ABSTRACT

Aims: We aimed to explore the role of personality traits between fall and loneliness. Methods: A questionnaire survey was used to investigate falls, the big five personality traits, and loneliness among older people (≥ 60 years old) in China mainland. Results: A total of 4,289 older people participated in the survey. There are significant differences in age, marital status, education level, residence, solitariness, and fall in relation to loneliness among older people. Falls, especially when they occurred one time increase the loneliness of older people. Agreeableness, conscientiousness, and neuroticism were significant mediating effects between falls and loneliness. Conclusion: This study implied that agreeableness, conscientiousness, and neuroticism were meditating factors between falls and loneliness. In the future, we should consider the big five personality traits more to understand loneliness and offer older people interventions for reducing their loneliness. The study design was cross-sectional, so the temporal precedence of mediators and causality could not be tested. Because the data were collected retrospectively, current loneliness is likely to have confounding effects on retrospective recall.

ABSTRACT

OBJECTIVE To evaluate the clinical efficacy and safety of XELOX chemotherapy (oxaliplatin+capecitabine) combined with antiangiogenic agent (apatinib) and immunotherapy (camrelizumab) in patients with inoperable metastatic colorectal cancer (CRC)of microsatellite stable (MSS) type. METHODS Clinical medical records of 40 patients with inoperable metastatic CRC of MSS type treated in Lishui People’s Hospital from January 2020 to January 2021 were retrospectively collected. According to the treatment plan, the patients were divided into control group (20 cases) and observation group (20 cases). Control group was given XELOX+apatinib regimen, while observation group was given XELOX+apatinib+camrelizumab regimen. Every 3 weeks was a treatment cycle, and the treatment lasted for 2 consecutive cycles. The objective response rate (ORR), disease control rate (DCR), progression-free survival (PFS), overall survival (OS) and adverse events (AEs) were recorded for all patients. RESULTS The ORR and DCR of observation group were 65.0% and 85.0%, respectively; and the ORR and DCR of control group were 35.0% and 75.0%, respectively, with no statistical significance between 2 groups (P>0.05). The median PFS of observation group and control groups were 16.0 months and 8.0 months, respectively; and the median OS were 19.0 months and 12.5 months, respectively, with statistical significance between 2 groups (P<0.05). Each patient in both groups had at least one AEs, and the incidences of reactive skin capillary hyperplasia and hyperthyroidism in observation group (40.0%, 20.0%) were significantly higher than those in control group (both were 0) (P<0.05). The incidence of nausea and vomiting in control group (90%) was significantly higher than observation group (10%) (P<0.05). There were 14 cases (70.0%) of patients with grade 3 or above AEs in observation group, and only 5 cases (25.0%) in control group, with statistical significance between 2 groups (P<0.05). However, no severe AEs that could not be tolerated or fatal occurred in the two groups, which could be alleviated after drug withdrawal or treatment. CONCLUSIONS The efficacy of XELOX chemotherapy combined with apatinib and camrelizumab in inoperable metastatic CRC patients of MSS type is comparable to that of XELOX chemotherapy combined with apatinib, but it has certain advantages in ORR, PFS and OS, and controllable safety.

ABSTRACT

This study aimed to evaluate the therapeutic potential of inhibiting protein arginine methyltransferase 5(PRMT5)in cisplatin-induced hearing loss.The effects of PRMT5 inhibition on cisplatin-induced auditory injury were determined using immunohistochemistry,apoptosis assays,and auditory brainstem response.The mechanism of PRMT5 inhibition on hair cell survival was assessed using RNA-seq and Cleavage Under Targets and Tagment-quantitative polymerase chain reaction(CUT&Tag-qPCR)analyses in the HEI-OC1 cell line.Pharmacological inhibition of PRMT5 significantly alleviated cisplatin-induced damage to hair cells and spiral ganglion neurons in the cochlea and decreased apoptosis by protecting mitochondrial function and preventing the accumulation of reactive oxygen species.CUT&Tag-qPCR analysis demonstrated that inhibition of PRMT5 in HEI-OC1 cells reduced the accumulation of H4R3me2s/H3R8me2s marks at the promoter region of the Pik3ca gene,thus activating the expression of Pik3ca.These findings suggest that PRMT5 inhibitors have strong potential as agents against cisplatin-induced ototoxicity and can lay the foundation for further research on treatment strategies of hearing loss.

