ABSTRACT

GOAL: Presentation of the first Russian computer program (www.carotidscore.ru) for risk stratification of postoperative complications of carotid endarterectomy (CEE). MATERIAL AND METHODS: The present study is based on the analysis of a multicenter Russian database that includes 25,812 patients after CEE operated on from 01/01/2010 to 04/01/2022. The following types of CEE were implemented: 6814 classical CEE with plastic reconstruction of the reconstruction zone with a patch; 18,998 eversion CEE. RESULTS: In the hospital postoperative period, 0.18% developed a lethal outcome, 0.14%-myocardial infarction, 0.35%-stroke. The combined endpoint was 0.68%. For each factor present in patients, a predictive coefficient was calculated. The prognostic coefficient was a numerical indicator reflecting the strength of the influence of each factor on the development of postoperative complications. Based on this formula, predictive coefficients were calculated for each factor present in patients in our study. The total contribution of these factors was reflected in "%" and denoted the risk of postoperative complications with a minimum value of 0% and a maximum of 100%. On the basis of the obtained calculations, a computer program CarotidSCORE was created. Its graphical interface is based on the QT framework (https://www.qt.io), which has established itself as one of the best solutions for desktop applications. It is possible not only to calculate the probability of developing a complication, but also to save all data about the patient in JSON format (for the patient's personal card and his anamnesis). The CarotidSCORE program contains 47 patient parameters, including clinical-demographic, anamnestic and angiographic characteristics. It allows you to choose one of the four types of CEE, which will provide an accurate stratification of the risk of complications for each of them in person. CONCLUSION: CarotidSCORE (www.carotidscore.ru) is able to determine the likelihood of postoperative complications in patients undergoing CEE.

ABSTRACT

The article presents the results of analysis of widespread of prevalence of additional medical services and voluntary medical insurance of working population of Russia. The quantitative values were received concerning widespread of disposal to Russian workers of various industries of social benefits on partial and full payment of treatment, degree of participation of workers in programs of voluntary medical insurance. It is demonstrated that besides sectorial characteristics the determinants of widespread of granting additional medical insurance are positioned as individual characteristics of workers and such obstacles as length of service, education, salary of worker, type of residence settlement, harmful and dangerous conditions of labor, type of property of enterprise (public/non-public).

ABSTRACT

The article presents the results of study of character of consumption of alcohol products by representatives of different professional groups. The research data was obtained from representative sampling of working population of Russia. On the basis of probit-modeling estimates of possibility of even if once consumption of alcohol were calculated. On the basis of tobit-modeling relative volume of consumed alcohol was evaluated. The results of analysis indicate that professional membership exerts significant but not determining influence on the character of alcohol consumption. The clear-cut age structure of volume consumption of particular alcohol beverages was established. The stable decrease of consumption of alcohol beverages during last years is mainly determined by financial economic crisis and not by measures of antialcoholic campaign.

ABSTRACT

The article provides the results of experimental welding occlusion of arterial and venous vessels 2-12 mm in diameter, including 998 bench tests of resected animal vessels, 256 bench tests of resected human vessels, and also experiments involving 42 live animals (pigs). During the above-mentioned tests 748 vessel obturations were performed. Manual regimen of welding enables the leakproof obturation of the vessel in 66.4% of animals, the result was instable in 22% of cases, no obturation was achieved in 11.6%. Selecting the optimal automatic regimen, we obtained the best results during whelming with the use of the program N.6. The mean strength of the suture obtained in 98.2% of cases was 91,066.7 +/- 215.20 mm Hg. The instable outcome of welding in 1.8% of cases of the use of program N 6 was the result of the use of different welding instruments. The best results (1324 +/- 258) mm Hg were obtained when the electrodes of the welding clamp were compressed at one peg and the force of compression was 91.9 +/- 0.2) MPa. We have established that the short-term impact of increased temperature (from (56 +/- 1.9) to (81 +/- 2.7) degrees C enables denaturation and coagulation of protein molecules, which results in welding connection without coagulation necrosis.

ABSTRACT

Individual measurement data on radiological surveillance of people residing in the territories most radioactively contaminated following the Chernobyl accident are analyzed to provide a comparison of doses from caesium radionuclides to children and adults. The external doses to children does not exceed those to adult inhabitants of the same settlement. By the results of surveillance in 1986 more than 80 thousand inhabitants of the western areas of Bryansk region, a specific activity of caesium radionuclides and corresponding internal radiation dose rate in a number of settlements appeared at children age under 3 years old exceeding the appropriate parameters for adult inhabitants. Among persons evacuated from the contaminated areas, such excess in day of departure from an area averaged 3.8 for surveyed pairs the child-parent.
