ABSTRACT

Objective:To understand the results of serum protein electrophoresis in patients with acute brucellosis, and to provide objective indicators for clinical diagnosis and treatment of acute brucellosis patients.Methods:Thirty acute brucellosis patients diagnosed by the Center for Disease Prevention and Control of Donghe District, Baotou City from January to June 2021 were selected as the case group, and 30 local healthy persons who took physical examination during the same period were selected as the control group. Blood samples were collected from the two groups of people, serum was separated and serum protein electrophoresis was performed. Independent sample t-tests were used to compare total protein concentrations, protein fractions (albumin, α1-globulin, α2-globulin, β-globulin and γ-globulin) and albumin/globulin (A/G) ratio. Results:The age of patients in the case group was (52.35 ± 6.74) years old, with 25 males and 5 females. The course of the disease ranged from 0.3 to 2.6 months, with an average of 1.78 months. The age of people in the control group was (52.74 ± 5.47) years old, with 25 males and 5 females. There were no statistically significant difference in age and sex distuibution between the two groups ( P > 0.05). The concentrations of total protein and α2-globulin fractions were not significantly different between the two groups ( t = 0.78, 2.04, P > 0.05, respectively). Compared with the control group, the differences in the percentage of albumin, α1-globulin, β-globulin, γ-globulin in total protein and A/G ratio were statistically significant ( t = 4.78, 4.76, 2.89, 2.20, 3.65, P < 0.05). Conclusions:The percentage of serum albumin in total protein and A/G ratio decrease in patients with acute brucellosis, while the percentage of α1-globulin, β-globulin and γ-globulin in total protein increase. The percentage of albumin, α1-globulin, β-globulin, γ-globulin in total protein and A/G ratio can be uesed as objective indicators for clinical treatment of acute brucellosis.

Objective@#To observe interleukin-6 (IL-6), interleukin-17 (IL-17), and tumor necrosis factor-α (TNF-α) levels in acute and chronic brucellosis before and after antibiotics treatment in order to assess the bone resorption.@*Methods@#Using group design, 36 acute and 36 chronic brucellosis before antibiotics treatment, and 36 acute and 36 chronic brucellosis after antibiotics treatment from the Endemic Disease Prevention and Control Center of Ulanqab City were selected as the research subjects, with 25 local healthy persons as the healthy control group from January to April 2017. Enzyme linked immunosorbent assay (ELISA) method was used to determine the serum levels of IL-6, IL-17, and TNF-α in acute brucellosis, chronic brucellosis and control groups before and after antibiotics treatment.@*Results@#The means of IL-6 [(23.19 ± 2.51), (21.93 ± 2.27), (14.52 ± 2.08) ng/L], IL-17 [(23.55 ± 2.24), (21.43 ± 2.44), (13.69 ± 1.85) ng/L], and TNF-α [(478.97 ± 47.11), (434.67 ± 42.11), (290.19 ± 34.67) ng/L] were significantly different in the acute brucellosis, chronic brucellosis and control groups before antibiotics treatment (F= 113.91, 152.88, 153.49, P < 0.01); compared to control group, the levels of IL-6, IL-17, and TNF-α in acute brucellosis and chronic brucellosis groups were significantly increased before antibiotics treatment (P < 0.05); the levels of IL-6, IL-17, and TNF-α in acute brucellosis group were significantly increased compared to those of chronic brucellosis group before antibiotics treatment (P < 0.05). The means of IL-6 [(17.83 ± 2.31), (18.45 ± 2.15), (14.52 ± 2.08) ng/L], IL-17 [(17.58 ± 2.21), (18.56 ± 2.29), (13.69 ± 1.85) ng/L], and TNF-α [(370.73 ± 41.04), (375.46 ± 44.61), (290.19 ± 34.67) ng/L] were significantly different in the acute brucellosis, chronic brucellosis and control groups after antibiotics treatment (F=25.94, 39.96, 38.30, P < 0.01); compared to the control group, the levels of IL-6, IL-17, and TNF-α in acute brucellosis and chronic brucellosis groups were significantly increased after antibiotics treatment (P < 0.05); the levels of IL-6, IL-17, and TNF-α in acute brucellosis group were not significantly different compared to those of the chronic brucellosis group after antibiotics treatment (P > 0.05).@*Conclusions@#This study shows that before antibiotics treatment, the bone resorption-related cytokines IL-6, IL-17, and TNF-α of acute brucellosis are higher than those of chronic brucellosis. Antibiotic treatment can significantly reduce their levels and may affect bone resorption in patients with brucellosis.