ABSTRACT

Production internship is an important teaching tache for undergraduate students to carry out engineering training by using professional skills, and it is a key starting point for fostering application-oriented talents in biotechnology. The Course Group of 'production internship of biotechnology majors' of Binzhou University is investigating application-oriented transformation for local regular colleges and universities, as well as fostering high-level application-oriented talents. By taking green fluorescent protein (GFP) polyclonal antibody as an example, the reform and practice on teaching content, teaching mode, assessment method, continuous improvement of curriculum were carried out. Moreover, the characteristics of the Yellow River Delta-Binzhou Biotechnology & Pharmaceutical Industrial Cluster were taken into account to intensify academic-enterprise cooperation. On one hand, this Course Group designed and rearranged the course contents, carried out essential training through online resources and platforms such as virtual simulation, and recorded, tracked and monitored the progress of production internship through practical testing and software platforms like 'Alumni State'. On the other hand, this Course Group established a practice-and application-oriented assessment method in the process of production internship and a dual evaluation model for continuous improvement. These reform and practices have promoted the training of application-oriented talents in biotechnology, and may serve as a reference for similar courses.

ABSTRACT

Using chemoproteomic techniques, we first identified EIF2AK2, eEF1A1, PRDX3 and VPS4B as direct targets of berberine (BBR) for its synergistically anti-inflammatory effects. Of them, BBR has the strongest affinity with EIF2AK2 via two ionic bonds, and regulates several key inflammatory pathways through EIF2AK2, indicating the dominant role of EIF2AK2. Also, BBR could subtly inhibit the dimerization of EIF2AK2, rather than its enzyme activity, to selectively modulate its downstream pathways including JNK, NF-κB, AKT and NLRP3, with an advantage of good safety profile. In EIF2AK2 gene knockdown mice, the inhibitory IL-1β, IL-6, IL-18 and TNF-α secretion of BBR was obviously attenuated, confirming an EIF2AK2-dependent anti-inflammatory efficacy. The results highlight the BBR's network mechanism on anti-inflammatory effects in which EIF2AK2 is a key target, and inhibition of EIF2AK2 dimerization has a potential to be a therapeutic strategy against inflammation-related disorders.

ABSTRACT

COVID-19 severity has been associated with alterations of the gut microbiota. However, the relationship between gut microbiome alterations and COVID-19 prognosis remains elusive. Here, we performed a genome-resolved metagenomic analysis on fecal samples collected from 300 in-hospital COVID-19 patients at time of admission. Among the 2,568 high quality metagenome-assembled genomes (HQMAGs), Redundancy Analysis identified 33 HQMAGs which showed differential distribution among mild, moderate, and severe/critical severity groups. Random Forest model based on these 33 HQMAGs classified patients from different severity groups (average AUC = 0.79). Co-abundance network analysis found that the 33 HQMAGs were organized as two competing guilds. Guild 1 harbored more genes for short-chain fatty acid biosynthesis, and fewer genes for virulence and antibiotic resistance, compared with Guild 2. Random Forest regression showed that these 33 HQMAGs at admission had the capacity to predict 8 clinical parameters, which are predictors for COVID-19 prognosis, at Day 7 in hospital. Moreover, the dominance of Guild 1 over Guild 2 at admission predicted the death/discharge outcome of the critical patients (AUC = 0.92). Random Forest models based on these 33 HQMAGs classified patients with different COVID-19 symptom severity, and differentiated COVID-19 patients from healthy subjects, non-COVID-19, and pneumonia controls in three independent datasets. Thus, this genome-based guild-level signature may facilitate early identification of hospitalized COVID-19 patients with high risk of more severe outcomes at time of admission.

