with HTG is characterized by more significant disorders in prostacyclin-thromboxane system consisting in an increase of plasma thromboxane system consisting in an increase of plasma thromboxane A2 concentration (p < 0.001) and a decrease of 6-keto-PGF1 alpha plasma level (p < 0.05). An inverse apolipoprotein A-1 plasma level (r = -0.48) has been revealed in the group of patients who had hypo-alpha-cholesterolemia combined with HTG.

ABSTRACT

The examination of 194 patients with coronary heart disease concurrent with clinical manifestations of angina pectoris established that the development of coronary atherosclerosis was accompanied by suppressed prostacyclin (PGI2) synthesis and increased thromboxane A2 (TxA2) leukotriene (LT) synthesis. The activation of synthesis of the vasoconstrictor and proaggregate TxA2 depended on the severity of clinical signs of angina pectoris. The imbalance of PGI2 to- TxA2 metabolite ratios was associated with dyslipidemias and the extent of coronary atherosclerosis. Bicycle ergometry testing resulted in prostanoid imbalance. The determination of the dynamics of prostanoids was used to check the efficiency of antianginal therapy. The concomitant application of inhibitors of thromboxane synthetase and LT synthesis was recommended for secondary prevention of thrombotic complications in patients with coronary atherosclerosis.

ABSTRACT

A study is presented of the reactions of prostacyclin-thromboxane system and functions of thrombocytes to physical loads inducing myocardial ischemia in patients with ischemic heart disease of different age groups. Examined were 62 patients with stable stenocardia of the II and III functional classes. Age of the patients: from 44 to 60 years. It was found that the elderly showed deterioration of the prostacyclin-thromboxane imbalance and a reduction of the sensitivity of thrombocytes to prostacyclin. The should be considered in instituting prolonged drug treatment.

ABSTRACT

Results of treatment of patients with stable stenocardia by means of nitrates indicate that the antianginal effect was parallelled by an increase of the sensitivity of thrombocytes to prostacyclin and level of the plasma metabolite prostacyclin. The hypolipidemic effect of miscleron therapy is accompanied in patients with ischemic heart disease by reduction of the aggregation properties of thrombocytes and level of the metabolite thromboxane A2 in the plasma. The rationality of combined treatment with miscleron and nitrates is substantiated from the position of aimed effect on important links of atherogenesis and ischemic heart disease, namely functional properties of thrombocytes, prostacyclin-thromboxane ratio and level of blood lipids.

ABSTRACT

The authors studied the functional activity of thrombocytes, levels of plasma and thrombocytic cyclic AMP (cAMP) in the dynamics of a veloergometric test in patients with IHD and neurocirculatory dystonia. The revealed signs of hyperactivation of thrombocytes and absence of compensatory response of cAMP in IHD patients against the background of thresholds loads allows to improve the quality of prediction of thrombotic complications and control of treatment efficacy with agents possessing antithrombocytic activity.
