The year 2003 will mark the fiftieth anniversary of the publication of Revista Española de Anestesiología y Reanimación (Spanish Journal of Anesthesiology and Postoperative Recovery). For that reason, it is appropriate to explore some of the details of the journal's early history. In 1954 and 1955 two official journals of anesthesiology co-existed in Spain: Revista Española de Anestesiología, which was the official organ of the Spanish Society of Anesthesiology and Postoperative Recovery, and Hypnos, which was the bulletin of the Association of Anesthesia of the Academy of Medical Sciences of Barcelona. The two publications merged in 1956 as Revista Española de Anestesiología, thereby forming the journal whose name eventually became Revista Española de Anestesiología y Reanimación.

En 1953 un grupo de ilusionados anestesiólogos constituía en Madrid la Asociación Española de Anestesiología y posteriormente celebraba en Granada la I Reunión Nacional de Anestesiología. In 1953 a group of hopeful anaesthetists founded in Madrid the Spanish Anaesthesiology Association and later it was held in Granada the 1º National Meeting of Anaesthesiology. In 2003 we celebrate the 50th Anniversary of this landmark in our history that means one of the most important achievernents of the Speciality of Anaesthesiology, Reanimation and Pain Therapy, and also of Spanish Medicine. For this celebration we call up the historical memory to release from oblivion and to do hornage to so many prominent figures of the Spanish and Aragonese Anaesthesiology, Reanimation and Pain Therapy of such a brilliant and exciting period, the 50´s of the twentieth century (AU)

En el año 2003 se cumplen 50 años desde la aparición de la Revista Española de Anestesiología y Reanimación, con tal motivo es interesante conocer algunas particularidades de su inicio. Durante los años 1954 y 1955 coexistieron en España dos revistas oficiales de Anestesiología: HYPNOS, Boletín Oficial de la Asociación de Anestesiología de la Academia de Ciencias Médicas de Barcelona, y la Revista Española de Anestesiología, Boletín Oficial de la Asociación Española de Anestesiología. A partir de 1956, ambas publicaciones se fundieron en una sola, bajo la denominación de Revista Española de Anestesiología (AU)

An 81-year-old woman with right lower limb ischemia was scheduled for revascularization of a femoropopliteal bypass grafted 10 years earlier. A popliteal blockade, attempted as part of regional anesthesia with the aid of a nerve stimulator, was not achieved because the posterior tibial nerve could not be located. After surgery, the patient mentioned symptoms in the region of the right knee consistent with complex regional pain syndrome (Ducke's stage 3); the symptoms appeared after the first operation and would explain the absence of response to the nerve stimulator. Using a nerve stimulator to facilitate location of the various nerve trunks for anesthesia involves obtaining a motor response to electrical stimulation. The procedure is becoming more and more frequent because of its many advantages over other more traditional methods. However, it may be impossible to locate a nerve for a variety of reasons.

Una paciente de 81 años con isquemia de la extremidad inferior derecha fue programada para una revascularización de una derivación femoropoplítea realizada 10 años antes. Se intentó la realización de un bloqueo poplíteo, como componente de la anestesia regional, con la ayuda de un neuroestimulador pero no se pudo lograr localizar por este método el nervio tibial posterior. Posteriormente a la intervención, la paciente refirió un cuadro compatible con síndrome de dolor complejo regional en estadio 3 de Duke en la región de la rodilla derecha, que se inició posteriormente a la primera intervención, y que explicaba la ausencia de respuesta a la neuroestimulación.El uso del neuroestimulador para facilitar la localización de los diferentes troncos nerviosos en anestesia troncular se basa en la obtención de una respuesta motora tras su estimulación eléctrica. Este procedimiento es cada vez más usado por sus numerosas ventajas sobre otros métodos más tradicionales. Sin embargo, hay diferentes causas que pueden impedir la localización de un tronco nervioso mediante el uso de este aparato (AU)