ABSTRACT

Cisplatin-related ototoxicity is a critical side effect of chemotherapy and can lead to irreversible hearing loss. This study aimed to assess the potential effect of the DNA methyltransferase (DNMT) inhibitor RG108 on cisplatin-induced ototoxicity. Immunohistochemistry, apoptosis assay, and auditory brainstem response (ABR) were employed to determine the impacts of RG108 on cisplatin-induced injury in murine hair cells (HCs) and spiral ganglion neurons (SGNs). Rhodamine 123 and TMRM were utilized for mitochondrial membrane potential (MMP) assessment. Reactive oxygen species (ROS) amounts were evaluated by Cellrox green and Mitosox-red probes. Mitochondrial respiratory function evaluation was performed by determining oxygen consumption rates (OCRs). The results showed that RG108 can markedly reduce cisplatin induced damage in HCs and SGNs, and alleviate apoptotic rate by protecting mitochondrial function through preventing ROS accumulation. Furthermore, RG108 upregulated BCL-2 and downregulated APAF1, BAX, and BAD in HEI-OC1 cells, and triggered the PI3K/AKT pathway. Decreased expression of low-density lipoprotein receptor-related protein 1 (LRP1) and high methylation of the LRP1 promoter were observed after cisplatin treatment. RG108 treatment can increase LRP1 expression and decrease LRP1 promoter methylation. In conclusion, RG108 might represent a new potential agent for preventing hearing loss induced by cisplatin via activating the LRP1-PI3K/AKT pathway.

ABSTRACT

Objective@#To evaluate response to public health emergencies in colleges and universities and associated factors, and to provide references for emergency response mechanisms in colleges.@*Methods@#Using stratified random sampling, recruited 7 719 students in 6 universities in Shandong Province during April May 2020, and analyzed the data with SPSS 25.0 software.@*Results@#About 65.5% of college students reported response to public health emergencies in their college, 67.7% of the students reported that colleges have emergency plans, 53.9% of the students reported that colleges have carried out simulated emergency training, and 64.7% of the students reported that colleges have carried out health education. Evaluation from students for responding to public health emergencies in colleges and universities scored(24.15±4.59), of which measures and policies related to epidemic control were highest(4.05±0.81), while mental health education or counselling services were lowest (3.95±0.85). Gender, grade, major, type of household registration, emergency response measures, emergency response plan, emergency simulation training, health education were associated with the evaluation of the measures by college students(t/F=5.04,43.83,79.92,-3.40,26.86,34.83,29.35,35.01,P<0.01).@*Conclusion@#While recognizing the overall performance on public health responses in universities, college students believe certain limitations still exist; colleges and universities should continuously improve the emergency prevention and control system of public health emergencies in response to the effectiveness and deficiencies in the prevention and control process.

ABSTRACT

Sixty general practitioners of in-service training undertaking rotation in gastroenterology department of Qingdao Municipal Hospital from July 2017 to July 2019 were randomized assigned in trial group ( n=30) and control group ( n=30). The problem-oriented mode was applied in trial group and conventional mode was applied in control group for teaching of two typical digestive diseases (upper gastrointestinal bleeding and acute pancreatitis). The formative evaluation and questionnaire survey were used to compare the teaching effects and the results of evaluation were compared with χ 2 test by SPSS 17.0 between two groups. The excellent and good rates of evaluation for the clinical psychological quality, clinical reasoning ability, doctor-patient communication ability and practice-based learning and improvement ability in trial group were significantly higher than those in the control group(χ2=7.38, P=0.03; χ2=12.96, P<0.01; χ2=23.33, P<0.01; χ2=16.14, P<0.01). Questionnaire survey showed more satisfaction towards teaching method in trial group was higher than that in control group(χ2=12.86, P<0.01); and the clinical reasoning ability, learning initiative and self-confidence in trial group were improved more markedly than those in control group(χ2=8.26, P=0.02; χ2=19.48, P<0.01; χ2=21.46, P<0.01). The problem-oriented clinical thinking teaching model demonstrates good effects on clinical comprehensive ability for general practitioners of in-service training during gastroenterology rotation, which is worth further promotion.

