ABSTRACT

The Children and Youth Health Service (CYHS) is the socio-paediatric partner of Kindergartens (KG) in public services. Since 1995 a KG-Place has been assured by law for every 3-year old child. Now the CYHS can reach more children than ever before. Primary preventive tasks of the CYHS in KG are: encouraging and organising standardised and systematic health promotion issues such as health education (including healthy nourishment), motion education, accident prevention, speech promotion and prevention of drug addiction.--Secondary and tertiary preventive tasks: stepwise screening of motoricity, speech, figure perception, auditive function, vision and behaviour appropriate to own resources; safeguarding therapeutic exercises; continuous attendance to those children suffering from handicaps or developmental problems who are not or insufficiently medically cared for.--Seven steps are described for realising health promotion actions in the KG.

ABSTRACT

In 1971, the first preventive examination programme for children up to 4 years of age was started in the Federal Republic of Germany; in 1977 followed a completely revised programme. However, even with this altered documentation a longitudinal analysis is not feasible. An effective evaluation should yield the following results: 1. Assessment of the success through differentiated recording of sufficiently early recognized health disorders and treatment effects. 2. Availability of reliable, regional and nation-wide statistics on incidence and prevalence. 3. An estimate of the number of health professionals required. - These aims can be achieved only by a longitudinal record linkage of the data on individuals of a sufficiently large random sample. For this purpose a revised data-sheet is presented. In addition, specific suggestions are made for longitudinal evaluations.

ABSTRACT

Data from 1288 illustrated questionnaires were analysed with regard to the correlation of pre-, peri- and postnatal complications and infant psychomotor development. The information on the infants' case history was obtained through the parents retrospectively. Data on psychomotor development were gained prospectively by means of continuous parental observation. Statistical evaluation showed that correlations among many risk factors in our sample and developmental items decrease already during the first six months of life. That is to say, depending on the length of time subsequent to their pre- and perinatal effects many obstetric complications lose their significance for a given state of psychomotor development as well as for prognosis. Furthermore, one should consider that even prediction through recent and current risk factors may often only be of short-term value.
