ROLE OF MICRORNA IN PREDICTING OF ADENOCARCINOMA DEVELOPMENT IN BARRETT'S ESOPHAGUS.

Barrett's esophagus is considered as a predictor of esophageal adenocarcinoma with multistage neoplastic progression at present time. The research assessed an expression of microRNA-21 in 25 patients with different degree of metaplasia and dysplasia of mucous coat of esophagus. The level of expression of microRNA depended on the presence and degree of expression of neoplastic changes of mucous coat of esophagus. The expression rising of microRNA was noted in patients with columnar-celled metaplasia and intraepithelial neoplasia and in case of esophageal adenocarcinoma. The treatment strategy could be determined by the method of estimation of the level of microRNA expression in biopsy material from mucous coat of esophagus in patients with gastroesophageal reflux disease and using other criteria.

ANTIREFLUX RESECTION OF MUCOUS MEMBRANE OF ESOPHAGOGASTRIC ANASTOMOSIS IN TREATMENT OF BARRETT'S ESOPHAGUS: INITIAL EXPERIENCE.

Antireflux resection of mucous membrane of esophagogastric anastomosis (ARMS) was performed on 6 patients with typical symptoms of gastroesophageal reflux disease (GERD). The patients had a short segment of сolumnar-celled metaplasia (1-2 cm) without radiological and endoscopic signs of hiatal hernia. All the patients received medicamentous therapy by antisecretory agents more than 3 years. The operation included the endoscopic resection of2/3 circle of mucous membrane of esophagogastric anastomosis and resection of the area of columnar-celled metaplasia. The results of treatment were assessed during 3-7 months after ARMS. There was noted an absence of clinical manifestations of GERD, regression of inflammatory signs of mucous coat of esophagus in 4 out of 6 patients. Symptoms of GERD remained in 2 patients, although the intensity of signs significantly decreased.

[Comparative analysis of the early postoperative period in patients with postoperative ventral hernias after different nonstrain hernioplastic techniques application].

The article presents the research of the early postoperative period in 64 patients with postoperative ventral hernias who underwent the operation by two different ways. The method of patch applied on the hernial orifice was used in 30 cases. In these cases prosthesis contacted with hypoderm. The modified way of Toskin-Zhebrovskiy was carried out in 34 patients. In these cases the prosthesis was isolated from hypoderm using the hernia bag flap. It was shown, that the frequency of seromas formation was significantly less in comparison with standard method of nonstrain hernioplasty in early postoperative period. Given method allowed shortening the terms of inpatient treatment on 3 days.

The first results of experimental investigation of the potencies of a new domestic electrosurgical apparatus "ESA-MINI-01" are presented. The investigation of the morphological alterations in the stomach wall after using different physical methods of dissection and coagulation has proved the advantages of the new apparatus which has the principle of work based on an original means of forming the outlet parameters.

[Clinico-morphological evaluation of physical methods of dissection and coagulation in stomach surgery (a preliminary report)]. / Kliniko-morfologicheskaia otsenka fizicheskikh sposobov dissektsii i koaguliatsii pri operatsiiakh na zheludke (predvaritel'noe soobshchenie).

The article presents preliminary results of experimental investigations of clinico-morphological alterations in the gastric wall occurring during operations on the stomach performed with using high frequency electrosurgical apparatuses. The authors stress the expedience of using the apparatuses with the working frequency of electric current higher than 3 MHz for the abdominal surgery.