ABSTRACT

Objective:To investigate the efficacy of different doses of apatinib combined with chemotherapy in the treatment of advanced gastric cancer and its effect on prognosis.Methods:Sixty-nine patients with advanced gastric cancer who received treatment in Lishui City People's Hospital from January 2015 to February 2019 were retrospectively analyzed. All patients received apatinib combined with teggio chemotherapy. These patients were divided into groups A, B and C according to the different dosages of apatinib used: 250 mg/d ( n = 21, group A), 500 mg/d ( n = 23, group B) and 850 mg/d ( n = 23, group C). The control rate of gastric cancer, serum levels of carcinoembryonic antigen, carbohydrate antigen 19-9 (CA19-9), carbohydrate antigen 72-4 (CA72-4), toxic and side effects, and survival within 1 year after surgery were determined among the three groups. Results:By the end of follow-up, one patient from group A was lost, one patient from group B interrupted medication because of personal reasons, and two patients from group C withdrew from the treatment due to serious discomfort caused by drugs. After treatment, disease control rate in group C was significantly higher than that in group A [91.30% (21/23) vs. 60.00% (12/20), χ2 = 6.484, P < 0.05]. Serum levels of carcinoembryonic, CA19-9 and CA72-4 in group C were (27.51 ± 2.21) μg/L, (101.46 ± 8.02) g/L, (46.34 ± 6.15) U/mL, respectively, which were significantly lower than those in group B [(29.33 ± 2.17) μg/L, (106.67 ± 8.10) g/L, (50.67 ± 6.20) U/mL, t = 2.786, 2.168, 2.352, all P < 0.05]. Serum levels of carcinoembryonic, CA19-9 and CA72-4 in group B were significantly lower than those in group A [(31.63 ± 2.92) μg/L, (112.12 ± 8.38) g/L, (55.12 ± 6.48) U/mL, t = 2.915, 2.142, 2.274, all P < 0.05]. The incidences of hand foot syndrome and gastrointestinal discomfort in group C were (34.78% (8/23) and (39.13% (9/23), respectively, which were significantly higher than those in group A [15.00% (3/23) and 25.00% (5/20), χ2 = 5.734, 4.769, both P < 0.05]. After 1-year follow-up,1-year survival rate in group C was significantly higher than that in group A [39.13% (9/23) vs. 10.00% (2/20), log-Rank χ2 = 6.600, P < 0.05]. Conclusion:High-dose apatinib combined with chemotherapy in the treatment of advanced gastric cancer has a high disease control rate and a high 1-year survival rate, but it has serious adverse drug reactions.

ABSTRACT

Background: Arm choice for peripherally inserted central catheter (PICC) insertion is often determined by PICC nurses. Objectives: To explore the impact of arm choice (dominant or non-dominant) on the rate of catheter-related complications and degree of comfort in patients with PICCs. Design: A cross-sectional study. Methods: We used questionnaires and scales to collect 255 patients' general information, catheter-related complications and comfort degree. They were divided into two groups (PICC inserted in the dominant or non-dominant arm). Results: There were significant differences in the rate of PICC occlusion and insertion site bleeding (χ 2 = 9.829, P < 0.05; χ 2 = 6.502, P < 0.05), both were lower in the non-dominant group than in the dominant group. Patients in the non-dominant group had a higher degree of comfort (Z = -10.166, P < 0.01). Conclusion: Inserting into non-dominant arms is related to the lower rate of catheter-related complications and higher degree of comfort for patients. Impact statement: These findings provide practical evidence for nurses to make a better choice.

OBJECTIVE@#To discuss the advantages and technical limitations of various molecular genetic techniques in the diagnosis of two infants featuring all-round developmental retardation.@*METHODS@#The two patients were initially screened by using chromosomal microarray analysis (CMA). For patient 1, his parents were also subjected to CMA analysis, and the data was analyzed by using ChAS and UPD-tool software. For patient 2, methylation-specific PCR (MS-PCR) was carried out.@*RESULTS@#Patient 1 was diagnosed with maternal uniparental disomy (UPD) type Prader-Willi syndrome (PWS) by CMA and UPD-tool family analysis. His chromosomes 15 were of maternal UPD with homology/heterology. Patient 2 was diagnosed with deletion type PWS by combined CMA and MS-PCR.@*CONCLUSION@#Correct selection of laboratory methods based on the advantages and limitations of various molecular techniques can help with diagnosis of genomic imprinting disorders and enable better treatment and prognosis through early intervention.

