ABSTRACT

OBJECTIVE: To study the instance of deep venous thrombosis(DVT) complicated by preoperative lower extremity close fracture. METHODS: Retrospectively analyzed the clinical data of 54 patients with DVT (through color Doppler to final diagnosis) complicated by preoperative lower extremity close fracture from March 2008 to February 2009, and explored the concomitant reason of DVT. There were 23 males and 31 females with age for 23-95 years old. RESULTS: DVT including thrombus of ilio-venae external, thrombus of femoral vein, thrombus of popliteal vein, thrombus of posterior tibial veins and thrombus of soleus venae. The instance of DVT complicated by preoperative lower extremity close fracture: (1) The age of 23 cases (42.59%) more than 70 years. (2) 11 cases (20.37%) combined with syndrome, 2 cases had hypertension, 3 cases had hypertension and diabetes, 2 cases hypertension and cerbral infarction, 4 cases had hypertension, diabetes and coronary heart disease. (3) 10 cases (18.51%) were multi-fractures. (4) 28 cases (51.85%) occurred DVT at 7-14 days after lower extremity close fracture. CONCLUSION: Advanced age, complicating hypertension, diabetes, coronary heart disease, cerbral infarction, multi-fractures may be lead to DVT. Multitudinous DVT occurred at 7-14 days after fracture, as early as possible operation has important significance on prevention DVT to decrease risk of pulmonary embolism.

ABSTRACT

OBJECTIVE: To compare the block characteristics of levobupivacaine and bupivacaine for hypobaric unilateral spinal block. METHODS: Sixty patients undergoing hypobaric unilateral spinal block for lower extremity operations were randomized into 2 equal groups. In one group the patients were asked to lie on the uncompromised side for slow injection of hypobaric bupivacaine solution (0.75% 6 mg, diluted to 2 ml with distilled water) at the rate of 1 ml/15 s through a 27-gauge needle. The patients maintained their position for 15 min before shifting to a supine position. In the other group levobupivacaine was used instead. RESULTS: Statistical study showed that the block characteristics were of no significant difference between the 2 groups (P>0.05) except that the duration of motor block was shorter in levobupivacaine group than in bupivacaine group (P<0.05). Blood pressure or heart rate showed no significant difference between the two groups during operation. CONCLUSION: The block characteristics of hypobaric unilateral spinal block with levobupivacaine or bupivacaine are similar and their effect on circulatory function is slight. Levobupivacaine provides an option in case bupivacaine is not available.

ABSTRACT

OBJECTIVE: To research the effect of Bu Yang Huan Wu Soup (BYHWS) and Tong Mai Soup (TMS) within varieties on D-Di, PLC, Pagt of traumatic fracture. METHOD: 48 cases are choosen, those are femoral-fractured, age between 18 to 45-year-old and in level I - II of ASA. The cases were randomly arranged into 3 groups, 16 cases are in each group. Group I is the controlled one, it isn't used any medicine which affects cruor. Group II is used BYHWS. Group III is used TMS. Group II and III are used medicine for 7 days from the first day in hospital. All patients in 3 groups are checked D-Di, PLC, Pagt in blood in the first day and the seventh day,and their results are compared with the healthy adult's. RESULTS: (1) Comparing the D-Di of all 3 groups with the healthy adult's on the second day, P <0.01. The D-Di value of group I and group III grows up in 7 days, group I's is higher than group III's, but both of them P > 0.01. Value of group II is decreased in 7 days, P < 0.05. (2)PLC of all 3 groups is compared with healthy adult's, P > 0.01. On the seventh day, PLC of group I and group III increase obviously, P < 0.01. Group II's increases to P <0.05. (3) Pagt of 1st day of 3 groups is compared with the healthy adult's, P <0.05. On the 7th day, Pagt increases,in group I P < 0.01, in group II P < 0. 05. in group III P > 0.01. CONCLUSION: Pagt rises after traumatic fracture, the blood is in high-agglomerate situation. Chinese traditional medicine BYHWS and TMS can decrease the blood platelet assemble after trauma a certain extent.
