ABSTRACT

Abdominal trauma is a rare and poorly documented cause of portal vein thrombosis. We report here the case of a patient in whom portal vein thrombosis was diagnosed one month after an abdominal blunt trauma. Post-traumatic origin of thrombosis was confirmed by the negativity of an exhaustive aetiological investigation. Thrombosis involved portal bifurcation and right and left portal veins, but remained asymptomatic. A particularity of this case was that a total regression of the thrombosis was observed under anticoagulation therapy.

ABSTRACT

Fecal concretions adherent to the caecal mucosa and an appendicular mucocele may present as a palpable mass in the right iliac fossa. The authors report a case of a young man with mucoviscidosis who presented with these two signs and they propose a non-invasive diagnostic approach consisting of a plain X-ray of the abdomen, an ultrasound examination and a hypertonic, hydrosoluble contrast enema. Fecal concretions do not produce any specific features on the plain X-ray or on ultrasound but they are seen as a constant filling defect on the contrast enema and they may be evacuated with this technique, thus avoiding the need for surgery. The mucocele is rarely calcified on the plain abdominal film and ultrasound reveals a liquid contents. It should be treated by limited excision of the appendix. The discovery of an appendicular mucocele at any age should lead to the investigation for mucoviscidosis.

For differential diagnosis between obstruction of the lower organ and paralytic ileus, an additional projection to the customary A.S.P. plates is proposed: profile of the rectosigmoid in left lateral decubitus with vertical rays, designed to back up the differential reasoning behind the presence or absence of a rectosigmoid distended by gas.

ABSTRACT

A 44-year-old woman who had been taking oral contraceptives for 10 yr developed abdominal pain and abnormalities of liver tests, which led to the discovery of multiple filling defects on liver scan and of large hepatic zones of decreased density on computerized tomography of the abdomen. These abnormalities were due to sharply demarcated areas of hepatic multilobular hemorrhagic necrosis, associated with marked changes of liver arterioles and small hepatic veins. Since oral contraceptives have been incriminated in the genesis of hepatic vascular alterations, the possible relationship between focal hemorrhagic necrosis of the liver and these agents is suggested.

ABSTRACT

Stenosis of the left coronary artery differs from other atheromatous lesions by the severity of the affection. It is not a rare condition as it occured in 50 out of 750 patients. Clinical and electrocardiographic signs are not truly specific and coronarography is essential for diagnosis even though this procedure carries a certain risk of accidents (2%). At a time when medical and surgical treatment of coronary insufficiency is being compared, stenosis of the left coronary insufficiency is being compared, stenosis of the left coronary artery definitely required operative intervention which improves both functional prognosis and mortality.

ABSTRACT

Transverse sections of the heart and great vessels may be obtained by tomodensitometry using an exposure time short enough to carry out the investigation during apnoea, and an infusion of contrast medium. The definition is good enough for the investigation to be considered as a useful adjunct to the normal investigations in complex congenital cardiac abnormalities. All measurements are instantaneous, including the study of pulmonary density which allows assessment of the pulmonary vascularisation.

ABSTRACT

Clear images of the chambers of the heart and the vessels can be obtained by rapid scanning with computed tomography, associated with rapid perfusion of a contrast medium. A study in 30 normal subjects demonstrated that it was possible to identify and localize the heart chambers and the large vessels in different planes, and to obtain precise measurements of the various diameters. These results could form the basis for etiological studies of the cardiopathies.

ABSTRACT

The value of different radiological examinations in the diagnosis of left ventricular aneurysms following infarcts is assessed using the results obtained in 36 operated cases. By correlating radiological and surgical findings the orientation value of standard examinations can be determined: almost constant cardiomegaly, irregularity of the borders of the heart, and more rarely, myocardial calcification, are significant signs. The two essential examinations are ventriculography and coronarography. Ventriculography can demonstrate anatomical abnormalities (protrusion), or dynamic changes only (hypokinesia, akinesia, systolic expansion). This remarkably reliable examination showed a predominance of anterior and anterolateral aneurisms (87% of cases), and enables definition of the critical cardiac surface area (about 25%) above which the aneurysm is operable. Coronarography shows that the anterior interventricular artery (A.I.V.) is affected in a large proportion (90%) of aneurysms. These two examinations can be used to select those patients suitable for surgery. Results were good and only one death occurred. In the other cases, the disorders of rhythm were reduced and the manifestations of cardiac insufficiency disappeared.
