ABSTRACT

As Taiwan's society ages, Tribal Cultural Health Stations serve as in situ long-term care centers that are committed to building a long-term care service model for indigenous peoples based on cultural care. The cultural sensitivity, cultural ability, and tribal ability of long-term care planners and professional helpers remains insufficient, making it difficult to achieve the policy goals of "designing for the tribes and for the locals" and "creating a cultural care mechanism". However, based on a foundation of local blood and geo-relationship and through the care expertise and interpersonal network established through long-term tribal cultivation and service, a cultural care mechanism that meets local awareness, local needs, and human trust has been formed by expanding linkages among the resources of all local stakeholders. During the COVID-19 epidemic, this has helped facilitate the recovery of the emotional and physical health of older tribal adults. For example, caregivers have been able to help ease the anxieties among older adults in indigenous communities regarding vaccination, fear of infection, isolation, and interpersonal suspicions. In addition, the positive role of the tribal cultural care mechanism as a social safety valve during the pandemic has been demonstrated.

ABSTRACT

The Taiwan Tilapia is an important aquaculture product in Taiwan. The aquatic by-products generated during Tilapia processing, such as fish bones and skin, are rich in minerals and protein. We aimed to explore the effect of a dietary supplement, comprising a mixture of fermented Tilapia by-products and Monostroma nitidum oligosaccharides as the raw materials, combined with physical training on exercise performance and fatigue. We used a mouse model that displays a phenotype of accelerated aging. Male senescence-accelerated mouse prone-8 (SAMP8) mice were divided into two control groups-with or without physical training-and supplemented with different doses (0.5 times: 412 mg/kg body weight (BW)/day; 1 time: 824 mg/kg BW/day; 2 times: 1648 mg/kg BW/day) of fermented Tilapia by-products and Monostroma nitidum oligosaccharide-containing mixture and combined with exercise training groups. Exercise performance was determined by testing forelimb grip strength and with a weight-bearing exhaustive swimming test. Animals were sacrificed to collect physical fatigue-related biomarkers. Mice dosed at 824 or 1648 mg/kg BW/day showed improvement in their exercise performance (p < 0.05). In terms of biochemical fatigue indicators, supplementation of 824 or 1648 mg/kg BW/day doses of test substances could effectively reduce blood urea nitrogen concentration and lactate concentration and increase the lactate ratio (p < 0.05) and liver glycogen content post-exercise (p < 0.05). Based on the above results, the combination of physical training and consumption of a dietary supplementation mixture of fermented Tilapia by-products and Monostroma nitidum oligosaccharides could improve the exercise performance of mice and help achieve an anti-fatigue effect.

ABSTRACT

BACKGROUND: Breast cancer is currently the type of cancer with the highest annual incidence among women in Taiwan, resulting in a median age of death of 57 years. Nevertheless, the proportion of Taiwanese women with a history of mammographic screening is relatively low. The international literature associates participation in mammographic screening with factors such as age, education level, ethnicity, and previous cancer history. Few such studies in Taiwan have addressed a cross-section sample that is representative of the overall population. PURPOSE: The present study investigated factors associated with non-utilization of mammographic screening in women aged between 50 to 69 years in Taiwan. METHODS: This study used secondary data analysis to investigate data obtained from the 2005 National Health Interview Survey in Taiwan. Researchers used logistic regression analysis to evaluate factors associated with mammographic screening in Taiwanese women based on the Andersen behavioral model of health services use. RESULTS: Only 24.3% of the survey population had received mammographic screening. Results of multiple logistic regression analysis indicated non-utilization of mammographic screening is associated with a relatively low education level, being currently employed, a relatively low average monthly salary, having no additional insurance coverage outside the National Health Insurance, having no physical examination history, and having no history of menopausal hormone replacement therapy use. CONCLUSIONS: Hospitals and health units may use findings from the present study to plan mammographic screening programs. Mammography promotional material should consider the needs of women with lower education levels; screening schedules should be coordinated with employers and made convenient for working women; and promotional materials should target women who have never previously received a physical examination. Enhancing the willingness of women to obtain mammography may reduce the threat of breast cancer to the lives of Taiwanese women.

ABSTRACT

In diagnostic studies, the receiver operating characteristic (ROC) curve and the area under the ROC curve are important tools in assessing the utility of biomarkers in discriminating between non-diseased and diseased populations. For classifying a patient into the non-diseased or diseased group, an optimal cut-point of a continuous biomarker is desirable. Youden's index (J), defined as the maximum vertical distance between the ROC curve and the diagonal line, serves as another global measure of overall diagnostic accuracy and can be used in choosing an optimal cut-point. The proposed approach is to make use of a generalized approach to estimate the confidence intervals of the Youden index and its corresponding optimal cut-point. Simulation results are provided for comparing the coverage probabilities of the confidence intervals based on the proposed method with those based on the large sample method and the parametric bootstrap method. Finally, the proposed method is illustrated via an application to a data set from a study on Duchenne muscular dystrophy (DMD).

ABSTRACT

In the cases with three ordinal diagnostic groups, the important measures of diagnostic accuracy are the volume under surface (VUS) and the partial volume under surface (PVUS) which are the extended forms of the area under curve (AUC) and the partial area under curve (PAUC). This article addresses confidence interval estimation of the difference in paired VUS s and the difference in paired PVUS s. To focus especially on studies with small to moderate sample sizes, we propose an approach based on the concepts of generalized inference. A Monte Carlo study demonstrates that the proposed approach generally can provide confidence intervals with reasonable coverage probabilities even at small sample sizes. The proposed approach is compared to a parametric bootstrap approach and a large sample approach through simulation. Finally, the proposed approach is illustrated via an application to a data set of blood test results of anemia patients.