ABSTRACT

Objective The concentrations of Th1/Th2/Th17 cytokines were compared in acute and chronic patients with brucellosis in order to understand the immune characteristics of acute and chronic brucellosis. Methods Using forward-looking design, 35 acute and 35 chronic patients with brucellosis were selected before treatment from the Endemic Disease Prevention and Control Center of Wulanchabu City as the research subjects with 25 local healthy persons as the healthy control group from March to June 2018. Enzyme linked immunosorbent assay (ELISA) method was used to determine the serum levels of Th1 cell cytokines interferon-γ(IFN-γ), tumor necrosis factor-α(TNF-α), and interleukin-12 (IL-12); Th2 cell cytokines interleukin-4 (IL-4), interleukin-6 (IL-6), and interleukin-10 (IL-10);and Th17 cell cytokines interleukin-17 (IL-17). Results There were totally 25 patients in the control group, aged (46.48 ± 9.74) years, 15 males and 10 females;35 patients with acute brucellosis, aged(54.00 ± 7.65) years, 23 males and 12 females; and 35 patients with chronic brucellosis, aged (49.04 ± 8.24) years, 22 males and 13 females. The means of IFN-γ[(280.50 ± 40.48), (462.79 ± 47.94), (431.91 ± 40.39) ng/L], TNF-α[(292.17 ± 31.45), (481.21 ± 43.14), (433.70 ± 41.23) ng/L], IL-12 [(45.13 ± 5.55), (70.74 ± 7.58), (62.56 ± 5.73) ng/L], IL-4 [(383.24 ± 53.98), (606.11 ± 51.86), (550.66 ± 51.56) ng/L], IL-6 [(14.48 ± 2.17), (23.54 ± 2.39), (21.89 ± 2.26) ng/L], IL-10 [(140.48 ± 110.97), (464.49 ± 52.10), (404.73 ± 52.10) ng/L], and IL-17 [(13.71 ± 1.58), (23.79 ± 2.15), (21.37 ± 2.26) ng/L] were significantly different between control group, acute brucellosis group and chronic brucellosis group (F = 126.13, 152.58, 106.81, 139.28, 113.80, 155.86, 152.91, P < 0.05). Compared to control group, the levels of the seven cytokines in acute brucellosis and chronic brucellosis groups were significantly increased before treatment (P < 0.05). The levels of all of the seven cytokines in acute brucellosis group were significantly increased compared to chronic brucellosis group before treatment (P < 0.05). Conclusions There is a difference in the expression of Th1/Th2/Th17 cytokines in patients with acute and chronic brucellosis group. In the acute brucellosis group, the cell immunity induced by Th1 cells and the humoral immunity guided by Th2 cells are stronger than those in chronic brucellosis group.

The means of IL-6 [(17.83 ± 2.31), (18.45 ± 2.15), (14.52 ± 2.08) ng/L], IL-17 [(17.58 ± 2.21), (18.56 ± 2.29), (13.69 ± 1.85) ng/L], and TNF-α[(370.73 ± 41.04), (375.46 ± 44.61), (290.19 ± 34.67) ng/L] were significantly different in the acute brucellosis, chronic brucellosis and control groups after antibiotics treatment (F = 25.94, 39.96, 38.30, P < 0.01); compared to the control group, the levels of IL-6, IL-17, and TNF-α in acute brucellosis and chronic brucellosis groups were significantly increased after antibiotics treatment (P < 0.05); the levels of IL-6, IL-17, and TNF-α in acute brucellosis group were not significantly different compared to those of the chronic brucellosis group after antibiotics treatment (P > 0.05). Conclusions This study shows that before antibiotics treatment, the bone resorption-related cytokines IL-6, IL-17, and TNF-α of acute brucellosis are higher than those of chronic brucellosis. Antibiotic treatment can significantly reduce their levels and may affect bone resorption in patients with brucellosis.