ABSTRACT

OBJECTIVE@#To diagnose a 46,XN,del(11)(q14q22) fetus by non-invasive prenatal testing (NIPT), karyotype analysis and whole genome sequencing (WGS).@*METHODS@#Peripheral blood sample of the gravida was taken for NIPT screening. Blood samples of the gravida, her husband, and umbilical cord blood were also taken for chromosome karyotyping and whole genome sequencing (WGS).@*RESULTS@#NIPT screening indicated the fetus has carried partial deletion of chromosome 11, while no chromosomal abnormality was found with the cord blood sample due to the low resolution of G-banding analysis. WGS analysis of the cord blood indicated 46,XN,del(11q14.3q22.1). seq[GRCh37/hg19] (90 623 404-97 469 319)×1, 6.85 Mb. The karyotype of the fetus was eventually determined as 46,XN,del(11)(q14q22). Karyotyping analysis suggested that the gravida and her husband were 46,XX,del(11)(q14q22)[8]/46,XX[92] and 46,XY, respectively. However, neither of them was found to harbor the del(11)(q14q22) by WGS.@*CONCLUSION@#The abnormal karyotype of the fetus has derived from its mother's low percentage mosaicism. Combined NIPT, karyotyping analysis and WGS can detect chromosomal disorders with accuracy.

ABSTRACT

Hepatocellular carcinoma (HCC) is one of the five most common malignant tumors. According to the latest statistics of the World Health Organization (WHO), the incident and mortality rates of HCC ranks the eighth and third in the world, respectively, which severely affect people's health. Exosomes are extracellular vesicles with a bilayer of phospholipids, which carry active substances such as proteins and nucleic acids derived from their mother cells. These exosomes greatly facilitate the exchange of substances and information between cells, and coordinate physiological and pathological processes in the body. In recent years, a large number of studies have shown that exosomal proteins play important roles in the tumorigenesis, development, diagnosis and treatment of HCC. Here we review the composition and functions of exosomes and the role of exosomal proteins in HCC.

ABSTRACT

Objective To investigate the significance of "merge" model versus "traditional" model in rotation training for professional clinical postgraduate students in department of gastroenterology. Methods The professional clinical postgraduate students who underwent rotation training in Qingdao Municipal Hospital from July 2012 to July 2016 were enrolled as subjects and were trained with the"merge" model and the "traditional" model, respectively. The two groups of subjects were compared in terms of clinical knowledge , skills , and core competencies . Results The postgraduates trained by the"merge" model had a significantly higher total score of clinical examinations than those trained by the"traditional" model [(92.60±2.52) vs. (83.80±3.14), t=10.93, P<0.01], while there were no significant differences in professional quality and doctor-patient communication between the two groups. Compared with those trained with the"traditional"model, the postgraduates trained by the"merge"model had significantly better understanding of clinical knowledge and clinical thinking ability, diagnosis and treatment skills, and psychological quality (χ2=27.00, 10.23, and 12.21, all P<0.01);however, there was no significant difference in clinical research ability between the two groups (χ2=1.39, P=0.24). Conclusion The model of professional clinical postgraduate training combined with standardized training of residents is superior to the traditional training model, and compared with the "traditional" model, the "merge" model is more beneficial to the training of comprehensive clinical practice abilities and thus holds promise for further application.

