ABSTRACT

The aim of this study is to assess outcomes of a lateral surgical approach for proximal interphalangeal joint arthroplasty with NeuFlex(®) silicone implants for primary degenerative osteoarthritis. We reviewed 51 arthroplasties performed on 43 patients. The mean follow up period was 36 months (range 18-42). The average preoperative range of motion (ROM) was 38°. The average postoperative ROM was 63°. In 21/51 (41%) cases, there was an average axial deviation of 17° (range 10-30°). VAS and Quick DASH scores improved. In 5/51 (10%) cases, further surgery was required. Our lateral approach seems to be effective and minimally invasive, providing adequate exposure. Contralateral incision and contralateral ligament reinforcement should be performed, whenever necessary, to improve lateral stability.

ABSTRACT

Cancer cell resistance to kinase inhibitors and targeted agents, acquisition of a multidrug-resistant (MDR) phenotype and/or intrinsic resistance to apoptosis prevent effective treatment in about 50% of solid cancers in adults, and the percentage is even higher in children. Glycyrrhetinic acid (GA) and some of its derivatives may offer hope in combating cancer types associated with poor prognoses. Some GA derivatives are indeed able to target both the proteasome and peroxisome proliferator-activated receptors (PPARs), two proteins that play major roles in cancer cell biology but are not related to MDR and/or apoptosis-related resistance phenotypes.

ABSTRACT

Penile verrucous carcinoma is a rare tumor. Verrucous carcinoma is thought by most to represent a well-differentiated or low-grade squamous-cell carcinoma. The term Buschke-Lowenstein tumor and giant condyloma have also been used to identify this histological lesion. A review of literature points to the role of human papillomavirus infection. The authors report a case of a penile verrucous carcinoma in a circumcised man.

ABSTRACT

In this paper, we comment on a patient who consulted us because of his "lower backpain" together with the appearance of a small swelling at the left side. Anamnesis and clinical examination were suggestive and further simple diagnostic methods confirmed the exceptional diagnosis of a "lumbar hernia of Petit". We describe the retroperitoneoscopic approach of this hernia, its reduction and the fixation of a polypropylene mesh at the surrounding structures with a Tacker. This approach provided a good postoperative comfort, a shorter hospital stay and an early recovery of autonomy and activity. Furthermore, we give a review of the literature concerning lumbar hernias and the evolution of the different reconstruction techniques.

ABSTRACT

Morton's neuroma is a frequent cause of pain in the forefoot that commonly occurs in the third intermetatarsal space. It is a type of nerve compression syndrome which involves the common digital plantar nerves. The diagnosis is typically made on the basis of history and careful physical exam. Imaging procedures may be useful to diagnose an atypical case or postoperative recurrence. Conservative treatments are successful in most cases, but when they fail surgery may be considered.

ABSTRACT

Emphysematous cholecystitis is a relatively rare variant of acute cholecystitis with infection by gas-producing organisms. Diagnosis involves the demonstration of gas within the lumen or wall of the gallbladder by ultrasound or CT scan. In contrast to acute cholecystitis, emphysematous cholecystitis occurs more commonly in elderly and diabetic patients, and is frequently associated with perforation and death. We report here a case of a 75-year old man who developed emphysematous cholecystitis.

ABSTRACT

The authors report a case of mucinous adenocarcinoma of the urachus treated with partial cystectomy. The urachal cancer is an uncommon clinicopathologic entity associated with a poor prognosis. Anatomical considerations, clinical features and treatment are discussed.

ABSTRACT

Combined Bennett's fracture and the body of the trapezium are rare. The authors report a case in which closed reduction and internal fixation were performed to stabilize these fractures. The result at 5-month follow-up was good in terms of range of motion and radiological aspect. A literature review and treatment methods are included.

ABSTRACT

Mirizzi syndrome is a partial obstruction of the biliary tree caused by a stone impacted in the cystic duct, with or without development of a cholecystobiliary fistula. Clinical signs are non-specific and suggest at first an obstructive jaundice. We describe a patient with a type I Mirizzi syndrome with a cholecystocolic and a cholecysto-internal biliary fistula. The diagnosis was suggested by ultrasonography and tomodensitometry, and confirmed by endoscopic retrograde cholangiopancreatography. A partial cholecystectomy with a Roux-en-Y hepaticojejunostomy reconstruction was performed. A review of the literature covering its clinical presentation, diagnosis and surgical treatment is presented.