ABSTRACT

Objective To observe the effect of early comprehensive intervention on the clinical efficacy of patients with acute poisoning combined with inhalation pneumonia. Methods One hundred and eighty-seven patients with acute severe poisoning combined with coma [Glasgow coma scale (GCS) ≤ 8] were admitted to the department of emergency intensive care unit (EICU) of Luoyang Central Hospital Affiliated to Zhengzhou University from January 2013 to December 2016. Forty-eight patients treated from January 2013 to December 2014 by conventional and symptomatic therapies were taken as a control group, and 56 cases treated with early comprehensive intervention method from January 2015 to December 2016 were assigned as an experimental group. The differences in the incidences of aspiration pneumonia and respiratory failure, the usage rate of antimicrobial drugs and their combination condition, length of hospital stay, length of stay in ICU and prognosis were compared between the two groups. Results After treatment, the incidence of aspiration pneumonia [10.71% (6/56) vs. 58.33% (28/48)], and the incidence of respiratory failure [8.93% (5/56) vs. 29.17% (14/48)] in the experimental group were significantly lower than those in the control group, the utilization rate of antimicrobial drugs [14.3% (8/56) vs. 62.5% (30/48) ], especially the combined use of triple and above was significantly reduced [0 vs. 10.0% (3/30)], the length of hospital stay (days: 7.80±3.79 vs. 10.21±4.58) and length of stay in ICU (days: 3.48±1.56 vs. 6.56±2.92) were obviously shorter in experimental group than those in control group (both P < 0.05). The clinical cure rate was also significantly higher in the experimental group [91.07% (51/56) vs. 77.08% (37/48), P < 0.05]. Conclusions The early comprehensive intervention for treatment of patients with GCS score ≤ 8 and acute poisoning can reduce the risks of the occurrences of inhalation pneumonia and respiratory failure and improve the medical pharmacy index and the prognosis of patients, thus the method is worthwhile to be applied extensively.

ABSTRACT

Objective To observe the levels of peripheral blood cytokines interferon-γ (IFN-γ)and interleukin-4 (IL-4) in patients with acute and chronic brucellosis before and during treatment,and to understand the differences of two immunocytokines in acute and chronic stage of brucellosis,and the effect of antibacterial therapy on these two cytokines to provide immunological basis for clinical evaluation of the therapeutic effect of brucellosis.Methods Research subjects were 36 pre-treatment acute brucellosis and 36 pre-treatment chronic brucellosis and 36 dur-treatment acute brucellosis and 36 dur-treatment chronic brucellosis,which were selected from Ulanqab Center for Endemic Disease Prevention and Control of Jining City with 25 local healthy persons as healthy controls.The levels of IFN-γ and IL-4 in serum were measured by enzyme-linked immunosorbent assay (ELISA) in patients with acute and chronic brucellosis and control group before and during treatment.Parameters of IFN-γ and IL-4 were analyzed with One-Way ANOVA analysis in pre-treatment and dur-treatment acute brucellosis,chronic brucellosis and control groups.Results The means of IFN-γ [(462.79 ± 47.94),(431.92 ± 40.39),(280.50 ± 40.48) ng/L] and IL-4 [(606.11 ± 51.86),(550.66 ± 51.56),(383.24 ± 53.98) ng/L] were significantly different in the three groups before treatment (F =141.84,139.28,P < 0.05);Compared to control group,the levels of IFN-γ and IL-4 in acute brucellosis and chronic brucellosis were significantly increased before treatment (P < 0.05).The levels of IFN-γand IL-4 in the acute brucellosis were significantly increased compared to those of chronic brucellosis before treatment (P < 0.05).After about ten days antibiotic therapy,the means of IFN-γ [(356.05 ± 43.75),(368.61 ± 35.69),(280.50 ± 40.48) ng/L] and IL-4 [(487.31 ± 51.59),(496.73 ± 48.70),(383.24 ± 53.98) ng/L] were significantly different in the three groups (F =39.57,41.99,P < 0.05).Compared to control group,the levels of IFN-γ and IL-4 in acute brucellosis and chronic brucellosis were significantly increased during treatment (P < 0.05).The levels of IFN-γand IL-4 in the acute brucellosis were not significantly different compared to those of chronic brucellosis during treatment (P > 0.05).Conclusion Different immunological characteristics of cytokines in peripheral blood of patients with acute and chronic brucellosis before treatment have affected the therapeutic effect and clinical outcomes.