Objective@#To explore the relationship between internalized stigma and insight, self-esteem in patients with schizophrenia.@*Methods@#A total of 144 schizophrenic patients were investigated by the general information questionnaire, insight and treatment attitude questionnaire (ITAQ), internalized stigma of mental illness scale-Chinese version (ISMI-C) and the self-esteem scale (SES). Pearson correlative analysis and Bootstrap program mediation effect test were used to data analysis.@*Results@#There was a significantly positively correlation between insight score(11.24±4.08) and internalized stigma score(2.18±0.65) of 144 schizophrenic patients(r=0.236, P<0.05). The Self-esteem score(27.57±3.76) was negatively correlated with insight score(r=-0.177, P<0.05) and internalized stigma score(r=-0.661, P<0.05). The mediation effect test analysis showed that the internalized stigma played a full mediating role in the relationship between insight and self-esteem, and the effect size was -0.143.@*Conclusion@#Insight can indirectly affect self-esteem via internalized stigma in patients with schizophrenia.Taking measures to decrease inpatients' internalized stigma level and improve their illness awareness should be paid attention in clinical practice.

ABSTRACT

Objective To investigate the value of standardized iodine concentration and energy spectrum curve of dual-source CT in diagnosis of metastatic lymph nodes of gastric cancer.Methods The clinical data of 60 patients diagnosed as gastric cancer were collected in our hospital.All patients underwent dual-source CT routine abdominal scanning and dual-phase energy spectrum enhancement.According to the pathologic results,the lymph nodes were divided into metastatic and non-metastatic groups.Then the enhanced degree,dual-phase normalized iodine concentration (NIC)were measured and compared between metastatic lymph nodes and non-metastatic lymph nodes.The slope of the energy spectrum curve of metastatic lymph nodes and non-metastatic lymph nodes were calculated by dual-energy software.Results When the enhanced degree was higher than 80 HU,the sensitivity (82.4%)and specificity (78.6%)were higher. The artery-phase NIC values of metastatic lymph node and non-metastatic lymph node were(0.546±0.086)and(0.144±0.077),respectively. The difference was statistically significant (P<0.05).The venous-phase NIC values of metastatic lymph node and non-metastatic lymph nodes were (0.479±0.013)and (0.307±0.082),respectively.The difference was statistically significant (P<0.05).The dual-phase energy spectrum curves of metastatic and non-metastatic lymph nodes were all descending patterns.The difference was obvious at 40-80 keV and minimal at 80-140 keV.The sensitivity of diagnosing metastatic lymph nodes was 83.5%,and the specificity was 85.6%when the arterial curve slope was 6.87.The sensitivity of diagnosing metastatic lymph nodes was 63.6% and the specificity was 96.7%when the venous curve slope was 6.67.Conclusion The multiple parameters provided by dual-source CT dual-energy imaging are helpful to the qualitative and quantitative diagnosis of metastatic lymph nodes in gastric cancer.

ABSTRACT

Objective To investigate the change rules of the lumbar vertebral bone marrow fat fraction(FF)and T2* values at different bone mineral density(BMD)groups,and their relevances and the applications on the diagnosis of the primary osteoporosis. Methods The patients who underwent the dual energy X-ray absorptiometry(DXA)and the routine lumbar MRI scan,mDIXON-Quant scan were collected,and the BMD,T-score,FF and T2* values of L1-L4 were measured.According to the T-score,the vertebral bodies were divided into the normal group,the osteopenia group and the osteoporosis group.The differences of the FF,T2* values and BMD between each group were analyzed by one-way analysis of variance,and their correlations with BMD were analyzed by Pearson correlation,the diagnostic effects of them were evaluated by receiver operating characteristic curve(ROC curve).Results The differences of the FF ,T2 * values and BMD among the three groups were statistically significant (P < 0 .05) .The correlations of the FF and T2 * values with BMD were negative (r = - 0 .628 ,P < 0 .05 and r = - 0 .468 ,P < 0 .05 ,respectively) .The area under curve (AUC) of the FF and T2 * values were 0 .82 ± 0 .03 ,0 .79 ± 0 .03 ,respectively .Conclusion mDIXON-Quant which can accuratly quantify fat content could evaluate the lumbar vertebral bone marrow fat content ,reflect the changes of the bone quality ,provide valuable information for the diagnosis of osteoporosis ,and is helpful to predict the risks of the lumbar fragility fracture.