ABSTRACT

Objective To investigate the levels of antibody to pertussis toxin (PT) IgG in newborns in Shunyi Women and Children's Hospital of Beijing Children's Hospital in 2016.Methods A total of 419 newborns were enrolled in this study.Umbilical cord blood sample was collected from each subject and detected by enzyme-linked immunosorbent assay to measure the concentration of PT-IgG.Besides,all newborns were followed up to January 31,2017.Chi-square test was used for statistical analysis.Results The detectable rate of umbilical cord blood samples for PT-IgG accounted for 30.1% (126/419).The median antibody level was < 5 U/ml,and the 90th and the 95th percentile were 14.3 and 24.0 U/ml,respectively.No cases of pertussis occurred at the end of follow-up.Conclusions The newborns born in Shunyi Women and Children's Hospital of Beijing Children's Hospital are generally lack of protective PT antibody.

ABSTRACT

Objective We compared peripheral blood nucleotide-binding oligomerization domain-leucinerich repeats containing pyrin domain 3 (NLRP3),cysteinyl aspartate specific proteinase-1 (Caspase-1),interleukin-1β (IL-1β) and IL-18 between acute and chronic brucellosis patients before treatment and revealed their immune characteristics,to find targets for immune intervention of brucellosis.Methods From March to April 2016,42 acute and 42 chronic brucellosis patients were selected from Ulanqab Center for Endemic Disease Prevention and Control as the research subjects,and 20 local healthy persons were selected as healthy control.Brucellosis were diagnosed according to the "Diagnostic Criteria of Brucellosis".Enzyme-linked immunesorbent assay (ELISA) method was used to determine serum levels of NLRP3,Caspase-1,IL-1β and IL-18.Results The expression levels of NLRP3,Caspase-1,IL-1β and IL-18 [(1 264.40 ± 424.74),(1 350.67 ± 468.93),(192.96 ± 61.52),(162.74 ±54.23),(172.44 ± 60.56),(120.10 ± 61.52),(47.23 ± 13.79),(46.68 ± 14.72),(27.71-± 8.71),(202.23 ± 65.24),(169.19 ± 54.33),(108.62 ± 41.39) ng/L] were compared in acute,chronic and control groups,the differences were statistically significant (F =61.96,6.26,16.68,18.31,P < 0.01).Compared to control group,the levels of NLRP3,Caspase-1,IL-1β and IL-18 in acute and chronic groups were significantly increased (P < 0.05);compared to chronic group,the levels of IL-18 in acute group were significantly increased (P < 0.05),and NLRP3,Caspase-1 and IL-1 β levels were not statistically different (P > 0.05).Conclusions This study has showed that the expression of NLRP3 inflammasome in innate immunity is not significantly different between acute brucellosis and chronic brucellosis.The difference of IL-18 levels between acute and chronic brucellosis may affect their immune response.

ABSTRACT

Objective To study the characteristics of liver function damage in brucellosis and provide theoretical basis for clinical treatment.Methods One hundred and thirty-six hospitalized patients with brucellosis from Endemic Disease Prevention and Control Center of Wulanchabu City from January 2011 to October 2012 were selected randomly as the research subjects.Parameters of Alanine transaminase (ALT),Aspartate transaminase (AST),Alkaline phosphatase (ALP),Gamma glutamyltransferase (γ-GT),Total protein (TP),Albumin (ALB),Albumin/globulin (A/G),total bilirubin (TBIL) and Direct bilirubin (DBIL) were determined.The means of liver function parameters with mid-value of all the reference ranges were compared by one sample t test;Parameters of liver function were analyzed in 70 acute and 66 chronic brucellosis by two independent-samples t test.Results The means of ALT,AST,ALP,γ-GT,TP,ALB,TBIL and DBIL ((x):36.18,28.20,95.87,29.29 U/L,65.08,40.84 g/ L and 19.76,5.86 μmol/L) in the 136 samples of brucellosis serum were significantly different from the medians of their reference ranges ((x):20.50,18.50,85.00,24.50 U/L,69.00,43.50 g/L and 11.80,3.40 μ mol/L,t =9.46,9.19,4.55,2.22,-2.71,-4.99,5.58,11.32,all P < 0.05);A/G was not significantly different ((x):2.14,2.00;t =0.94,P > 0.05);Parameters of liver function were not significantly different between acute and chronic brucellosis (all P > 0.05).Conclusions This study confirmed that the liver function of the 136 patients with brucellosis is impaired.The level of the liver function impairment is usually mild and moderate and no difference in acute and chronic brucellosis is found.
